Description
COMBINED SYNOPSIS / SOLICITATION NOTICE:� This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; Quotes/proposals are requested and a written solicitation will not be issued.� The 177FW, NJANG has a requirement for Optimizing the Human Weapons System (OHWS) Pre-habilitative services as per the attached PWS. 1. Solicitation W50S8F-22-Q-0002 is issued as a Request for Quotes (RFQ). 2. Quotes shall be valid for 90 days are requested. 3. This solicitation incorporates provisions and clauses for commercial items IAW Federal Acquisition Regulation and its applicable supplements. 4. This requirement is a total Small Business set-aside.� The associated NAICS code for this acquisition is 621340.� Small business size standard is $8.0M. 5. The contractor shall provide all personnel, equipment, tools, supervisor and all other items and services necessary to perform pre-habilitative services in accordance with the Performance Work Statement (PWS).� The contractor shall provide a narrative on how they intend to perform the work to meet the requirements defined in the PWS and any attached documents. 6. Quotes shall be submitted with pricing according to each individual like item listed below.� The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer. CLIN# DESCRIPTION QTY/UNIT PRICE 0001 � The contractor shall provide all personnel, equipment, tools, supervisor and all other items and services necessary to perform pre-habilitative services in accordance with the Performance Work Statement (PWS).� Period of Performance is one year.� Anticipated start date is 1 March-01 April 2022�������� 12 Mo. $ 0002 - The contractor shall provide all personnel, equipment, tools, supervisor and all other items and services necessary to perform pre-habilitative services in accordance with the Performance Work Statement (PWS).� Period of Performance is one year.� Anticipated start date is 1 March-01 April 2023.����������� 12 Mo. $ 0003 - The contractor shall provide all personnel, equipment, tools, supervisor and all other items and services necessary to perform pre-habilitative services in accordance with the Performance Work Statement (PWS).� Period of Performance is one year.� Anticipated start date is 1 March-01 April 2024.����������� 12 Mo. $ 0004 - The contractor shall provide all personnel, equipment, tools, supervisor and all other items and services necessary to perform pre-habilitative services in accordance with the Performance Work Statement (PWS).� Period of Performance is one year.� Anticipated start date is 1 March-01 April 2025.������������� 6 Mo. $ 1 Job (Base plus 2 Options Years & a potential 6-month option if required):� TOTAL: $ 7. The intended contract vehicle will be a Firm-Fixed Price Contract.� 8. The provision FAR 52.212-1, Instruction to Offerors, Commercial in its latest edition applies to this solicitation. 10. Quotes will be evaluated for conformance with the requirements of the solicitation, the Performance Work Statement and contractor documentation submitted.� The government intends to award a contract to the responsible offeror whose offer conforming to the solicitation will provide the Best Value to the Government in meeting the technical requirements IAW PWS and in price.� � 11. Quotes shall include a completed copy of the provision at 52.212-3 Alt 1, Offer Representations and Certifications � Commercial Items, 52.222-22, Previous Contracts and Compliance Reports (FEB 1999) and 52.222-25, Affirmative Action Compliance (APR 1984), with their offer.� These provisions can be found at https://sam.gov/. 12. The clause FAR 52.212-4, Contract Terms and Conditions - Commercial items in its latest edition applies to this solicitation. 13. All information relating to this solicitation, including, changes/amendments, questions and answers, will be posted on the following website: http://SAM.Gov.� Formal communications such as request for clarifications and/or information concerning this solicitation shall be submitted in writing via e-mail to 177FW.Contracting.Org@us.af.mil.� Emails sent to any other email will not be answered. All answers (on a non-attribution basis) will be posted on the SAM.Gov web site.� They will be contained in the in a document titled �Questions and Answers�.� Offerors shall submit questions via email to 177FW.Contracting.Org@us.af.mil not later than 12:00 EST, 08 February 2022.� Terms of the solicitation and specifications remain unchanged unless the solicitation as amended in writing.� If an amendment is issued, normal procedures relating to the acknowledgement and receipt of solicitation shall apply. 14. Wage Determination 2015-4189 Rev 19 as of 27 December 2021 applies to this solicitation. Award can only be made to contractors registered in System for Award Management (http://www.sam.gov).� Contractors shall be registered in SAM at the time they provide their quote. Quotes are due NLT 21 February 2022 at 12:00 PM EST to the attention of MSgt Tammy Hickerson at 177FW.Contracting.Org@us.af.mil.� Emails sent to any other email will not be answered. It is the best interest of the offeror to ensure the quote submitted is received by the 177FW NJANG Contracting Office no later than the date and time specified or it may be considered non-responsive.
