Description
This synopsis/solicitation notice is a request for proposals for a Non-Personal Service Contract for a Contractor to provide registered nurse services for the Operating Room Department, Northern Navajo Medical Center, Highway 491 North, Shiprock, New Mexico, 87420 - Navajo Area Indian Health Service (IHS) throughout the duration of the contract period of performance.� Services shall be performed in accordance with the attached Performance Work Statement (PWS).�� Refer to the PWS� and the Position Description for specific duties. There are 2 OR PACU nurse (s) to fill with contact nurses. This Request for Quote (RFQ) is Unrestricted for a Non-Personal Service Contract per FAR 37.401(a) for RN services to be performed at the Northern Navajo Medical Center, Shiprock, New Mexico of the Indian Health Service (IHS).� The tasks to be performed are cited in the attached Performance Work Statements. The Period of Performance will be issued as Base plus Option Periods: Base Period: Date of Award through 7/31/2022 1st Option Period: 08/01/2022 through 01/31/2023 The assignment is through the stated period of performance or until the position is filled with a permanent hire.� In such instances, the nurse will be released from contract assignment. WORK SHIFT: The shift hours are 8 hours is assigned for staggering hour of 8:00am-4:30pm, 8:30am-5:00pm and 9:00am-5:30pm Monday through Friday in a seniority order. The call starts at the ending of the shift hour for weekdays and weekend hour starts on Saturday morning at 7:30am. Must be able to take a call & respond within 45-60 minutes once they are notified of a surgical case. The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The Supervisor at IHS shall determine specific tour of duty. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated. REQUIRED DOCUMENTS: 1. Quotation in the line items below. The Government will consider competitive rates upon negotiation. Hourly Rate shall be an All-Inclusive Hourly Rate, Meaning travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local). The Attach Wage Determination Service Contract Act applies. 2. Past Performance: Provide experience of comparable scope and complexity in providing services within the past three (3) years of operation. 3. Qualifications: Provide complete candidate Profile (Curriculum vitae, updated resume, licensures / certifications, currently/valid contact information: phone number, email address, Professional Nursing Skills and all relative documents for proposed candidate. Refer to PWS Section 5.0 & 7.0 4. Availability of candidate(s). NOTE: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted. 5. Contractor will not come to work until Contract Office signs the Purchase Order with a start date. The tasks to be performed shall be in accordance with the attached Performance Work Statement (PWS). Housing: No Government Housing available. The nearby towns are Farmington, NM , 29 miles East of Shiprock, NM and in Cortez, CO, 41 miles North of Shiprock, NM. There are hotels, restaurants, mall, etc. in the two towns CONDITION OF EMPLOYMENT: Type of Citizenship: U.S. Citizenship is required. Background Investigation: All qualified referred candidates will undergo Fingerprinting process Fingerprint Card: FBI FD-258 (provided by the Federal Government): Fingerprinting MUST be cleared and approved prior to contact assignment. Contract assignment(s) are conditional on a cleared background check. OPM requires Electronic Questionnaire for Investigation Processing (e-QIP) for registered nursing services exceeding One Hundred Twenty (120) days. Electronic Questionnaire for Investigation Processing (e-QIP): Executive Orders 10450, 12968, and HSPD-12 require a background investigation as a condition of employment. This investigation will be processed utilizing the US Office of Personnel Management (OPM) system. QUALIFICATION / EVALUATION: The Government reserves the right to issue a single award or multiple awards to any contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, Price and other Evaluation Factors will be considered: Past Performance and Qualifications. The Evaluation Factors and significant subfactors when combined are significantly more important than cost or price. As part of the evaluation process, candidates shall be interviewed to ensure their understanding of the PWS and to verify their qualifications to perform required services. The socio-economic status of an offer may also be considered, should one or more quotes represent the best value. All candidates are required to have an official United States social security number. INITIAL APPOINTMENTS REQUIREMENTS: Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant. Factor 1: Agency's Past Performance Subfactor: Number of similar contracts Subfactor: Timeliness of providing candidate(s) Factor 2: Registered Nurse Certification Requirements (Refer to PWS 7.4) Subfactor: a valid and unrestricted State Nursing License Subfactor: a current Basic Life Support (BLS) Certification with American Heart Association Subfactor: a current Advance Cardiac Life Support (ACLS) Certification with American Heart Association Subfactor: Pediatric Advance Life Support (PALS) Certification with American Heart Association Medical Evaluation: The contractor shall provide a fitness for duty certificate issued by a licensed physician to perform the proposed job without significant risk to personal health or the health and safety of others Immunization: Contractor shall provide evidence of the following: - MMR (measles, Mumps, and Rubella), - Immunity to Hepatitis A and B - Flu Shot - History of chicken pox disease of positive titer - Tetanus Diphtheria (Td) within the last 5 years - Documentation of receiving a TB skin test (PPD) within the past 12 months with documentation of follow-up for a positive test. - A signed delineation of the Hepatitis B vaccination series shall be accepted ***A copy of the document shall be submitted to the Employee Nurse upon arrival at the hospital before providing service. Immunization: Contractor shall provide evidence of current immunization documentation for Flu Shot, Immunity to Hepatitis B series, and History of chicken pox (varicella) disease or positive titer, Tetanus Diphtheria (Td) within the last 5 years. Documentation of receiving a TB Mantoux skin test (PPD) within the past 12 months with documentation of follow-up for positive test. Factor 3: Registered Nurse Availability Subfactor: Upon fingerprinting clearance, contractor shall be available as soon as possible to start. Subfactor: Registered Nurse shall be flexible to allow for short term (13-weeks), long term (1-year) and, where feasible, include intermittent support to meet unplanned needs that develop on a frequent basis. Subfactor: The contractor shall rotate into other duty sections as needed to support patient care. Factor 4: Professional Nursing Skills: 2 years or more OR experience; ASPAN member a plus. Subfactor: Associate RN Degree, Minimum Subfactor: Circulating role for General Surgery, ENT, Ortho, Oral Surgery, OB/GYN, Orthopedic, Podiatry, Urology, Pedi Dental and Endoscopy; scrubbing role skills a plus. Subfactor: Familiar with the ASPAN standards, Infection Control, Safety and Joint Commission standards. Subfactor: Ability to function for a schedule surgical and emergency procedure. Subfactor: Ability to work with Electronic Health Record & Barcode Medication Administration Scanners Subfactor: Able to float to other departments upon completion of a float competency skills checklist. Subfactor: Ability & knowledge to learn the different types of equipment. Licensure: Must possess and maintain current State licensure while employed with Indian Health Service (IHS).
