Description
STATEMENT OF WORK Courier Services Robley Rex VA Medical Center and Community Based Outpatient Clinics (CBOC) REQUIREMENT: The contractor shall furnish all personnel, equipment, and supplies necessary to provide courier services Pharmacy, Laboratory, for the Robley Rex VA Medical Center and eight (8) Community Based Outpatient Clinics assigned to the Robley Rex VA Medical Center. BACKGROUND: The Robley Rex VA Medical Center located in Louisville, KY, currently includes eight (8) Community-Based Outpatient Clinics (CBOCs) that are geographically dispersed in the States of Kentucky and Indiana to provide Health Care services for our Veteran population. The contractor shall provide courier services to transport lab specimens, medical records, inter-office mail, office supplies, pharmaceutical, and medical supplies to and from the CBOCs and the Medical Center and other designated points which can include long-term care facilities or residences. PERIOD OF PERFORMANCE: Will begin on the date of Award and continue to the end of the fiscal year with the Government having the option to renew four (4) 1-year options periods. Base Year: 01 February 2022 30 September 2022 1st Option Period 01 October 2022 30 September 2023 2nd Option Period 01 October 2023 30 September 2024 3rd Option Period 01 October 2024 30 September 2025 4th Option Period 01 October 2025 30 September 2026 PLACE OF PERFORMANCE: Robley Rex VA Medical Center located in Louisville, KY, and eight (8) Community-Based Outpatient Clinics (CBOCs) assigned to the facility as indicated in the chart below Robley Rex VA Medical Center, 800 Zorn Ave Louisville KY 40206 Carrollton CBOC 1911 US Highway 227 Carrollton, Ky. 41008 Ft. Louisville, KY 40299 PERFORMANCE REQUIREMENTS: GENERAL: The contractor shall provide pick-up and delivery services Monday through Friday, excluding federal holidays and weekends and on an as-needed basis with the following schedule. As-needed is often determined at the start of the workday. Any changes will be coordinated through the Contracting Officer s Representative (COR) and authorized by the Contracting Officer. Carrollton CBOC- Once (1) daily as needed at 2:00 p.m. Ft. Knox CBOC- two (2) times daily as needed at 10:30 a.m. and 3:00 p.m. Grayson CBOC- two (2) times daily as needed at 10:30 a.m. and 3:00 p.m. (this CBOC is on CST) New Albany CBOC- two (2) times daily as needed at 11:30 a.m. and 3:00 p.m. Newburg CBOC- two (2) times daily as needed at 11:30 a.m. and 3:15 p.m. Scottsburg CBOC- two (2) times daily as needed at 10:30 a.m. and 2:00 p.m. Stony Brook CBOC- two (2) times daily as needed at 10:30 a.m. and 3:00 p.m. Greenwood CBOC- two (2) times daily as needed at 12:00 noon and 3:15pm As needed defined as the need of the COR, COR will coordinate with the courier when pick-up/Delivery are required. The contractor shall deliver medical records and inter-office mail as required. Office supplies and medical supplies shall be delivered on an as-needed basis. Weight capacity shall not exceed 50 lbs. Pharmaceutical supplies destination is varied and can change frequently due to the nature of the service, i.e., surgical preparatory antibiotics, thus do not follow a standard schedule. A majority of these are within the Louisville city limits. Laboratory specimens may require addition destinations depending on the nature of the sample. A majority of these are within Louisville city limits. Drop off/pickup locations at the Robley Rex VAMC include: Laboratory, Room C-306 3rd Floor, Bldg. 1; Mailroom, Room A017 Basement Building 1; Medical Records, 2nd Floor Building 1; Warehouse Room A023 Basement Building 1; and Pharmacy Svc, Room A033 Basement Bldg 1. Radiology, Room A203, 2nd floor Bldg-1. After hours pick-up (after 1630) and delivery will be at the Administrative Officer of the Day (AOD) office, Bldg 1. Robley Rex VAMC Contact Personnel: Transportation; Troy Stephens, COR: (502)287-5418 (502)643-0847 (812)800-8042 Alicia Scott: (502)287-5418 (502)592-5181 Robley Rex VAMC reserves the right to adjust the pickup hours and locations as needed. The COR will notify the contractor if there is a change in the pickup or delivery schedule. Contractor is responsible for training transport personnel in appropriate safety and transporting procedures suitable to biological specimen types and distances transported. Contractor shall transport samples in such a manner as to ensure the integrity of the specimens. OSHA approved laboratory containers/coolers with refrigerant packs will be provided by the Contractor. Coolers should be approximately 12"" high 12"" wide 9"" deep. NO DRY ICE. All containers/coolers will be labeled with a biohazard label. All containers will be able to be closed securely. Contractor shall be responsible for complying with all regulations that define appropriate transportation of specimens and shall supply in writing that all drivers have been adequately trained on an annual basis. The contractor s employees/couriers transporting test specimen shall be trained in Universal Precautions in the handling of biohazard materials (specimens). Training must include utilizing clinical procedures in case of biohazard spills and accidents. The contractor must furnish a record of all required training to the COR. QUALITY CONTROL: The contractor shall develop and maintain a quality control program that ensures courier services are performed in accordance with these specifications. The contractor shall develop and implement procedures to track, identify, prevent, and remedy and ensure non-recurrence of defective services. The contractor shall provide a method to accept and resolve customer complaints and notify the customer of the resolution. The contractor shall immediately notify the COR upon receipt of a customer complaint in order that joint validation may be accomplished. The contractor is responsible for quality control and specification compliance. CONTRACTOR COURIERS/DRIVERS: The contractor shall be responsible for furnishing all vehicles, personnel, equipment and supplies, vehicle fuel, uniforms, name tags, and/or badges for performance of services under this contract. As a Non-Personal Services contract, contractor personnel are not to be considered VA Employees for any purpose. Contractor retains all control over personnel and responsibility for direct supervision. Contractor will ensure that all drivers utilized under this contract will be licensed in accordance with the laws of the Commonwealth of Kentucky and the State of Indiana, for the transportation purposes for which this contract is intended. Contractor personnel shall always wear visible identification, while on the premises of any Federal property. A record of each driver as to character and physical capabilities for performing as a courier must be maintained and made available to the Department of Veterans Affairs Medical Center upon request for inspection. The contractor personnel performing services to the Government shall always conduct themselves in a professional manner, present a neat appearance and be easily recognized as a contract employee by having the company name, and employee name affixed to the clothing or uniform. Employee identifications shall be worn in clear view above the waist. CONTRACTOR VEHICLES: Contractor will maintain registration, licensing and insurance as required by the Commonwealth of Kentucky and the State of Indiana on all vehicles utilized under this contract, for the transportation purposes for which this contract is intended. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Vehicle size has no requirement. Contractor shall ensure vehicle utilized for pickup/delivery has the ability to securely carry the load associated with the pickup/delivery request.
