Description
THIS IS A REQUEST FOR INFORMATION (RFI) SOURCES SOUGHT 1. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought Announcement is for market research purposes only, to make appropriate acquisition decisions and to gain knowledge of Small Businesses, Veteran Owned Small Businesses (VOSB), including Service Disabled Veteran Owned Small Businesses (SDVOSB), interested and capable of providing Removal of Expired Controlled Substance Medications that are set for destruction, per paragraph five (5) below, for the West Los Angeles Inpatient Pharmacy, the Los Angeles Ambulatory Care Center (LAACC) Outpatient Pharmacy, and the Sepulveda Ambulatory Care Center (SACC) Outpatient Pharmacy. 2. The NAICS for this requirement is 561990, All Other Support Services. The product service code is 6505, Drugs and Biologicals. 3. Interested and capable Contractors should respond to this notice not later than January 14, 2022 by providing the following via email only to Solly.Kim@va.gov. (a) company name (b) address (c) point of contact (d) phone and email of primary point of contact (e) DUNS number (f) A capability statement that addresses the organization s qualifications and ability to perform as a contractor specifically for the work described in paragraph 5 below. Respondents are also encouraged to provide specific examples (e.g. contract number, point of contact information) of the Contractor s experience providing the same or similar services described in paragraph 5 below. 4. SDVOSB/VOSB respondents. SDVOSB/VOSB respondents are encouraged to provide proof of registration in the Veteran Information Pages at https://www.va.gov/osdbu/. Further, SDVOSB and VOSB Contractors are asked to acknowledge that they understand the limitations on sub-contracting pursuant to FAR 52.219-14, Limitations on Sub-Contracting, which will be included in the impending solicitation. 5. Required Services. The Pharmacies at the following locations require services for the removal of expired controlled substance medications set for destruction: West Los Angeles Medical Center, Inpatient Pharmacy 11301 Wilshire Boulevard, Bldg. 500, Room #0204, Los Angeles, CA 90073 Los Angeles Ambulatory Care Center, Outpatient Pharmacy 351 East Temple Street, Room #B128, Los Angeles, CA 90012 Sepulveda Ambulatory Care Center, Outpatient Pharmacy 16111 Plummer Street, Room #1202, North Hills, CA 91343 The repackaging of all controlled substance medications set for removal from the pharmacies shall be performed by the vendor and/or its representatives. Tracking numbers shall be provided by contractor to the designated pharmacy representative. Services include the following: The contractor shall provide all labor, computer equipment, boxes, shipping labels and anything else necessary for the removal of expired or controlled substances (CS) and CS prescriptions set for destruction. The contractor shall be on site during the confirmed dates and times for each location and contact the pharmacy prior to arrival. The contractor shall provide inventory reports which documents each medication removed at each pharmacy site in detail to be acceptable to the Drug Enforcement Administration (DEA). The following information shall be provided in each service report: Date of service, facility DEA number, name of drug, strength, size, and quantity. A copy of each report shall be left with the pharmacy. The services at West Los Angeles Inpatient Pharmacy shall be performed every two months, while services at LAACC and SACC shall be performed quarterly. 6. The intended contract period of performance is anticipated to begin 10 days from the contract award to Dec 31, 2022. 7. Please note that if VA s pending market research establishes that two or more VOSB or SDVOSB, who are certified in VA s Vendor Information Pages (VIP) can perform the required services, the VA will set aside the underlying solicitation per 38 U.S.C. ยง8127. --End of Sources Sought Announcement--
