This request for capability information does not constitute a request for proposals; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred.� The Indian Health Service (IHS), Phoenix Area Office (PAO), Department of Acquisition Management issuing this sources sought synopsis as a means of conducting market research to identify parties having an interest in and the resources to support this requirement for providing Dental Prosthetic Appliance Fabrication to the Indian Health Service, Hopi Health Care Center located in Polacca, AZ, 86042.� The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 339114 (750 Employees Size).� The result of this market research will contribute to determining the method of procurement, including type of small business set-aside or unrestricted. This requirement is for dental prosthetic appliance fabrication for one base year and four one year options.��� We are requesting from the sources to provide a list/retail price on how their products are priced out in order for us to create a price schedule.
