Description
Project: Site Prep for Hazardous Drugs Mixing IV Cleanroom Suit Trailer I. Scope of Work The Montana VA Health Care System requires a site preparation contract that will allow for immediate use of Germfree s Hazardous Drugs Mixing IV Cleanroom Suit Trailer upon delivery. The trailer will be located at the Fort Harrison VA Medical Center, 3687 Veterans Drive, Fort Harrison, MT 59602. Placement will be at the Administration Building 141. Intent of this project is to provide site preparation to install the Hazardous Drugs Mixing IV Cleanroom Suit Trailer on campus. The trailer will be used by the Fort Harrison VA Medical Center pharmacy staff due to the event that sterile compounding services are temporarily suspended within the facility and interruptions in patient care must be avoided. As part of this site prep, the Contractor shall coordinate site-preparations. The trailer will require connections for electrical, potable water, and data. Complete fire suppression system is not anticipated. The Contractor shall provide all necessary labor, materials, tools, and equipment, to properly execute this site preparation project. The installation shall meet all NFPA, VA, state, and local codes and ordinances. As part of this project, drawings for the trailer are provided by Germfree. These drawings are for reference and informational use only. They shall not be used to exclusively define existing conditions. It shall be the responsibility of the Contractor to perform site investigation to identify site conditions and quantities to ensure integrity throughout the project prior to submitting an offer. Contractor to phase and coordinate all site preparation accordingly to ensure trailer is equipped for immediate occupancy and use. II. General Work includes providing site preparation project for the installation of Germfree s Hazardous Drugs Mixing IV Cleanroom Suit Trailer. The following shall be performed, not necessarily in this order, as part of the trailer installation (see As-Build drawing package). Hazardous Drugs Mixing IV Cleanroom Suit Trailer Site Set-Up A. Germfree provides the delivery and placement of the trailer to Fort Harrison VA Medical Center at the desired location determined by the VA. Orientation of the trailer as per Pictures Stie Plan_Pharmacy Trailer location. B. Fresh water supply building connection to be provided by the VA. Contractor responsible for installation of rigid pipe directly from the building connection to the trailer connection. Due to freezing temperature concerns, the rigid pipe requires a heated solution to be protected from the elements. C. Electrical service, end-to-end, connection shall be the Contractor s responsibility. Electrical to be run from the main basement electrical Room B-21, inside Building 141. Existing penetration exists through the exterior wall at Room B-23A and is to be utilized for electrical service. Once service is outside, use canopy structure for conduit run to nearest connection point on the trailer. A disconnect shall also be required near the connection point to the trailer. D. Data and communication shall be tied into the trailer by the Contractor. Data utility must run at least 2 PCs and label printer in addition to the digital video cameras and digital video recorder, included with the trailer. E. Install card reader (PIV badge compliant) and any associated hardware on exterior of the trailer entrance door for staff accessibility and security. Tie into data connection. The entrance door shall also have a dead-bolt lock from outside to the office to secure the trailer when not in use. F. Trailer entry ramp to be provided as part of this effort. Ramp, landing(s) and handrailing shall be provided as part of a complete system. The trailer entrance door is approximately four (4) feet off the ground. Ensure provided ramp conforms to anti-slip requirements. G. Install hard barrier enclosure over trailer landing and ramp to mitigate the risk of meds being exposed to weather. Enclosure will not be insulated or heated. Ensure adequate lighting is provided within the enclosure. H. Install hard barrier enclosure along canopy length to be tied into the ramp barrier for additional protection from the weather. Enclosure will not be insulated or heated. I. All hard barriers installed as part of this project are to be constructed to be rigid and provide protection from wind and weather. Barrier framework to be covered in a weather resistant sheeting to keep moisture out while providing an aesthetic look. J. Contractor to take all appropriate measures to ensure trailer undercarriage is protected from freezing from the cold weather environment. Installation of trailer skirting or similar insulating material recommended. K. Vehicle rated security barricades shall be required for protection of the trailer. Barricades to include fencing that can be installed as part of a system onto the barricades for pedestrian access control. L. Pedestrian barriers shall be required to mitigate foot traffic around undesired areas of the trailer where utility connections will be run. The above itemized list constitutes a basic outline of work to be performed and in no way summarizes all details for the construction of this project. It is strongly recommended that a detailed inspection of the premises and Germfree s drawings be initiated, by the Contractor, to determine actual needs and conditions for the construction of this project. Contractor shall be responsible for verifying site conditions, determine the type of equipment needed, the number of personnel required and the level of effort to complete the job in the time framed specified in section IV, Period of Performance, below. III. Site Visit The Contractor and subcontractors are strongly encouraged to attend an onsite visit prior to submitting an offer. The Contractor should make an onsite visit prior to delivery to coordinate with Fort Harrison VA Medical Center Engineering for any pre- delivery issues (power, plumbing, etc.). The Contractor shall provide site preparation, delivery, and field installation including seismic strapping or bracing required by VA Seismic Building Code, connection of main power supply, connection of network/security systems, connection of grey water (if required) and connection of fresh water. Site preparation will not require Fort Harrison VA Medical Center to pour a level pad or require a crane to install at the location scouted at the facility. VI. Bid Schedule Base Bid The Contractor shall provide all necessary labor, materials, tools, equipment, and services to complete all work as stated in this scope of work. VII. Privacy Statement The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. A contractor/subcontractor shall request logical (technical) or physical access to VA information and VA information systems for their employees, subcontractors, and affiliates only to the extent necessary to perform the services specified in the contract, agreement, or task order. All contractors, subcontractors, and third-party servicers and associates working with VA information are subject to the same investigative requirements as those of VA appointees or employees who have access to the same types of information. The level and process of background security investigations for contractors must be in accordance with VA Directive and Handbook 0710, Personnel Suitability and Security Program. The Office for Operations, Security, and Preparedness is responsible for these policies and procedures. Contract personnel who require access to national security programs must have a valid security clearance. National Industrial Security Program (NISP) was established by Executive Order 12829 to ensure that cleared U.S. defense industry contract personnel safeguard the classified information in their possession while performing work on contracts, programs, bids, or research and development efforts. The Department of Veterans Affairs does not have a Memorandum of Agreement with Defense Security Service (DSS). Verification of a Security Clearance must be processed through the Special Security Officer located in the Planning and National Security Service within the Office of Operations, Security, and Preparedness. Custom software development and outsourced operations must be located in the U.S. to the maximum extent practical. If such services are proposed to be performed abroad and are not disallowed by other VA policy or mandates, the contractor/subcontractor must state where all non-U.S. services are provided and detail a security plan, deemed to be acceptable by VA, specifically to address mitigation of the resulting problems of communication, control, data protection, and so forth. Location within the U.S. may be an evaluation factor. The contractor or subcontractor must notify the Contracting Officer immediately when an employee working on a VA system or with access to VA information is reassigned or leaves the contractor or subcontractor s employ. The Contracting Officer must also be notified immediately by the contractor or subcontractor prior to an unfriendly termination. VIII. Safety The Dept. of Veterans Affairs (VA) requires strict adherence to the Code of Federal Regulations 29, Part 1926. This includes the requirement that the contractor have a Competent Person on the job at all times (must have a 30-hour OSHA card), and that all contractor employees have received a minimum of ten (10) hours of OSHA construction safety training. Documentation of these requirements shall be submitted and approved prior to performing the work. Contractor must submit a company Site Specific Safety Plan (SSSP) that must be approved by VA Safety prior to work beginning. This plan shall detail fire, construction, and health-related safety measures, in addition to listing Training Records for all employees for the previous year. A company standard or boilerplate safety plan will not be acceptable plan should contain a detailed hazard analysis for each element of construction to be performed such as an Activity Hazard Analysis (AHA) or Job Hazard Analysis (JHA). This entire project shall conform to 100% of all VA standards and specifications, OSHA, AIA Construction Guidelines, Joint Commission Environment of Care Standards, and applicable building codes. The Contractor shall be responsible for the health and safety of their own employees. The VA will inform the Contractor of any hazards such as designated permit confined spaces, electrical hazards, etc. which may affect Contractor employees. The Contractor must take precautions appropriate for the safety of their employees as well as provide equipment necessary for these actions. Contractor employees will notify the VA Representative before entering a designated permit required confined space, so precautions can be taken in advance of the work. The Contractor shall hold harmless and indemnify the Government against any and all liability, claims, and costs of whatsoever kind and nature for injury to or death of any person or persons and for loss or damage to any property occurring in connection with, in any way incident to, or arising out of the occupancy, use, service, operation or performance of work under the terms of the contract, resulting in whole or in part from the negligence acts or omissions of the Contractor, any subcontractor, or any employee, agent, or representative of the Contractor or subcontractor The Contractor is responsible for protection of existing finishes, and VA property, furnishings, fixtures, and equipment (FF&E), located in the areas where demolition, construction, preventative maintenance, and repair work is being performed, as well as along paths of travel to and from these areas. Both exterior and interior finishes and property must be maintained. The contractor is liable for any damage to VA property during the course of the contract. Any damage to existing finishes must be repaired by the Contractor to equal or greater quality than existing finishes. All tasks accomplished by the Contractor personnel will be performed to preclude damage or disfigurement of government-owned FF&E and architectural or building structures. Contractor will report any damage or disfigurement to these items when caused by the Contractor s personnel and will perform repairs or replace government-owned equipment, fixtures, furnishings, grounds and architectural or building structures to their previously existing condition. VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced. VIX. Period of Performance The period of performance for this contract is thirty (30) days after full NTP. END OF STATEMENT OF WORK
