SOURCES SOUGHT ANNOUNCEMENT: Corporal Michael J. Crescenz Veterans Affairs medical Center, CMCVAMC is located at 3900 Woodland Avenue, Philadelphia, Pennsylvania 19104, is conducting market research to identify potential sources to provide Community Based Health Care for Homeless Veterans (HCHV) program. The goal of the HCHV program is to remove homeless Veterans from the street or habitation unfit for Veterans and place them in community-based, residential environments with sufficient supportive services to meet their needs and ultimately facilitate the improvement of their housing situation. The CMCVAMC is seeking 24-hours a day / 7 days a week (24/7) community-based early recovery model through a Safe Haven model of residential care at a facility located within the city of Philadelphia and accessible by public transportation that serves homeless Veterans, including those with serious mental illness, physical disabilities and substance use disorders. CMCVAMC requires two separate programs that serve two distinct levels of HCHV Contracted Residential Services. The purpose of this sources sought is to obtain interests from contractors who can provide housing, outreach services, case management services, and rehabilitative services within the city of Philadelphia for Veterans experiencing homelessness and/or chronic homelessness including Veterans diagnosed with severe mental illness and/or substance use disorders, in community-based residential facilities. This announcement is issued for informational planning purposes only and is not a request for quote or announcement of a solicitation. Submitting information for this announcement is voluntary and VA assumes no responsibility for any costs incurred associated with the preparation of responses submitted as a result of this notice. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. The purpose of this market research is to identify capable sources that are both experienced and qualified to provide the required services stated in the PWS. The contractor shall have all requirements as outline by the PWS. Capability Statement and Documentation: Companies are invited to provide their capabilities, experience, and knowledge in providing Community Based Health Care for Homeless Veterans (HCHV) Services. Sources are asked to provide the following information: 1. Company name, address, website, DUNS number, business type, socio-economic status (e.g., SDVOSB, VOSB, Woman-Owned, Disadvantaged Small Business, 8(a), general small business, large, business, etc.), and person of contact including telephone number and email address). 2. Indicate and explain your capability of providing this service. 4. Indicate your company s size under NAICS 624221. 5. Please indicate any government contracts held including government agency, contract number, dollar value, period of performance, and government contact information. In addition to a Capability Statement which includes the above information, submissions can include, but not limited to, brochures etc. as attachments. Information regarding your company s capacity and capability to provide such services are to be submitted electronically on or before Thursday, 01/13/2022 prior to 7:00 am (EST) addressed to Cynthia Laemmerhirt, Contract Specialist cynthia.laemmerhirt@va.gov.
