Description
Question & Answer (Q&A) For RFQ 36C25622Q0130 Sterile Processing Tech Services (MEDVAMC) Questions Answer Since file sizes are limited to 5mb, will a copy of the offeror s representation to FAR 52.212-3 (b) suffice for the requirement to submit a copy of the offeror s Annual Representations and Certifications? A copy of the offeror s representation to FAR 52.212-3 (b) will suffice for the requirement to submit a copy of the offeror s Annual Representations and Certifications Attachment D.2 is labeled as a Past Performance Questionnaire, but on page 51 of the solicitation, it is referred to as the Past Performance Worksheet. Is the offeror intended to fill out this worksheet, or should it be completed by the offeror s reference? No, Past Performance Questionnaire and Past Performance Worksheet is one in same. Any and all references to a Past Performance Worksheet should be looked upon as Performance Questionnaire Attachment D.2. How often will the Sterile Processing Technicians be expected to be on-call? Depending on workload and demand of MEDVAMC s schedule. See PWS section 1.4 If there is an incumbent, could you please provide the current / previous contract number? Information not available. Requested service has not been performed at MEDVAMC. PWS Section 4.2.1: May the government clarify what should be submitted by the offeror regarding documentation proving that Stryker s proprietary training can be obtained? Would this be a document from the offeror stating that candidates will be required to complete this training to meet PWS qualifications? PWS Section 4.2.1 clarified: Offerors shall be required to provide certification that training has been completed or certification will be completed within 90 days after contract award. If training is not completed prior to contract award, Offerors shall provide evidence with their quotations that onsite training has been scheduled and approved by Stryker to be completed within 90 days after contract award.
