Page 7 of 7 THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of qualified sources to perform the potential requirement. This source sought is currently restricted to only Small Business providers capable of fulfilling the requirement in accordance with the Statement of Work. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. The Department of Veterans Affairs (VA), Network Contracting Office (NCO) 20, is conducting market research to identify potential Small Business sources which can provide the following services: Full-Service maintenance of the Zeiss Tivato and Kinevo Systems for the Portland VA Medical Center in accordance with the attached Statement of Work. Potential sources having the capabilities necessary to provide the above services which meet all the requirements outlined in the attached statement of work are invited to respond to this Sources Sought Notice via e-mail to Charles.Hartwick@VA.Gov no later than noted in the RFI. No telephone inquiries will be accepted. Responses should include the following information: Company name, address, and business size; point of contact name, phone number, and e-mail address; A capability statement addressing the requirements in the attached Statement of Work to include: Authorized distributor letter from Zeiss to provide OEM replacement parts, services and software support to the Federal Government as identified in the Statement of Work References for similar services currently being provided or previously provided at the required service levels as outlined in the Statement of Work. NAICS Code 811219 is applicable to determine business size standard. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. The Government will treat any information received as proprietary and will not share such information with other companies. Any organization responding to this Sources Sought Notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. The Government may or may not issue a solicitation as a result of this announcement. There is no solicitation available at this time. 852.215-70  Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors. Deviation per Class Deviation Veterans First Contracting Program (VFCP 2016), dated July 25, 2016, revises the clause at VAAR 852.215-70 to update the web address for the VIP database. This deviation is effective until incorporated into the VAAR or the VAAM or is otherwise rescinded.] SERVICE-DISABLED VETERAN-OWNED AND VETERAN-OWNED SMALL BUSINESS EVALUATION FACTORS (JUL 2016) (DEVIATION) (a) In an effort to achieve socioeconomic small business goals, depending on the evaluation factors included in the solicitation, VA shall evaluate offerors based on their service-disabled Veteran-owned or Veteran-owned small business status and their proposed use of eligible service-disabled Veteran-owned small businesses and Veteran-owned small businesses as subcontractors. (b) Eligible service-disabled Veteran-owned offerors will receive full credit, and offerors qualifying as Veteran-owned small businesses will receive partial credit for the Service-Disabled Veteran-Owned and Veteran-owned Small Business Status evaluation factor.  To receive credit, an offeror must be registered and verified in Vendor Information Pages (VIP) database (https://www.vip.vetbiz.va.gov). (c) Non-Veteran offerors proposing to use service-disabled Veteran-owned small businesses or Veteran-owned small businesses as subcontractors will receive some consideration under this evaluation factor.  Offerors must state in their proposals the names of the SDVOSBs and VOSBs with whom they intend to subcontract and provide a brief description of the proposed subcontracts and the approximate dollar values of the proposed subcontracts.  In addition, the proposed subcontractors must be registered and verified in the VetBiz.gov VIP database (https://www.vip.vetbiz.va.gov). (End of Clause) STATEMENT OF WORK (SOW) 1. Background Portland VA Medical center requires a full-service contract to ensure that two (2) Carl Zeiss Kinevo 900s and one (1) Carl Zeiss Tivato 700 are kept in proper operating condition and ready for use on patients. This contract includes preventive and corrective maintenance as well as remote support. 2. Scope All work is to be performed onsite at the Portland VA campus or via remote support over the telephone for the following units listed by serial number (SN). SN Model 6643122187 Tivato 700 6640122739 Kinevo 900 6640122738 Kinevo 900 3. Specific Tasks Vendor to provide parts, labor, and travel to repair, maintain, and keep the existing Carl Zeiss Kinevo 900 and Tivato 700 surgical microscopes in good working order, meeting all OEM specifications for operation throughout the term of the contract. Additional requirements include: Unlimited telephone technical support available M-F from 8:00am- 7:00pm PST. When a call for service is placed, virtual technical support will respond within one (1) hours. Onsite response, once determined it is needed, will be within four (4) hours of the request notification. Corrective maintenance and repairs, including parts, labor and travel/travel-related costs during normal coverage hours of Monday through Friday 8AM to 5PM. Preventive maintenance procedures to match the manufacturers recommended frequency per published service manual. Each visit includes: 1. Planned maintenance 2. Calibration of the Product 2. Functional checks 3. All PM parts, required replacement parts, labor, and travel/travel-related costs, excluding consumables. A service report submitted after each act of maintenance or repair via hardcopy Unlimited hospital staff applications support 4. Performance Monitoring The tasks are well defined and can, therefore, be easily monitored by the Government point of contact (POC), who shall represent the government for this process. The contractor performance will be monitored and the work certified by the POC. The POC will assure contractor quality by routine inspections, product sampling, and monitoring of work while work is being performed. 5. Security Requirements Contractor s work will be monitored. Onsite contractors will be escorted. 6. Other Pertinent Information or Special Considerations. None 7. Inspection and Acceptance Criteria. The POC is responsible for certifying that the work done under this contract is performed to standard. The POC is also responsible to assure the inspection and acceptance of products provided incidental to services. 8. Place of Performance. Work will primarily be performed at the PVAMC. 9. Period of Performance. 2/1/2021 through 1/31/22 with unilateral option to extend for Three (3) option years 10. Delivery Schedule.
