THIS IS NOT A SOLICITATION. This is a Sources Sought Notice only and is issued to conduct market research and gain knowledge of current market capabilities. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. This notice is issued solely for information and planning purposes and does not constitute a solicitation or a guarantee to issue a solicitation in the future. This notice does not commit the Government to contract for any supply or service. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Not responding to this notice does not preclude participation in any future solicitation, if issued. This announcement is based on the best information available at this time and is subject to future amendment. The Department of Veterans Affairs, Network Contracting Office 4, is conducting a market survey to identify potential sources to perform a firm fixed price contract for procurement of adaptive driver evaluation and training for Butler VA Medical Center, 325 New Castle Rd., Butler, PA 16001: Draft Requirements Description (NOT COMPREHENSIVE) The Butler VA Health Care system is seeking to procure a contract for adaptive driver evaluation and training to cover a five-year period. The contractor will be required to go to the Veteran s home or the Butler VA Health Care System or the surrounding area for evaluations and trainings. Definition Within the Department of Veterans Affairs, driver evaluation and training for the disabled is designed to provide professional evaluation and instruction for eligible Veterans in the safe, competent utilization of driving a motor vehicle independently and in accordance with State Department of Motor Vehicle (DMV) regulations. These evaluations and/or training may include the mastery of specific skills to safely and competently utilize special add-on or modified equipment. The types of Disabilities that are treated in the VA Driver Rehabilitation Program include, but are not limited to: Paraplegia Tetraplegia Brain dysfunction Hemiplegia Neurological disorders Amputation, i.e., upper and lower extremities Orthopedic problems Psychiatric disorders Disabilities associated with aging or acute or chronic illness Services Required Include: Pre-Driver Evaluation 2. On-Road Evaluation Car 3. On-Road Evaluation Van 4. Car Training 5. Van Training 6. Adapted Vehicle Modification Evaluation 7. Vehicle Consultation and Selection Components of Driving Evaluation Services Pre-Scheduling Process Required from contractor for all referrals. The Driver Rehabilitation Specialist shall address the following areas as part of the initial assessment and evaluation. Initial Contact Have the initial contact with patient (driver evaluation/rehabilitation candidate) in an interview atmosphere. Verification of Driver s License The contractor will confirm the patient s driver s license is valid and current for use with no restrictions. A copy of the license will be made to add to the final written report. VA Form 10-9028, Driver Training Functional Evaluation Record Complete patient s VA Form 10-9028, for each patient s continual use throughout evaluation or training period. History Obtain from patient the history of the patient s driving record, including any citations, accidents, or suspensions. Medical Clearance Inform patient of steps to be taken to obtain medical clearance if such is required from the State medical authority. Pre-driving Testing This evaluation will be completed only when specifically requested by the points of contact (POCs) from Butler VA for a patient and will be comprised of the following elements. Vision evaluation evaluate visual acuity, field of vision, depth-perception, color-vision, peripheral-vision, night acuity, and glare recovery (tests may be administered by use of visual screening tools) Physical evaluation manual muscle tests, balance (static and dynamic), transfer ability, ambulation ability, endurance, range of motion of all extremities, plus neck, if necessary (emphasis on exactness of upper extremity range of motion becomes greater if lower extremities are nonfunctional) Cognitive/Memory evaluation VAMC St. Louis Mental Status Exam (SLUMS), Cognitive Linguistic Quick Test (CLQT), Trail-making A & B tests Basic perceptual testing (e.g., dynamic figure-ground) Coordination testing Hearing (subjective) Sitting tolerance Activity tolerance and susceptibility to fatigue Spasticity Bowel or bladder control Reaction time, i.e., response time from accelerator to brake Sensation and proprioception Functional activities of daily living (ADL) Educational training Sign recognition Pennsylvania Drivers Manual Reaction Time Test accelerator to brake On-Road Testing This evaluation will be completed only when specifically requested by the points of contact (POCs) from Butler VA for a patient and will be comprised of the following elements. Transfer to and from conveyance Stowage and retrieval of wheelchair or ambulatory aid Orientation to vehicle s controls (e.g., turn signals, windshield wipers, headlights, etc.) Proper body positioning and alignment Pre-driving checks, including external considerations (e.g., lights, tire pressure), and internal (e.g., gas supply) Mirror references, including blind spot checks and tests Passenger responsibility (for example, seat belts, door locks) Starting and stopping the vehicle Right and left turns, U-turns and 3 point turnabouts Centrifugal forces Backing up Parking, parallel parking Lane changes Emotional stability behind the wheel Hill driving, uphill and downhill parking Passing other vehicles Hazardous driving situation (i.e., inclement weather, brake failure, flat tire, stuck accelerator) Changing traffic flows Pedestrian hazards Freeway entry and exit Basic driving skills in a low-traffic area Complex driving skills in a high-traffic area (i.e., city driving) Reaction time Night driving Driving safety tips, e.g., Smith System, SIPDE System, 3-5 Second following distance, 12-15 second visual lead time Vehicle Modification and Adapted Equipment Evaluation This evaluation will be completed only when specifically requested by the points of contact (POCs) from Butler VA for a patient. The purpose of this referral is to evaluate the need for vehicle modification and equipment adaptations to accommodate a Veteran with functional limitations in order to increase his/her ability to independently drive safely in accordance with DMV regulations. The following areas of clinical evaluation are defined as follows and will be comprised of the following elements. Vision Portable vision devices provide an evaluation of certain vision factors necessary to perform a safe driving task. Equipment must be available to measure the following: visual acuity, color perception, field of vision, depth perception, glare recovery, and night vision. Reaction Time A device is used to measure reaction time from acceleration to braking. Cognitive and Perceptional Screening In screening of basic cognitive and perceptional motor materials, identification and/or verification of a potential problem must be referred to the appropriate professional for a more in-depth evaluation. Neuromotor Assessment This is a physical assessment of range of motion, strength, sensation, coordination, and endurance. Assessment of Medical Need Vendor will assess medical necessity for certain modifications, including, but not limited to, ingress/egress needs, hand controls, left-foot accelerator, van lift, electric door opener, raised roof, raised doors, lowered floors, swivel transfer seat, powered transfer seats, wheelchair tie-downs, chest and shoulder harnesses, back up battery systems, relocation of gas tank. Vehicle Consultation and Selection This evaluation will be completed only when specifically requested by the points of contact (POCs) from Butler VA for a patient. The purpose of this referral is to assist the Veteran patient with selecting an appropriate vehicle that best suits their specific needs. The following areas of clinical evaluation are defined as follows and will be comprised of the following elements. The Veteran s functional limitations; his/her mobility devices (e.g., power chair, scooter, etc.); the need for and type of mechanical lift; the need for the vehicle to accommodate medically necessary modifications including, but not limited to, ingress/egress needs, van lift, electric door opener, raised roof, raised doors, lowered floors, swivel transfer seat, powered transfer seats, wheelchair tie-downs, chest and shoulder harnesses, back up battery systems, relocation of gas tank. Components of Driver Training Services Training services will be completed only when specifically requested by the points of contact (POCs) from Butler VA for a patient and will be comprised of the following elements. Contractor will follow the same scheduling processes and documentation standards as outlined in Components of Driving Evaluation Services. The patient will acquire specific knowledge in all areas of the driver training course including didactic theory and practical experience. Psychomotor improvements in handling segments of the driver training task will be noted by comparison of function before and after completion of the training course. The patient will maximize independence in all aspects of driving. Supporting adaptive equipment will enable patients to mainstream themselves into society. Adapted Driver Training for Van or Car Transfer to and from conveyance Stowage and retrieval of wheelchair or ambulatory aid Orientation to vehicle s controls Proper body positioning and alignment Mirror references, including blind spot checks and tests Passenger responsibility (for example, seat belts, door locks) Pre-driving checks, including external considerations (e.g., lights, tire pressure), and internal (e.g., gas supply) Starting and stopping the vehicle Use of hand controls, spinner knob, left foot accelerator Right and left turns, U-turns and 3 point turnabouts Centrifugal forces Backing up Parking, parallel parking Lane changes Emotional stability behind the wheel Hill driving, uphill and downhill parking Passing other vehicles Hazardous driving situation (i.e., inclement weather, brake failure, flat tire, stuck accelerator) Changing traffic flows Pedestrian hazards Freeway entry and exit High-traffic area (i.e., city) driving Night driving Scheduling & Documentation -- Required for all referrals. Facility POC will contact contractor when consult for driver rehabilitation evaluation and/or training is ordered. Veteran will be contacted by contractor within five (5) business days of referral to schedule the appointment. Contractor will schedule the patient and notify referring VA provider with date/time of appointment. Evaluation or training will be completed within ten (10) business days of contractor s initial contact with Veteran unless Veteran prefers a later time. The contractor will be required to go to the Veteran s home or the Butler VA Health Care System or the surrounding area for evaluations and trainings. Contractor will submit a written report using VA Form 10-9028, within five (5) calendar days of evaluation to the designated POC at the Butler VA Health Care System. The written report will include recommended driving restrictions or limitations (e.g., no night driving, no city driving, revocation of driver s license, etc.); recommendations for further evaluation; recommended driver training services, and so forth. The vendor will first convey these findings to the patient and/or significant other. No additional services will be provided without seeking and receiving written approval from the Butler VA POC. Vendor and Staff Qualifications The facility must be regulated by the Pennsylvania Department of Education and all employees before hire must have the appropriate background investigations to be completed every three years. Before someone is hired, they must have the following: 1. Act 34 - Child Abuse 2. FBI fingerprinting 3. State Police background check 4. Drug & Alcohol screening 5. Driver's license check Contractor must submit in writing annually that all employees background investigations and documentation are up to date; this written verification will be submitted to the facility POC by close of business on the last working day of September every year. It is required that at least one contract driver rehabilitation specialist in the organization be licensed, registered or certified by their professional organization. All specialists must have a minimum of a Baccalaureate Degree in adapted physical education, kinesiotherapy, physical therapy, occupational therapy or a specifically related rehabilitation therapy area. Completion of the National Driver Rehabilitation Specialist Examination is preferred. Maintenance and Safety. Routine maintenance and weekly safety checks of all adaptive equipment shall be scheduled and documented. This documentation will be submitted to Butler VA POCs upon request on a quarterly basis. The documentation will be submitted every year by close of business on the last working day of December, March, June, and September. Capabilities Statement and Documentation: Companies are invited to provide their capabilities, experience, and knowledge in the provision of adaptive driver evaluation and training services. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.
