Description
Department of Veterans Affairs, Stratton VA Medical Center, Network Contracting Office 2, is issuing this Notice of Intent to Sole Source to Advanced Sterilization Products Services (ASP) to provide Annual Maintenance on STERRAD Sterilization System equipment, located at the Hudson Valley VA Medical Center Castle Point Campus. SERVICES REQUIRED: Contractor shall provide annual service for preventive maintenance, repair and software upgrade for one ST100NX 1 DR (SN 1043150130), and one STERRAD 100NX ALLCLEAR 1-DR DUO (SN 1047191504), both located at the Hudson Valley VA Medical Center Castle Point Campus, 41 Castle Point Road, Wappingers Falls, NY 12590 from February 01, 2022 through January 31, 2023 with provision for four (4) Option Years. INTERESTED & CAPABLE RESPONSES: NCO 2 is seeking responses from businesses that are interested in this procurement and consider themselves to have the resources, rights, and capabilities necessary to provide these services. Please provide all of the following in your response: Business name, business type, socio-economic status (e.g., Veteran-Owned, Woman-Owned, Disadvantaged Small Business, 8(a), etc.), person of contact (including telephone number & email address), GSA Contract number if applicable, and DUNS number. Please also include the North American Industry Classification System (NAICS) Code for which you propose to provide the service under for verification of your size standard. A Capability Statement which provides detailed, specific information that demonstrates understanding of the tasks required and capability to perform the tasks as stated in the attached Statement of Work. Failure to tailor capability statement to this specific requirement may result in your company being determined non-responsive. Responses must be submitted by 10:00 AM EST, Monday, January 10, 2022. Responses to the information requested above may be submitted via email to Levi.Russin@va.gov. This Special Notice of Intent to Sole Source is for planning purposes ONLY. The results of this notice may assist in the development of (1) the requirement, and (2) the acquisition strategy (e.g., small business set-aside, full and open competition, etc.). VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted for this notice. STATEMENT OF WORK STERRAD FULL PREVENTIVE MAINTENANCE SERVICES A. BACKGROUND: The Department of Veterans Affairs, Hudson Valley VA Medical Center Castle Point Campus is currently seeking semi-annual full preventive maintenance service contract for Sterrad Sterilizer Equipment. These machines sterilize and properly clean medical instruments to ensure they are prepared for use in a timely manner. In order to keep these systems in optimal functioning order and to reduce downtime due to malfunctioning units, we require full service for semi-annual maintenance, repair, and software updates. B. SCOPE OF WORK: Contractor shall provide factory trained technicians and all resources (labor, tools, equipment, travel, parts, and services) necessary to accomplish the preventive maintenance, repair, and software update of two Sterrad systems. The following two systems are located at the Hudson Valley VA Medical Center Castle Point Campus: Equipment list: DESCRIPTIPTION PM SERIAL NUMBER STERRAD 100NX 1 - DR 2/YR 1043150130 STERRAD 100NX ALLCLEAR 1-DR DUO 2/YR 1047191504 All work shall be performed in accordance with the Original Equipment Manufacturer (OEM) standards by an experienced and qualified technician identified to work on the equipment listed above. All equipment shall be maintained in proper operating conditions as specified by the manufacturer. Contractor shall have access to Original Equipment Manufacturer parts, manuals, and schematics to perform the service. Contractor is required to furnish all parts and labor to accomplish the task of equipment preventative maintenance and repairs. Parts required should be from the OEM. Contractor shall test the functionality of the equipment upon completion of the corrective maintenance. If this is a test for repair, the test should ensure that the damaged parts have been successfully repaired and all functions, including but not limited to, those related to the repaired parts, are operational. If this is a test for a routine PM service, inspection, etc., then the test should just ensure that the equipment is functioning within the guidelines of the OEM or the applicable standards (OSHA, ASSE, JCAHO, NEFP, etc). Preventive maintenance will be scheduled in advance and conducted semi-annually. It is the contractor's responsibility to schedule preventive maintenance as listed. PM procedures described to must follow manufacturer s specifications and must be documented and submitted in writing to Biomedical Engineering. Upon completion of PM procedures, an applicable label should be applied to the unit, indicating the date of PM performance, the next date following PM is due, as well as the initials of the individual that performed the PM. At inception of contract, PM shall include, at a minimum, electrical safety testing, lubrication adjustment, calibration, testing and replacement of faulty parts and/or parts which are likely to fail at no additional charge. At the completion of each service call/PM, the contractor will provide a written service report to Biomedical Engineering. This report will clearly show the date of service, type of service performed, model and serial numbers, name of field technician(s), description of the work performed, a list of any parts replaced, and the unit condition as left at the completion of said work. Contractor shall provide technical support between the hours of 7AM-7PM EST. C. OTHER REQUIREMENTS: The contractor shall: Be a firm regularly engaged in the repair, maintenance, and servicing of Sterrad equipment described in this Statement of Work. Have expertise, experience, and regular business to include the repair, maintenance of Sterrad equipment. Address repairs and emergency calls as outlined in the SOW. Possess all the manpower, equipment, tools, and ready sources of parts as necessary to promptly, skillfully, and expertly carry out the conditions of the requirement. D. NONCOMPLIANCE The Contracting Officer or his/her designee will notify the contractor of any noncompliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his/her representative at the site of the work, shall be deemed sufficient for the purpose aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all and any part of the work and hold the Contractor in default. E. SPECIFICATIONS: Upon completion of the maintenance service, the COR/POC will perform the inspection in the presence of the contractor personnel, prior to the contractor personnel leaving the facility. This inspection should ensure that all equipment is returned to full functioning condition. No invoices will be processed for payment prior to the completion of COR/POC inspection. F. RESPONSE TIME Contractor shall provide a phone call back by a qualified technician within 4 hours of a service request and an onsite response by a qualified technician within 24 hours of original request for service. Vendor shall be on site to begin repairs within twenty-four (24) hours of initiation service call by the Hudson Valley VAMC Castle Point Repairs shall be completed, and unit returns fully to manufacturer specification within sixteen (16) hours of initiation service call Major assemblies replaced will be guaranteed against defects and/or failure for a period of one (1) year from date of installation Work to be performed not covered under this contract must be pre-approved by the contract specialist/officer and a separate purchase order will be issued for the work to be performed G. OTHER RELATED SERVICES (NOT INCLUDED IN THE STATEMENT OF WORK Any services not expressly stated in this statement of work are not authorized. Services such as inspection, Original Equipment Manufacturer (OEM) required maintenance, or other maintenance and repair vital to the continued functioning of the systems may be added only after written modification from the contracting officer, unless they are inclusive to the requested services. No other person, including the COR or any other VA personnel are authorized to request additional work be performed by the contractor. Any additional services performed that are not authorized by the contracting officer will be considered inclusive.  H. WORKING BUSINESS HOURS: All service will be performed during normal business hours of coverage unless proper alternate arrangements have been coordinated beforehand with the COR. Work hours will be from 8:00 AM 5:00 PM, Monday-Friday, excluding Federal Holidays. I. NATIONAL HOLIDAYS: Federal Holidays which are observed by the Federal Government are annotated below: - New Year s Day - Martin Luther King s Birthday - President s Day - Memorial Day - Juneteenth - Independence Day - Labor Day - Columbus Day - Veteran s Day - Thanksgiving Day - Christmas Day And any other day specifically declared by the President of the United States to be a national holiday. J. PERIOD OF PERFORMANCE The base period of performance will be one (1) year from the date of award with provision of four (4) Option Years. K. PLACE OF PERFORMANCE Department of Veterans Affairs Hudson Valley VA Medical Center Castle Point Campus 41 Castle Point Road Wappingers Falls, NY 12590 L. VHA SUPPLEMENTAL CONTRACT REQUIREMENTS FOR COMBATTING COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
