Page 2 of 2 Notice Type: Special Notice of Intent to Sole Source This is a SPECIAL NOTICE ANNOUNCEMENT ONLY. The Department of Veteran Affairs, VISN 2 Network Contracting Office intends to award a sole source contract for the Syracuse VA Medical Center within Upstate NY Healthcare Network shown below. This proposed contract action is for a service in which the Government intends to award a Base Period plus four (4) Option Periods, firm fixed-price contract with only one source; University Pathologists Laboratory, 224 Harrison Street, Suite 600, Syracuse, New York 13202 under the authority Federal Acquisition Regulation (FAR) Part 6.302-1 (a)(2), only one responsible source. Places of Performance: Department of Veterans Affairs VISN 2 VA Medical Centers Syracuse VAMC 800 Irving Avenue, Syracuse, NY 13210-1276 Purpose and Objectives: The intent of this Special Notice is to identify potential offerors capable of providing analysis, monitoring, and reporting findings of laboratory and pathology tests for VA Upstate NY Healthcare System at Syracuse VAMC. Contractor shall provide STAT laboratory testing on Veteran patient specimens seven days a week, including holidays, 24 hours a day. Contractor laboratory requires mandatory licensing and accreditation possessing a NYS Department of Health Clinical Laboratory Permit, Clinical Laboratory Improvement Amendments (CLIA) License and College of American Pathologists (CAP) Accreditation. General Requirements The contractor must comply with the following general requirements: Contractor shall utilize board certified pathologists and accredited technologists to perform services for this contract. The Contractor shall hold the following certifications and provide documentation of the same with the proposed quote: NYS Department of Health Clinical Laboratory Permit CLIA License CAP Accreditation and/or Joint Commission Contractor shall provide services to include sample pick-up, transport, processing, analysis and diagnostic reporting of surgical and clinical specimens. 1. The Contractor must pick up specimen within 30 minutes of the request for service. Contractor shall perform specimen processing and diagnosis utilizing electron microscopy, immunofluorescence, immuno-histochemical, molecular, genetic and any state of the art studies as needed for a STAT diagnosis. Contractor must complete testing and call the VA with results, faxing completed written reports to Syracuse VA Medical Center within 5 working days. The Contractor testing services shall be available 24 hours a day, 365 days a year (including holidays). Contractor shall provide requisition forms pre-printed with Syracuse VA Medical Center client information for processing and billing purposes. Upon the request of an authorized VA employee, Contractor shall call back the results of critical tests by telephone as soon as results are known. The information shall be documented in the follow-up printed report including the date and time of the call, and the identification of the caller and recipient of the call. 1. Critical results require immediate notification to an authorized provider or staff member to facilitate prompt patient intervention. Contractor shall provide Syracuse VAMC telephone number(s) and contact person(s) available to the VA for specimen problem inquiries, problem solving and status of pending tests at all times including weekends and holidays. Response: Responses to this Special Notice should be able to demonstrate the firm's ability, capability, and responsibility to deliver the service listed above. The purpose of this Special Notice announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. No funds have been authorized, appropriated, or received for this effort. Responses should include the following information: Business name, address, business size and Socioeconomic Category (HUBZone, 8(a), Service Disabled Veteran, Small Business, Large Business, etc.), and point of contact to include e-mail address and telephone number. All offerors must be registered in the System for Award Management (www.sam.gov) and Representations and Certifications must be completed prior to any future award. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to provide the full maintenance and repair service on equipment listed. Determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Important information: The Government is not obligated to, nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 621511 ($35 Million). Any offeror capable of providing this service shall notify the Contracting Officer by email within 10 calendar days from the date of the publication of this Special Notice.
