- INTENT TO SOLE SOURCE The Department of Veterans Affairs, Network Contracting Office 16 (NCO16) intends to issue a Sole Source contract for Calibration Services of the TempTrak Devices at the G.V. Sonny Montgomery VA Medical Center, Jackson, MS 39216. All work is to be completed in accordance with Statement of Work. This sole source contract will be solicited and negotiated with, TrillaMed, 30100 Telegraph Rd. Ste. 366, Bingham Farms, MI 48025, under the authority of FAR 8.405-6 (a)(1)(B) Limiting sources. Only one source. The North American Industry Classification System (NAICS) code for this procurement is 811219-Diagnostic Imaging Equipment Repair and Maintenance Services. The Small Business Administration (SBA) size standard is $22.0 Million. No solicitation will be made available. This is not a request for quotations. Inquiries must be sent via email to Contract Specialist, sharon.griffey@va.gov. Contracting will proceed with the sole source negotiation with TrillaMed. This is a one-time service requirement. Statement of Work Calibrations for TempTrak Monitoring Equipment G.V. Sonny Montgomery VAMC 1500 E. Woodrow Wilson Drive Jackson, MS 39216 1. GENERAL: G.V. (Sonny) Montgomery (GVSM) VA Medical Center (VAMC) is requesting a one-time calibration service for the TempTrak Devices deployed in the GVSM VAMC. The period of performance will be a one-time service performed at a scheduled time once service is acquired. 2. DESCRIPTION OF SERVICES: The contractor shall provide all personnel, equipment, supplies, transportation, tools, materials, supervision, and administration to perform the necessary calibrations on the TempTrak devices stated herein. The devices to be calibrated under this agreement are listed in Table 1 below. All the equipment listed is owned by the G.V. (Sonny) Montgomery VA Medical Center. The table below has brief description of equipment and quantity. A more detailed list of equipment can be found at Attachment A below the table. Table 1. Detailed listing of devices and equipment: Calibration services will be performed at the main facility, the GVSM VAMC, and Community Based Outpatient Clinics (CBOCS) in Columbus, Greenville, Hattiesburg, Kosciusko, Meridian, McComb, and Natchez (See detailed addresses below at #6.) 3. PERIOD OF PERFORMANCE: The expected period of performance for this requirement is a One time service performed at a scheduled time established with the Program Office/Contract Officer Representative (COR) as identified at Table 2 below: Table 2. WORK HOURS AND SCHEDULING ARRANGEMENTS: Services will be performed only during the facility s normal business hours (8:00 a.m. to 5:00 p.m. Monday through Friday), unless requested or approved by the COR. Prior to work being performed proper coordination shall be initiated between the contractor and the government to ensure that services/work can be executed when scheduled. Work will not be scheduled or be performed on federal holidays. The Federal Holidays which are observed by the federal government are annotated below. -New Year s Day -Martin Luther King s Birthday -President s Day -Memorial Day -Independence Day -Labor Day -Columbus Day -Veterans Day -Thanksgiving Day -Christmas Day -Any other day specifically declared to be a national holiday (per Federal Statute, Executive Order, or by the President s Proclamation). An alternative schedule (different from the schedule described above) may be utilized if there is mutual agreement between the government and the contractor regarding this alternative schedule. 5. SCOPE OF SERVICES: Required Deliverables DELIVERABLE #1: CALIBRATIONS (ANNUAL AND SEMIANNUAL): Contractor shall perform all calibrations on the equipment stated herein in accordance with the OEM (Original Equipment Manufacturer) recommendations to ensure that all equipment functions as intended by the manufacturer. It is the responsibility of the contractor to provide all required calibrations on the equipment to include being a subject matter expert (SME) in this area. Only qualified, capable, knowledgeable, trained, and/or certified personnel shall be permitted to perform calibrations, servicing, and/or other related work on the equipment. DELIVERABLE #2: SCHEDULING AND CHECK IN Prior to work being performed and/or services being executed such work/services shall be scheduled and the facility notified in advance. A mutually agreeable date shall be determined before such work/services begin. (The government may not be able to accommodate the contractor without prior notification, coordination, and scheduling of services in advance). Contractor representatives and/or technicians shall check in with the appropriate facility representative when on site and before beginning work. DELIVERABLE #3: REQUIRED REPORTING (FOR CALIBRATIONS, WORK, AND/OR SERVICES RENDERED) Contractor shall provide the government individual service reports after completing calibrations on the equipment. These reports shall be provided via e-mail to the appropriate government representative. The government reserves the right to request hard copies of these reports if needed. Such reports shall include (but are not limited to) the following information. Name of Contractor. Name of person, representative, and/or technician who performed the work/services. Date, Time (starting and ending), and location for all work performed. Itemized Description of all Work Performed and Services Rendered. Equipment Name, Manufacturer Name, Model Number, and other identifying numbers of equipment that was calibrated and/or serviced. Equipment downtime and/or other mission related factors that may or may not impact the future delivery of patient care at the facility. A specified format for the required service reports is not specified. However, such Reports shall be thorough and compete enough to demonstrate that all required calibrations In Accordance With manufacturer specifications, recommendations, and guidelines have been performed by the contractor and/or contractor representatives for the equipment specified. 6.
