1. This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: VA Long Beach It Equipment Rack and Power Installation 1.B. Description: VA Long Beach Healthcare System is seeking a contractor to install IT Equipment Rack and Power for the Guest Internet Project in the Main Frame Distribution (MDF) room located in Building 126 Basement. 1.C. Project Location: VA Long Beach Healthcare Systems, 5901 East 7th Street, Long Beach, CA 90822 1.D. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26222Q0313. 1.E. Set Aside: This solicitation is 100% set aside for Service-Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127. 1.F. Applicable NAICS code: 238210, Electrical Contractors and Other Wiring Installation Contractors 1.G. Small Business Size Standard: $16.5 Million 1.H. Type of Contract: Firm Fixed Price 1.I. Period of Performance: Contractor shall begin work from awarded date and notice to proceed and complete all required work within 15 days from commencing work. 1.J. Wage Determination: SCA Wage Determination 2015-5613 R20 1K. Key solicitation milestones are: 1.K.1. A site survey will be conducted at 10:00 AM Monday, January 24, 2022. Participants will meet outside of the IT trailer 167T. All participants must have face covering and practice social distancing. Provide at least 3 reference projects in a hospital environment where you have completed IT Equipment Rack and Power Installation. 1.K.3.F. Provide a detailed quote breakdown for both material and labor 1.K.3.G. Provide copies of OSHA training certificates. 2. Statement of Work. 2.A. Scope. VA Long Beach Healthcare System is seeking a contractor to install IT equipment rack and power for the Guest Internet project in Main Frame Distribution (MDF) room located in building 126 basement. Contractor shall furnish all necessary labor, materials, supplies, equipment and services (except as otherwise specified herein) and perform the work set forth within this Statement of Work listed below: VA Long Beach Healthcare System is seeking a contractor to install Future Flex Air Blown fiber from the basement of building 126 Main Distribution Frame (MDF) to the Individual Distribution Frame, IDF closet in building 164, 2nd floor and 166, 1st floor. Contractor shall unbolt the existing 2 post equipment rack and reinstall it about 1 foot to the right. Since there are existing IT equipment in the rack, VA staff will work with contractor to move them along with the 2 post rack. Contractor is responsible to re-organize all IT cables and electrical cables after re-install the 2 post rack at the new position. Contractor shall provide longer cables if need. Contractor shall install 1 VA provided 4 post equipment rack with 1 new vertical cable wire manager. Contractor shall install 2 new 20 Amp, 250 Volt Receptacle, NEMA L5-20R from newly installed 4 post equipment rack to nearest existing electrical panel within the MDF. Install electrical conduit long the pathway. Ground and bond to nearest communication grounding bus bar. Contractor shall be qualified work on the energized electrical panel. Contractor shall obtain the VA LONG BEACH ELECTRICAL WORK PERMIT prior to perform the job. Contractor shall provide all necessary material and accessories required to perform all the tasks listed above. Job site visit is highly recommended for this project. Job site visit schedule will be determined by Requesting Service and Contracting Officer before the project send out to bid. Contractor shall provide a detailed quote breakdown for both material and labor. Contractor shall provide warranty service for both material and labor. Material warranty shall base on the cable manufacturer warranty policy and labor warranty is one year after the work has been accepted by Veteran Affair Department. All deliverables will be inspected by the VA for completeness, and conformance to the items listed above by OI&T and Contracting Officer. Contractor must have 30 hours construction OSHA training for Project manager/Leader and 10 hours construction OSHA training for each contractor s employee. Contractor must indicate in the proposal that they meet all OSHA training requirement and certifications shall be submitted to Contracting Officer before start the job. Contractor is required to have TB test for all contractors employees whom work on the job sites. TB test report shall be submitted to Contracting Officer before start the job. Contractor is required to obtain the Above Ceiling Permit and ICRA at Hospital s Safety Office. Infection Control Class will be classified base on the job site and environment around it. The locations above in this SOW will fall in between Class I (lowest) to class III (highest). See below for Class I, II, and III requirements: Description of Required Infection Control Precautions by Class During Construction Project Upon Completion of Project Class I Execute work by methods to minimize raising dust. Immediately replace ceiling tile if displaced. Clean work area upon completion of task. Remove all visible debris with a wet towel and/or mop. Environmental Management Service to wipe work surfaces and floors with disinfectant. Class II Provide active means to prevent airborne dust from dispersing into atmosphere. Water-mist work surfaces to control dust while cutting. Seal unused doors with duct tape. Place sticky mat at entrance and exit of work area Remove or isolate HVAC system in areas where work is being performed. Cover construction waste before transport in covered containers. Wipe work surfaces with cleaner/disinfectant. Contain construction waste before transport in tightly covered containers. Wet mop and/or vacuum with HEPA filtered vacuum before leaving work area. Upon completion, restore HVAC system where work was performed. Environmental Management Service to wipe work surfaces and floors with disinfectant. Class III Remove or Isolate HVAC system in area where work is being done to prevent contamination of duct system. Complete all critical barriers i.e. sheetrock, plywood, plastic, to seal area from non-work area or implement control cube method (cart with plastic covering and sealed connection to work site with HEPA vacuum for vacuuming prior to exit) before construction begins. Maintain negative air pressure within work site utilizing HEPA equipped air filtration units. Negative pressure monitoring devices should be visible from outside the worksite and readings should be documented as least daily. Contain construction waste before transport in tightly covered containers. Maintain clean sticky mat at entrance and exit of work area. Cover construction waste before transport in covered containers. Do not remove barriers from work area until completed project is inspected by the Safety Department and Infection Prevention & Control and thoroughly cleaned by the Environmental Management Service. Remove barrier materials carefully to minimize spreading of dirt and debris associated with construction. Vacuum work area with HEPA filtered vacuums. Wet mop area with cleaner/disinfectant. Upon completion, restore HVAC system where work was performed. All Contractors must have a temporary ID badge issued by the VA Police, located in building 5C. Contractor is required to wear ID badges at all times when working in the hospital. At all the job sites listed above, contractor shall keep the areas clean, safe and shall clean up at the end of each day. Contractor must comply with requirements specified in ICRA for areas of this quote. Temporary wall is required around the working area if the job site is close to or inside the clinical area or VA employees working space. Contractor must maintain Medical Center fire barriers in accordance with NFPA. This includes maintaining integrity of all fire-stop assemblies when penetrated by cable, wires, and pathways. All fire-stopping shall comply with local Engineering requirements and applicable codes. Engineering and Safety will approve fire-stopping material and installation for each location. The contractor will provide the locations for the fire-stopping in each of the above ceiling permits for Engineering to inspect when complete. Contractor is required to follow the Hospital Infection control policy at all times when working in the Hospital. The infectious control policy will be presented to the contractor at pre-start safety meeting with safety and infectious control staff. It will also be detailed for each area in the ICRA plan. This is not only to protect the patients, employees and visitors but also to protect the contractor while he/she is working at VA Long Beach Healthcare System. 2. PERFORMANCE PERIOD: 15 business days from the awarded date. Contractor must be able to start immediately upon award and must be flexible to meet the VA s schedule. Contractor is required to propose the starting date in their proposal. 3. Place of Performance: VALB HCS; 5901 East 7th Street Long Beach- CA- 90822. 4. BASIS of ACCEPTANCE: Non-conforming products or services will be rejected. Deficiencies will be corrected by the contractor within 5 business days of rejection notice. If deficiencies cannot be corrected within 5 business days, the contractor must immediately inform the Contract Officer of the reason for the delay and provide a corrective action plan within 2 business days. B. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor C. SECURITY Information System Security The contractor shall ensure adequate LAN/Internet, data, information, and system security in accordance with VA standard operating procedures and standard contract language, conditions laws, and regulations. The contractor s firewall and web server shall meet or exceed the government minimum requirements for security. All government data shall be protected behind an approved firewall. Any security violations or attempted violations shall be reported to the VA project manager and the VBA Headquarters Information Security Officer as soon as possible. The contractor shall follow all applicable VA policies and procedures governing information security, especially those that pertain to certification accreditation. The Veterans Affair Acquisition Regulation (VAAR) security clause (cited below) must be included in all contracts: VAAR- 852.273-75 SECURITY REQUIREMENTS FOR UNCLASSIFIED INFORMATION TECHNOLOGY RESOURCES (a) The contractor and their personnel shall be subject to the same Federal laws, regulations, standards and VA policies as VA personnel, regarding information and information system security. These include, but are not limited to Federal Information Security Management Act (FISMA), Appendix III of OMB Circular A-130, and guidance and standards, available from the Department of Commerce's National Institute of Standards and Technology (NIST). This also includes the use of common security configurations available from NIST's Web site at: http://checklists.nist.gov (b) To ensure that appropriate security controls are in place, Contractors must follow the procedures set forth in ""VA Information and Information System Security/Privacy Requirements for IT Contracts"" located at the following Web site: http://www.iprm.oit.va.gov. D. ELECTRONIC AND INFORMATION TECHNOLOGY STANDARDS INTERNET/INTRANET The contractor shall comply with Department of Veterans Affairs (VA) Directive 6102 and VA Handbook 6102 (Internet/Intranet Services). VA Directive 6102 sets forth policies and responsibilities for the planning, design, maintenance support, and any other functions related to the administration of a VA Internet/Intranet Service Site or related service (hereinafter referred to as Internet). This directive applies to all organizational elements in the Department. This policy applies to all individuals designing and/or maintaining VA Internet Service Sites; including but not limited to full time and part time employees, contractors, interns, and volunteers. This policy applies to all VA Internet/Intranet domains and servers that utilize VA resources. This includes but is not limited to va.gov and other extensions such as, .com, .edu, .mil, .net, .org, and personal Internet service pages managed from individual workstations. VA Handbook 6102 establishes Department-wide procedures for managing, maintaining, establishing, and presenting VA Internet/Intranet Service Sites or related services (hereafter referred to as Internet ). The handbook implements the policies contained in VA Directive 6102, Internet/Intranet Services. This includes, but is not limited to, File Transfer Protocol (FTP), Hypertext Markup Language (HTML), Simple Mail Transfer Protocol (SMTP), Web pages, Active Server Pages (ASP), e-mail forums, and list servers. VA Directive 6102 and VA Handbook 6102 are available at: Internet/Intranet Services Directive 6102 http://www.va.gov/pubs/directives/Information-Resources-Management-(IRM)/6102d.doc Internet/Intranet Services Handbook 6102 http://www.va.gov/pubs/handbooks/Information-Resources-Management-(IRM)/6102h.doc Internet/Intranet Services Handbook 6102 Change 1 updates VA's cookie use policy, Section 508 guidelines, guidance on posting of Hot Topics, approved warning notices, and minor editorial errors. http://www.va.gov/pubs/handbooks/Information-Resources-Management-(IRM)/61021h.doc In addition, any technologies that enable a Network Delivered Application (NDA) to access or modify resources of the local machine that are outside of the browser's  sand box"" are strictly prohibited. Specifically, this prohibition includes signed-applets or any ActiveX controls delivered through a browser's session. ActiveX is expressly forbidden within the VA while .NET is allowed only when granted a waiver by the VA CIO *PRIOR* to use. JavaScript is the preferred language standard for developing relatively simple interactions (i.e., forms validation, interactive menus, etc.) and Applets (J2SE APIs and Java Language) for complex network delivered applications. E. SECTION 508 The contractor shall comply with Section 508 of the Rehabilitation Act (29 U.S.C. § 794d), as amended by the Workforce Investment Act of 1998 (P.L. 105-220), August 7, 1998. In December 2000, the Architectural and Transportation Barriers Compliance Board (Access Board), pursuant to Section 508(2) (A) of the Rehabilitation Act Amendments of 1998, established Information Technology accessibility standards for the Federal Government. Section 508(a)(1) requires that when Federal departments or agencies develop, procure, maintain, or use Electronic and Information Technology (EIT), they shall ensure that the EIT allows Federal employees with disabilities to have access to and use of information and data that is comparable to the access to and use of information and data by other Federal employees. The Section 508 requirement also applies to members of the public seeking information or services from a Federal department or agency. The preliminary and final deliverables and all associated working papers, application source code, and other material deemed relevant by the VA which has been generated by the contractor in the performance of this task order are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. The CO will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order presented to the contractor shall be submitted to the CO for response. Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. G. Work Hours: Monday through Friday from 7:00am to 5:00pm, excluding federal holidays. There may be some areas that require after working hours. G.1 The Vendor is not required to provide service on the following National holidays, nor shall the contractor be paid for these holidays. The following national holidays observed by the Federal Government: New Year s Day 1 January Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day 4 July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11 November Thanksgiving Day 4th Thursday in November Christmas Day 25 December If a holiday falls on Sunday, the following Monday shall be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government agencies. Also included would be any day specifically declared by the President of the United States. H. Changes to contract: Only those services specified herein are authorized. The Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. Changes to the contract are not authorized and reimbursement shall not be made for any work services performed outside the scope of this contract, unless prior authorization is obtained in writing from the Contracting Officer. J. Safety J.1. All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering on VA property shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. J.2. Contractor shall perform work in strict accordance with existing relevant, accredited standards and codes to minimize the possibility of injury to personnel, or damage to Government equipment. Every effort shall be made to safeguard human life and property. J.3. Contractor shall comply with all applicable Federal, State, and local requirements regarding workers health and safety. The requirements include, but are not limited to, those found in Federal and State Occupational Safety and Health Act (OSHA) statutes and regulations, such as applicable provisions of Title 29, Code of Federal Regulations (CFR), Parts 1910 and 1926, NFPA 70E, NFPA 99, Article 130.7 29 CFR 1910.132(d)(1)(iii), 29CFR1910.137 J.4. Contractor shall provide all necessary tools, and Personal Protective Equipment (PPE) to perform the work safely, effectively, and timely. J.5. Contractor shall take all precautions necessary to protect persons and property from injury or damage during the performance of this contract. Fire and safety deficiencies, which exist and are part of the responsibility of the Contractor, shall be immediately corrected. If the Contractor fails or refuses to correct deficiencies promptly, the COR may issue an order stopping all, or any part, of the work. Contractor shall be responsible for any injury to himself/herself, his/her employees, or others, as well as for any damage to personal or public property that occurs during the performance of this contract that is caused by him/her or his/her employee s fault or negligence. Contractor shall maintain personal liability and property damage insurance prescribed by the laws of the Federal Government. J.6. Contractor must maintain Medical Center fire barriers in accordance with NFPA. This includes maintaining integrity of all fire-stop assemblies when penetrated by cable, wires, and pathways. All fire-stopping shall comply with local Engineering requirements and applicable codes. Engineering and Safety will approve fire-stopping material and installation for each location. The contractor will provide the locations for the fire-stopping in each of the above ceiling permits for Engineering to inspect when complete. J.7. Contractor must comply with requirements specified in ICRA for areas of this project. Temporary walls are required around the working area if the job site is close to or inside the clinical area or VA employees working space. J.8. Contractor is required to follow the Hospital Infection control policy at all times when working in the Hospital. The infectious control policy will be presented to the contractor at pre-start safety meeting with safety and infectious control staff. It will also be detailed for each area in the ICRA plan. This is not only to protect the patients, employees and visitors but also to protect the contractor while he/she is working at VA Long Beach Healthcare System. J.9. Contractor is required to obtain the Above Ceiling Permit and ICRA at Hospital s Safety Office. Infection Control Class will be classified base on the job site and environment around it. The locations above in this SOW will fall in between Class I (lowest) to class III (highest). See below for Class I, II, and III requirements: J.9.a. CLASS I: - Execute work by methods to minimize raising dust from construction operations. - Immediately replace any ceiling tile displaced for visual inspection - Minor Demolition for Remodeling J.9.a. CLASS II: - Use an EC unit or a plastic barrier (with zipper) sealed to floor and ceiling tile. - The entire opening must be enclosed. - Use a HEPA vacuum to clean tops of tile (in ceiling space) in enclosed work area. - Use a HEPA vacuum when making penetrations in walls to control any dust. J.9.a. CLASS III: - Use an EC unit or a plastic barrier (with zipper) sealed to floor and ceiling tile. - The entire opening must be enclosed. - Use a HEPA vacuum to clean tops of tile (in ceiling space) in enclosed work area. - Use a HEPA vacuum when making penetrations in walls to control any dust. - Apply negative air to the EC unit or containment by means of HEPA filtered device. The HEPA filtered air outlet (exhaust) must be routinely checked for particulates. K. Insurance Coverage. K.1. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. K.2. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. K.3. Within 15 days of the awarded contract, the Contractor shall furnish to the Contracting Officer certification from his/her insurance company indicating that the coverage has been obtained and that it may not be changed or cancelled without guaranteed thirty (30) day notice to the Contracting Officer. New certifications shall be furnished prior to the expiration date. 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2021-06 Effective July 12, 2021.
