- THIS IS A SOURCES SOUGHT ANNOUNCEMENT FOR INFORMATION ONLY. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT WILL BE AWARDED FROM THIS ANNOUNCEMENT. The purpose of this notice is to conduct market research and obtain information from qualified businesses with special interest in seeking: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), Hub Zone Small Business, Women Owned Small Business (WOSB), Small Disadvantaged Business (SDB), Small Business, or Large Business. No solicitation is being issued at this time. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. NIHCS has a requirement for Ceiling Patient Lift Preventative Maintenance. The locations are listed below. Responses are due by 02/04/2022 at 10:00 AM EST. Responses shall be submitted via email. The Government will not review any other information or attachments included in your submission that is not listed herein or exceeds the10-page limitation. SCOPE Contractor shall furnish all parts, labor, supervision, material, and test equipment including transportation cost necessary to perform Preventative Maintenance (PM) on the listed serial numbers. Contractors shall maintain a call center to serve as a central point of contact for coordinating all equipment service activities. The contractor must complete the OEM checklist and the VA PM Checklist upon the completion of the PM services in accordance with patient safety alert 14-07. Storage for Contractor equipment is not provided at the VA Health Care System. The Contractor is responsible for the security of its equipment. The Government is not responsible for the security of Contractor equipment. III. COMPREHENSIVE MANTENANCE COVERAGE Unless otherwise noted, the maintenance program shall cover the cost of on-site service, including parts, labor, travel, and Preventative Maintenance visits per OEM specifications. Services shall be performed between normal business hours of 8am-5pm, Monday through Friday, except Federal Holidays. Standard coverage includes: Authentic OEM approved parts Trained and certified Labor Travel Full safe working/rated load weight test Battery replacement every 24 months Battery conditioning every 12 months Preferred labor and travel rates for corrective or planned maintenance outside service agreement coverage hours Preventative Maintenance per OEM recommendation Phone Support Free labor to install authentic OEM lift parts during inspection and preventative maintenance IV. QUALIFIED SERVICE PROVIDERS This contract shall allow the VA Health Care System to utilize OEM and/or ISO service providers to perform the preventative maintenance service for all service events. The cost of all covered service events shall be covered by the Contractor throughout the contract term. V. SERVICE REQUESTS The Contractor shall maintain a call center to receive preventative maintenance requests. Equipment end users shall be provided with a toll-free number to request maintenance as frequently as recommended by the lift s OEM. VI. AFTER HOURS COST PROTECTION In the rare event after-hours or holiday services are requested, Contractor shall cover maintenance charges at the service provider s standard prevailing rates 24x7x365. VII. VENDOR NETWORK DATABASE Contractor shall maintain a database of diverse service providers capable of performing service on a billable basis. The Contractor shall ensure that each service vendor operates ethically, performs quality workmanship, and consistently delivers service in a timely manner. The Contractor shall provide VA Northern Indiana Health Care System with service options when necessary to service non-proprietary systems or components. VIII. FIRM-FIXED PRICE ARGREEMENT The maintenance agreement shall be Firm-Fixed Price and have a common anniversary date. Invoicing shall be monthly in arrears (Net 30). IX. CONFORMANCE STANDARDS All services provided under this contract shall be performed in conformance with the National Fire Protection Associate (NFPA) Code No. 450, Occupational Safety and Health Administration (OSHA) Standard 1910, and Original Equipment Manufacturer (OEM) standards and specifications. Contractor must be knowledgeable with the Patient Safety Alert AL 14-07 X. CALIBRATION AND DOCUMENTATION OF CONTRACTOR S TEST EQUIPMENT Test and measuring equipment used by the Contractor in performance of this contract must be calibrated in the last twelve months to a traceable national or international standard such as National Institute of Standards and Technology (NIST). For each service instance in which Contractor test or measuring equipment is used to calibrate, maintain, or repair the medical equipment included in this contract, the Contractor must track the specific test or measuring equipment used. Ideally, this test or measuring equipment shall be uniquely identified on the service documentation provided to the facility at the completion of the service instance. This identification shall be traceable back to the specific test and measuring equipment used. Upon request, Contractor must provide COR/Biomedical Engineering with documentation of the calibration of this test and measuring equipment. This documentation shall include at least the following: Unique identifier for the test and measuring equipment. This shall match the identifier on the service documentation provided to Biomedical Engineering. Testing interval. Test date. Next test due date. As received and As returned status of the equipment being tested (e.g., was equipment in tolerance or not when received for testing and was equipment in tolerance or not when returned). Calibration results data. Identification of the device used to test equipment. Statement of traceability to the national or international standard used in the testing and calibration of the equipment. The national or international standard will be specified on the certificate. Reference to the testing procedure used. Calibration company identification information including company name, address, phone number and name of specific representative completing performance testing and calibration service. XI. VHA SUPPLEMENTAL CONTRACT REQUIREMENTS FOR COMBATTING COVID 19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation shall not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they shall have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
