- Description
- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. This solicitation is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-07; 11 August 2021. This acquisition is a full and open competition solicitation. This solicitation is issued as a multiple award solicitation, by which, the Government intends to award up to three (3) orders resulting from this requirement. The North American Industry Classification System (NAICS) Code is 334510 - Electromedical and Electrotherapeutic Apparatus Manufacturing with a size standard of 1250 employees. Awards resulting from this solicitation shall be made on a competitive best value basis using a �Best Value Trade-Off� approach. Reference attached RFQ for detailed information on the requirement, how to respond, and evaluation criteria. Questions: All questions pertaining to the RFQ shall be submitted via e-mail to Ms. Karyn Williams, Contract Specialist, at karyn.m.williams2.civ@mail.mil no later than 3 January 2022 at 2:00 PM EST. The subject header of the e-mail submission of questions shall include the RFQ number W81XWH22R0001. Questions received after this date will not be accepted. RFQ Due Date: Response to this RFQ is due via e-mail no later than 7 January at 2:00 PM EST via e- mail to Ms. Karyn Williams, Contract Specialist, at karyn.m.williams2.civ@mail.mil. No telephone calls will be accepted. AMENDMENT 0001 TO RFQ W81XWH-22-R-0012 is posted to incorporate responses to all questions received in regards to this RFQ.� See attachment for listing of all questions and responses. AMENDMENT 0002 TO RFQ W81XWH-22-R-0012 is posted to extend the RFQ closing date from 7 January 2022 at 2pm to 10 January 2022 at 2pm. All other terms and conditions remain unchanged. AMENDMENT 0003 TO RFQ W81XWH-22-R-0012 is posted to extend the RFQ closing date from 10 January 2022 at 2pm to 13 January 2022 at 2pm. All other terms and conditions remain unchanged.
