Description
Change Order Notice Notice of Intent to Extend a Sole Source Requirement Notice of Intent to extend the period of performance of a Sole Source Requirement. This is a supplemental agreement within scope of contract 36C24221C0177. The Department of Veterans Affairs, VA Healthcare System, New York Harbor Healthcare System (Brooklyn Campus) VISN 02 Contracting Activity intends to award a sole source modification contract under the authority of FAR Part 52.243-1 Changes-fixed-price. The associated NAICS code for this requirement is 621512; the size standard is $15 million. The awardee will be Insight Health Corp, 26250 Enterprise Ct Ste 100, Lake Forest, CA 92630. Description of equipment requirements: The requirement is for a rental Philips Mobile MRI (for full description see attached draft statement of work). A firm fixed price contract for an additional six (6) months is anticipated and shall be awarded. It is the Government's belief that only the proposed candidate possesses the required capabilities to successfully meet this requirement. It is the Government's intention to solicit only the listed source. This notice of intent is not a request for competitive proposals. It is not a solicitation or request for offers. No solicitation package is available. Any response to this notice must show clear and convincing evidence that competition would be advantageous to the Government. Responses must be received by 1:00 pm EST on Wednesday, December 22, 2021. STATEMENT OF WORK MRI Trailer Lease at the VA New York Harbor Healthcare System Location: VA NY Harbor Healthcare System, 800 Poly Pl, Brooklyn, NY 11209. Period of Performance: 01/01/2022-06/30/2022 The VA NY Harbor Healthcare System is seeking to extend its current rental of a Philips Mobile MRI rental while the permanent MRI suite is being built. The Special Notice current contract is due to expire on 12/31/2021. We are seeking a 6-month extension option year contract from 01/01/2022 to 06/30/2022. The Philips Mobile MRI rental must be capable of supporting the ongoing clinical programs at the Brooklyn campus, including Cardiology and Cardiac Surgery, Neurology and Neurosurgery, Orthopedics, Vascular Surgery, and Oncology. The vendor must also have the capability of providing a mobile MRI unit to be on site 7 days per week. The Vendor must provide specific services as follows: ? Mobile Philips Ingeina MRI at the VA Brooklyn Campus. ? Mobile MRI unit on site 7 days per week ? The MRI must meet the specifications (MRI System Requirements Below) ? VA MRI technologists are already trained in the Philips Ingenia MRI that will require minimum applications training. ? The Vendor is responsible for transporting the unit to and from the location. ? The Vendor is responsible for the maintenance of the unit, including all preventive maintenance, and all parts and labor. ? The Vendor is responsible for the removal of the mobile unit as well as the existing retired Siemens Symphony MRI. ? System must be able to transfer image and data to multiple modalities including PACs, VISTA Imaging, and printers. OEM Engineer will establish network and printer connectivity at the site in coordination with local IT staff. Verification of connectivity must be made prior to the start of patient procedures. Special Notice ? System scan protocols must be verified and validated by VA reading radiologists. ? Biomed will remove system Hard Drive at contract end and destruct/dispose according to VA policy. ? The system will be installed and operated in compliance with all applicable VA security and privacy standards, practices, and policies. MRI Trailer 1. Trailer size is 704�� long x 241��wide. Responsibility of VA Facility: VA Medical Center is responsible for the following: ? VA will provide electrical power, and Emergency Generator Backup, 480 volt AC 3 phase 125 KVA fused at 150 amps Maximum voltage variation is +- 5% from nominal steady state, Voltage surges must not exceed +5%, 5 wire Russellstoll receptacle DF 2504 FRAB0, waterproof. ? VA will provide 3 data lines and 3 telephone lines and connections to the VA Police/Fire Alarm system and the Code Alarm system. Contractor to connect power, data, phone and alarm systems to the Mobile MRI trailer. ? VA will provide network access for MRI remote diagnosis connection between trailer and service organization. ? VA will provide water supply with a flow rate of 5 gallons per minute (45- 60PSA), maximum temp. of 70 degrees F. ? VA will provide means of sanitary wastewater drainage from the system ? The services of MRI technologists/technicians to perform all scans. Special Notice ? The VA will be responsible for cleaning and infection control measures. ? VA will be responsible for all professional staff and MRI procedures, safety, and protocols. ? VA Radiologist(s) will provide guidance for protocols and assume responsibility for medical care of patients. Radiologist(s) will also review studies for appropriateness according to established Department of Radiology protocols. ? VA will be responsible for scheduling of patients to maximize patient throughput and will provide clerical support for scheduling, patient registration and other administrative responsibilities related to the MRI Schedule. ? Provision of staff and equipment for emergency care and all CODE procedures. ? Provision of medical supplies necessary to perform the procedures including gadolinium, injector kits, laundry and other necessary medical supplies and all consumable supplies. ? Provision of film media (film, CD�s) specific to each the needs of each site, where required. ? Transportation of patients to/from reception area and /or medical ward, to the MRI trailer. MRI System Requirements ? Minimum requirements for this system are a state of the art 1.5T short bore magnet ? The MRI must have the same capabilities of the standard configuration for a Philips Ingenia 1.5 T MRI System or better with TIM technology ? ? Gradients : Omega 33mT/m Special Notice ? Slew rate: 120 mT/m/ms ? Software Level: R5 ? System must have capabilities for quantitative cardiac, neuro, and vascular post-processing. Cardiac post-processing includes capability for: volumes, flow, velocities, function; neuro cases: perfusion/diffusion; Vascular , 3D/MIP, and stenosis analysis. ? Minimum of 425 lb load table limit ? Gradient noise level of 99 db or below without reducing performance ? Equipment will be capable of sending images to a minimum of 3 destinations (DVD, PAC�s, Hospital RIS System, etc) ? Power injector ? Patient physiologic monitor ?
