- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number 36C25722Q0254 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-02. The NAICS Code for this request for quotes is 621511 ($35.0 million). No set-aside is used for this solicitation. The services being requested are for the reference laboratory testing of patient specimen. Services are to be performed at the South Texas Veterans Healthcare System, 7400 Merton Minter Blvd. San Antonio, TX 78229. The testing required for this request are located in attachment 36C25722Q0254 LineItems.xlsx . All vendors interested in responding to this solicitation must, at minimum, provide the following documentation: Completed Attachment ""36C25722Q0254 LineItems.xlsx"" Proof of accreditation and licensing CAP accreditation with deemed status from the Center for Medicate and Medicaid Services AABB certification CLIA certification Professional Staff Curricula Vitae Technical Staff Competency Skills Assessment Technical and Courier Staff Shipping and Handling of Laboratory Specimen/ Hazardous Materials Training and Assessment Proof of CAP/ Proficiency Exam Participation for past two (2) years Proficiency Examination Exception Records for past two (2) years, if applies Policy, list of tests, and associated results for critical and alert value notification Performance Improvement (Quality Assurance) Program/ Plan Current year performance improvement monitors Other Quality Assurance Monitors Implementation Plan Three (3) references of current or prior contracts - minimum two (2) governmental references, including one federal; including facility address, phone number, contact person's name and email who can validate experience within the last (2) years Provide copy of a signed letter issued to the VA Office of Small and Disadvantaged Business Utilization approving the offeror's Mentor-Protégé Agreement. The evaluation criteria are technical acceptability and price.
