This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice.� This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. THIS IS A NATIVE-OWNED SMALL BUSINESS/BUY INDIAN SET-ASIDE. �The solicitation number is 246-22-Q-0036 for Ultrasound System to include installation and warranty.� This solicitation is issued as a Request for Quotation (RFQ).� The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2020-07. �This acquisition is 100% Set aside for Buy - Indian Small Business. The NAIC code is 334510 and the small business size standard is 1,250 employees. The closing date for receipt of quotes is February 4, 2022 2020 at 2:00 pm.� Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, ATTN: Ronay English, no later than 2:00 p.m., on February 4, 2022.� The offer must be submitted: showing the solicitation number, and your name and address.� Qualified vendors will review the following and submit applicable information.� No phone inquiries! Questions are due by January 31, 2022: Ronay.English@ihs.gov Delivery shall be at the new EL RENO INDIAN HEALTH CENTER, 1801 PARKVIEW AVE, EL RENO, OK 73036 Technical Specification:� See attached Statement of work Evaluation Criteria: � The due date for this requirement is February 4, 2022 by 2:00 PM CT.� THE INDIAN HEALTH SERVICE WILL NOT CONSIDER YOUR QUOTE FOR AWARD IF YOU FAIL TO RESPOND TO THESE EVALUATION CRITERIA AS FOLLOWS: � Registration in the System for Award Management (SAM) database (reference https://www.sam.gov/portal/public/SAM/.� Please note that all parties interested in doing business with the Indian Health Services must be registered in the SAM database. Firm-Fixed price quote. Technical specifications (section III) and submittals of the Statement of Work. Proposals shall be evaluated on an acceptable/non-acceptable basis. Unacceptability of �A� �B� or�C� (stated above) shall render the entire quote unacceptable. Evaluation shall proceed as follows: Quotes shall be ranked by price, from low to high. Quotes shall then be evaluated for technical acceptability beginning with the lowest priced quote. The process shall proceed until a technical acceptable quote is identified.
