Description
COMBINED SYNOPSIS/SOLICITATION 628th Medical Group TSI PortaCount Respirator Fit Testing Kits This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. FA4418-22-Q-0017 is issued as a request for quotation (RFQ) for six (6) TSI PortaCount (Model 8048T) Respirator Fit Testing Kits with Tablet and Accessories. A firm-fixed purchase order is anticipated. Provisions and clauses in this RFQ are those in effect through Federal Acquisition Circular 2022-02, dated 14 January 2022 and Defense Federal Acquisition Regulation Supplement Change Number 12/30/2021. This acquisition is being solicited on an unrestricted brand name only basis. The associated NAICS code is 334519 with a 500 employee size standard. The contractor shall submit a quote for brand name only items that meets exact specifications as noted in this document (see attached Justification & Approval). Solicitation pricing information shall be filled out on the attached pricing sheet and submitted with total price. All responsible sources may submit a quote, which shall be considered. Contract items/supplies shall be delivered to the 628th Medical Group Military Treatment Facility, 105 Arthur Drive, Building 1001, Joint Base Charleston, SC. FAR 52.212-1, Instructions to Offerors � Commercial Items applies to this acquisition with the following Addenda; Offerors shall utilize FA4418-22-Q-0017 RFQ Price Sheet to submit offered pricing in response to this RFQ. FAR 52.212-2, Evaluation � Commercial Items The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers in the following relative order of importance. 1. Price. The Government will evaluate the total price of the offer for award purposes. 2. Technical acceptability. Only quotes offered to the Government for brand name TSI PortaCount Respirator Fit Testing Kits (Model 8048-T) will be deemed as technically acceptable. The Government will award a contract to the lowest price, technically acceptable offeror. �All offerors shall include a completed copy of FAR 52.212-3, Offeror Representations and Certifications --Commercial Item or complete electronic annual representations and certifications at the System for Award Management. The clause at FAR 52.212-4, Contract Terms and Condition�Commercial Items, applies to this acquisition. See additional Provisions and Clauses in the accompanying solicitation document. AFFARS 5352.201-9101 5352.201-9101 OMBUDSMAN (OCT 2019) �������������� (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (d) The ombudsman has no authority to render a decision that binds the agency.
