Description
Sources Sought Only This solicitation is a REQUEST FOR INFORMATION only. This Sources Sought Notice is not a request for formal proposals or quotes. It is neither a solicitation announcement nor a request for proposals or quotes and does not obligate the Government to award a contract. Requests for a solicitation will not receive a response. Responses to this Sources Sought must be in writing. The purpose of this Sources Sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HubZone and other Small Businesses interested and capable of providing the services described below. Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition. Responses to this notice shall include the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of Business, e.g. Services Disabled Veteran Owned Small Business, Veteran-owned small business, 8(a), HUBZone, Women Owned Small Business, Small disadvantaged business, or Small Business HUBZone business, or Large Business (i) must provide a capability statement that addresses the organizations qualifications and ability to perform as a contractor for the work described below. The intent is to award a Firm Fixed Price contract. Place of Performance: Albany Stratton VA Medical Center, 113 Holland Ave., Albany, NY 12208 Buffalo VA Medical Center, 3495 Bailey Avenue, Buffalo, NY 14215 Finger Lakes VA Medical Centers: Bath VA Medical Center, 76 Veterans Avenue, Bath, NY 14810 Canandaigua VA Medical Center, 400 Fort Hill, Canandaigua, NY 14424 Rochester Outpatient Clinic, 260 Calkins Road, Rochester, NY 14623 Syracuse VA Medical Center, 800 Irving Avenue, Syracuse, NY 13210 Period of Performance: April 1, 2022 to March 31, 2022 with provision of (4) option years Purpose and Objectives: Contractor shall provide on-site services as well as same day pick up and drop of for refurbishing of reusable medical equipment and if necessary, off-site repairs for instruments that require extensive repairs. Instruments shall be Inspected, sharpened, and repaired on various stainless-steel instruments, complete instrument trays and peel pack single instruments, power equipment repair, pneumatic equipment repair, power equipment battery service, light cables, fiberoptic retractors, phaco handpieces, dental handpieces and laparoscopic instruments, repairing minor nicks, bents, rim alignments, remove minor burrs from containers that house the instruments, and if necessary marked unrepairable during scheduled weekly, bi-weekly, monthly, quarterly and/or annual visits. Instruments may originate from each discipline at the facilities listed above, examples: Operating room, Dental, Neuro, Ophthalmology, Ear Nose & Throat, and Vascular reusable medical equipment. A comprehensive maintenance report shall be provided for each visit to the facilities. All work performed on surgical equipment shall be performed in accordance with Original Equipment Manufacturers (OEM) specifications Response: Responses to this Sources Sought Notice should be able to demonstrate the firm's ability, capability, and responsibility to deliver the service listed above. All work shall be performed by experienced and qualified personnel. The contractor shall be both ISO 9001:2015 and ISO 13485 certified. Responses should include the following information: A summary of your related capabilities to provide cooling tower fill / services listed above. It should be brief and concise, yet clearly demonstrate your abilities to meet the stated requirements Limit responses to a total page limit of 5 pages in a Microsoft Word compatible format. Address your approach to how the service would be provided as requested above. Provide no more than five contract history matrices for any contract relevant to this effort. For all current or past contracts deemed relevant, provide administrative reference information. Determination by the Government not to compete this proposed contract based on responses to this notice is solely within the discretion of the Government. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this sources sought synopsis request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. Response Date: The North American Industry Classification Systems (NAICS) code for this requirement is 811219 Other Electronic and Precision Equipment Repair and Maintenance with a Small Business size standard of $22 million. Any offeror capable of providing this service shall notify the Contracting Officer by email no later than January 31, 2022 10:00 AM EST. Phone calls will not be accepted.
