This is a combined synopsis/solicitation for commercial services/items prepared in accordance with the format in FAR Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. 1.A. Project Title: To perform Boiler Plant Annuals and Semi-Annuals Inspections, and Safety Testing at the VA LL Healthcare System 1.B. Project Location: VA Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92357 1.C. Solicitation Type - Number: Request for Quotes (RFQ) - 36C26222Q0339. 1.D. Set Aside: This solicitation is 100% set aside for Service-Disabled Veteran Owned Small Businesses IAW Public Law 109-461, 38 USC Section 8127 1.E. Applicable NAICS code: 238220, Plumbing, Heating, and Air-Conditioning Contractors 1.F. Small Business Size Standard: $16.5 Million 1.G. Type of Contract: Firm Fixed Price 1.H. Period of Performance: Base year and up to four (4) one (1) year option periods. 1.I. Wage Determination: SCA WD 2015-5629 Rev 17 2. Key solicitation milestones are: 2.A. Site Visit: A site visit is scheduled for February 1, 2022, at 10:00 AM PDT at VA Loma Linda Healthcare System (VALLHS), 11201 Benton Street, Loma Linda, CA 92357. Participants will meet at the front of the medical center main doors at 11201 Benton Street, Loma Linda, CA 92357. Participants must wear masks and practice social distancing. Please email isaac.shimizu@va.gov to indicate that you are participating in the site visit. No later than January 31, 2022 12:00 PM PDT. Ensure to reference solicitation # 36C26222Q0339- VAGLL Boilers SITE VISIT within subject line of the email. Attendance is NOT mandatory, but highly encouraged so offeror(s) can inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent the information is reasonably obtainable. 2.B. Submit any questions regarding this procurement via email to isaac.shimizu@va.gov no later than 12:00 PM PDT, Tuesday, February 8, 2022. No questions will be answered after this date/time unless determined to be in the best interest of the Government as determined by the Contracting Officer. Telephonic questions will not be accepted or returned. RFI responses will be responded to as necessary in amendment format which will be posted to contract opportunities at https://sam.gov/. 2.C. Submit written offers in accordance with the following instructions, as well as Addendum to FAR 52.212-1, Instruction to Offerors. Oral offers will not be accepted. Documentation to verify offeror is an authorized Cleaver Brooks Service Provider and Distributor. 2.C.8. Copy of California C4 contractor s license or equivalent. 3. Scope. VA Loma Linda Health Care System located at 11201 Benton Street, Loma Linda, CA 902357 is seeking a contractor to perform the Boiler annuals semi-annuals inspections & safety testing for the following (3) Boilers Cleaver Brooks Cable 400-horse power located in boiler plant (2) Cleaver Brooks Clear Fire Condensing boiler 700 located at Cancer Center (1) Cleaver Brooks Clear Fire-700 Condensing Boiler located at Valor Building.(1) Cleaver Brooks Clear Fire -750 Condensing Boiler located at CLC#2. The inspection, calibration, and testing will occur be performed on an annual and semi-annual basis. Contractor shall perform all testing in accordance with the VHA Boiler Safety Device Testing Manual (Fifth Edition), VHA Directive 1810, and all other applicable laws / regulations. The Contractor must be a authorized Cleaver Brooks Service Provider and Distributor. 3.A. Specific Requirements. 3.A.1 The Contractor shall have, but not limited to the following qualification, experience and certifications: Shall possess a valid California C4 contractors license, or equivalent Technicians shall have completed at least a one (1) year trade school and have five (5) years successful experience in this field. The experience shall be largely with institutional and industrial boiler plants similar in design to the VALLHS plant. Technician must demonstrate knowledge and familiarity with NFPA 85, Boiler and Combustion Systems Hazards Code and VHA Boiler Plant Safety Device Testing Manual, 5th Edition 2018. 3.A.2. The contractor shall provide all necessary parts, labor, materials, equipment and transportation to perform the maintenance service which shall include the following tasks in accordance with VHA Directive 1810 and Boiler Plant Safety Device Testing Manual 5th Edition: Perform all annuals and semi-annuals with the VA employee verifying the tasks are being complete. Contractor will perform proper lockout tagout on all equipment being worked on. Contractor will ear proper protective equipment. Contractor will provide inspector for annual certification of boilers. Contractor will open DA and surge tanks inspect and pressure clean tanks. Contractor will install new door seals on both tanks. Contractors will provide list of all calibrated gauges. Contractor will perform (1) annual and (1) semiannual yearly. Adjust burner combustion settings and calibrate oxygen trim. Calibrate instrumentation, monitoring, and control systems. Calibrate pressure gages and thermometers. Contractor will remove sea and install new seals on all equipment in boiler plant. 10.Contractor will perform tune up on fuel oil. 11.Contractor will perform tune up on natural gas. 12.Contracor will perform calibrations on all gauges and thermometers. 13.Contractor will perform all this tasks as per manufacture specifications. 14. Contractor will perform all task as per VHA Directive 1810. 15. Contractor will perform tasks as per Boiler device safety testing manual 5thEdition. 16. Contractors will clean up trash and properly dispose of properly. 17. Contractor will operate boilers and tanks to verify proper operation. 18. Contractor will provide all paperwork pertaining to job. 19. Contractor will provide semiannual and annual documentation. 20. Contractor will provide list of all calibrated gauges. Upon completion of full testing, contractor will submit an engineering report for our records. The engineering report will summarize work completed, detail corrective action required and recommended actions. 4. Place of Performance: VA Loma Linda Healthcare System (VALLHS), 11201 Benton Street, Loma Linda, CA 92357. 4.A. Period of Performance: Base year and up to four (4) one (1) year option periods. As follows: 5. CHANGES: The awarded Contractor is advised that only the Contracting Officer, acting within the scope of the contract has the authority to make changes which affect the contract in terms of quality, quantity, price or delivery. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the contract price to cover any increase in costs incurred as a result thereof. 6. VA POLICY: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. 7. SAFETY AND FIRE PREVENTION: All personnel employed by the Contractor in the performance of this contract, or any representative of the Contractor entering the VAGLAHS shall comply with applicable Federal, State, and VA safety and fire regulations and codes, which are in effect during the performance period of the contract. 8. PARKING POLICY: It is the responsibility of Contractor personnel to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s personnel under any circumstances. 9. IDENTIFICATION BADGES: All Contractor personnel are required to wear identification (I.D.) badges during the entire time they are on the VA Healthcare System grounds. I.D. badges must have an identification picture and shall state the name of the individual and the company represented. 10. SMOKING POLICY: Smoking is not permitted within or around the VA Healthcare System facilities, except in designated areas. 11. NORMAL WORKING HOURS: Contractor will shall be performing services between the hours of 8:00 AM through 4:00 PM Monday through Friday, with exceptions to National Holidays. 12. OVERTIME AND NATIONAL HOLIDAYS: Any overtime or holiday pay that may be entitled to the Contractor s employees shall be the sole responsibility of the Contractor and shall not be billed to nor reimbursed by the Government. The Contractor is not required to provide service on the following U.S. Government holidays nor shall the Contractor be paid for these days: New Year s Day January 1 Martin Luther King s Birthday Third Monday in January President s Day Third Monday in February Memorial Day Last Monday in May Independence Day July 4 Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day November 11 Thanksgiving Day Fourth Thursday in November Christmas Day December 25 13. Insurance Coverage: 13.A. The Contractor agrees to procure and maintain, while the contract is in effect, Workers Compensation and Employee s Public Liability Insurance in accordance with Federal and State of California and/or Nevada laws. The Contractor shall be responsible for all damage to property, which may be done by him, or any employee engaged in the performance of this contract. 13.B. The Government shall be held harmless against any or all loss, cost, damage, claim expense or liability whatsoever, because of accident or injury to persons or property of others occurring in the performance of this contract. 13.C. Before commencing work under this contract, the Contracting Officer shall require the Contractor to furnish certification from his/her insurance company indicating that the coverage specified by FAR 52.228-5 and per FAR subpart 28.307-2 has been obtained and that it may not be changed or canceled without guaranteed thirty (30) day notice to the Contracting Officer. 13.D. Contractor is required to provide copies of proof of workers Compensation and Employee Public Liability Insurance within fifteen (15) calendar days after notification of contract award. 13.E. VHA Supplemental Contract Requirements for Ensuring Adequate COVID-19 Safety Protocols for Federal Contractors 13.E.1. Contractor employees who work in or travel to VHA locations must comply with the following: 13.E.1.a. Documentation requirements: 13.E.1.a.1. If fully vaccinated, shall show proof of vaccination 13.E.1.a.1.i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination 13.E.1.a.2. If unvaccinated and granted a medical or religious exception, shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test. 13.E.1.a.3. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. 13.E.1.a.4. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 13.E.2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the following website: COVID-19 Screening Tool. Regularly check the website for updates. 13.E.2.a. Contractor employees who work away from VA locations, but who will have direct patient contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 13.E.3. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. 14. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-02 Effective January 14, 2022.
