- Spokane VAMC is soliciting for a SDVOSB to completely prepare site for building operations, including demolition and removal of existing structures, and furnish labor and materials and perform work for 668-22-102 Install Eye Washes in Housekeeping Closet Campus Wide as required by drawings and specific. Questions and Answers 1 - Has a Hazardous Materials Survey been conducted and available for all aspects of this construction project? Federal Regulation (40 CFR Section 61.145) requires that building owners conduct, or have conducted a Survey for Asbestos Containing Materials (ACM), or Presumed Asbestos Containing Materials (PACM) and other hazardous materials (such as lead, PCB s, mold, etc.) prior to the start of any demolition or renovation activities. The Survey must be conducted by an AHERA/EPA Certified Building Inspector. Copies of any materials testing data, and the complete report must be maintained by the Owner, and provided to any and all contractors who propose to work for the specific renovation activities. No, a Hazardous Materials Survey will have to be conducted by the contractor. 2 - Section 01 00 00 1.29 Construction Sign & 1.30 Safety Sign Will signs or signage be required for this project? Yes, a construction sign is required, can be a piece of paper taped to the door with all the pertinent project information. A safety sign is not required. 3 - Section 01 35 26 1.9 - Inspections SSHO / CSP / CSHT. Will a CSP or CSHT inspection as identified in 01 35 26 1.9B be required on this limited project. No, a SSHO is sufficient. 4 - Section 01 35 26 1.13 Tuberculosis Screening Will Tuberculosis screening be required at this VA facility? Contractor personnel must meet appropriate health screening requirements for the VA facility. 5 - Section 01 45 00 3.4 Quality Control Organization calls for a QC team with extensive qualifications. Due to the limited size and nature of this project, will this in-depth QC Organization be required? No 6 Can the project Superintendent also act as the SSHO and QC? These duties can be performed by qualified, full-time, on-site staff. 7 - Section 01 91 00 General Commissioning Requirements - It is not clear as to who is responsible to hire and retain the Commissioning Agent (VA or Contractor). Please advise. Contractor is required to appoint an individual, company or firm to act as the Commissioning Agent (01 91 00 1.10 A.) . 8 - Attachment 20 - Price Breakdown sheet is for a project for the Boise VA. Please provided updated price sheet. 9 - SOW 3.7 Contractor shall provide a qualified plumber to check complete installation for conformance to manufacturer s recommendation, to place the system into service, to make all adjustments required for full conformance to specified requirements and to perform all demonstrations and testing. Can this be performed by the installing plumbing contractor or will this have to be an independent 3rd party plumber or Testing & Balance contractor? Note similar requirement reference is also in SOW section 4.1.9. This can be performed by the installing plumbing contractor in both instances. 10 During the site walk we were unable to view Housekeeping Closet in Building 1 Floor 5 Room A531 due to COVID restrictions. Please provide pictures of this space. Will there be any additional measures needed to be in place to perform this work or will this work be scheduled for when COVID restrictions are lifted? See attached pictures. Work can be performed in this area without taking any additional measures beyond the standard COVID and designated ICRA precautions. Work does not need to be scheduled for when COVID restrictions are lifted, it can be completed at any time. 11 The Housekeeping Closet in Building 1 Floor 8 Room B806 is located within the surgery suite and we were unable to view this room during the site walk. Please provide pictures of this space. Will there be any additional requirements (Tyvek suits, gowns, etc.) for when working within in this surgical area? See attached pictures. Tyvek suits, booties and hair nets must be utilized at all times while in the surgical area, as well as the standard COVID and designated ICRA precautions. Work will have to be performed overnight when the surgery suite is not in use.
