THIS REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT DISCLAIMER This RFI is issued solely for information and planning purposes only and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and allowed under this RFI In Accordance With (IAW) FAR Part 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 541330. Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The following VAAR policies and clauses will be applicable to this requirement if an SDVOSB/VOSB entity is interested and qualified in this requirement: limitations on subcontracting requirements set forth in 15 U.S.C. § 657s and in VAAR 852.219-10 and 852.219-11); 852.219-77, VA Notice of Limitations on Subcontracting Certificate of Compliance for Services and Construction The Department of Veterans Affairs (VA), VISN 15 Network Contracting Office, is seeking sources that can provide the following: The following is the Statement of Work (SOW) for the fulfillment of a requirement to review, modify and update the existing The Joint Commission (TJC), Statement of Conditions Life Safety Drawings and Documentation for the VA Medical Center in Marion Illinois and its associated facilities. 1. The surveys shall be in accordance with The Joint Commission (TJC) Comprehensive Accreditation Manual for Hospitals, effective January 2021 (or later editions). The surveys will focus on TJC Environment of Care (EC) and Life Safety (LS) standards for the Statement of Conditions (SOC) documentation. All SOC documentation shall be prepared in accordance with TJC Elements of Performance criteria LS.01.01.01. 2. Survey will cover only those buildings where a Statement of Conditions is required by The Joint Commission (TJC). a. This will include the following buildings on the Marion campus, 2401 West Main Street, Marion, IL 62959: i) 1 - 82,048 SF ii) 2 - 29,013 SF iii) 38 - 25,048 SF iv) 42 82,074SF à 92,074 SF Spring 2023 v) 43 - 31,197 SF vi) 81 - 18,820 SF vii) 82 - 16,695 SF viii) 83 - 29,127 SF The following building is not on the Marion Campus: Evansville IL Health Care Center (118,776 SF). b. The following off campus facilities will also require Life Safety Inspections but will not require annual updates discussed later in this statement of work. i) Carbondale IL CBOC 9999 SF ii) Effingham IL, CBOC 9999 SF iii) Madisonville KY, CBOC 2700 SF iv) Harrisburg IL, CBOC 8814 SF v) Mayfield KY, CBOC 8500 SF vi) Mount Vernon IL, CBOC 5570 SF è Relocating Spring 2022 = 15,000 SF (If the relocation occurs after the base year inspections, the new location will need to be inspected in the first option year.) vii) Owensboro KY, CBOC 8347 SF è Relocating Summer 2022 = 11,500 SF (If the relocation occurs after the first option year, the new location will need to be inspected in the second option year.) viii) Paducah KY, CBOC 9745 SF ix) Vincennes IN, CBOC 8961 SF è Relocating Fall 2023 = 11,500 SF (If the relocation occurs after the second option year, the new location will need to be inspected in the third option year.) x) Primary Care Annex, Marion IL 9617 SF è Relocating Fall 2024 = 20,033 SF (If the relocation occurs after the third option year, the new location will need to be inspected in the fourth option year.) c. All buildings/space for which TJC requires a Statement of Conditions documentation will be inspected. The work will include visual inspection of smoke and fire barrier doors, suites, and hazardous areas. The surveyor will consult with the hospital Engineering and Safety Sections to identify areas of the hospital that meet the mixed occupancy criteria as outlined in NFPA 101. d. Upon completion of the inspection, the surveyor shall provide an overview of findings and identify any serious deficiencies that need immediate attention to hospital leadership and Environment of Care staff. e. Follow-up technical support (12 hours annually) will be provided by telephone/electronic mail to the site up through the TJC accreditation survey. 3. The surveyor will use the most current Life Safety Code Standard (2021 Edition). All SOC documentation shall also be in accordance with the Memorandum of Understanding (Effective May 1, 2021) between the VA and TJC for implementation of TJC Environment of Care and Life Safety standards. Where the standards differ significantly from the Life Safety Code that TJC currently uses, he/she will point out the differences and advise on compliance alternatives. 4. A pre-survey telephone conference will be held with the Chief of Engineering, the Chief of Safety, and leadership staff at a mutually agreed upon time within 15 days of contract ward. Survey schedules will be approved / finalized during the pre-survey conference. 5. The start date of the SOC survey should be within 45 days of contract award. The facility survey will occur on consecutive days within a single week or consecutive weeks. 6. The contractor will assist the hospital by video session (within 14 days of the site survey) on data entry of the latest Statement of Conditions documentation Basic Building Information electronically on TJC extranet website. The contractor will also provide 1-2 hours of training specifically geared towards the processes of accessing, updating, and maintaining the website. 7. The contractor/surveyor will submit three (3) copies of the draft written report within thirty (30) days after the initial site visit. Three (3) copies of the final written report shall be provided within 14 days after receipt of the draft report review comments. The final report shall be also submitted in an electronic format in Microsoft Word and AutoCAD. 8. The contractor/consultant firm shall possess the appropriate qualifications and credentials for conducting life safety surveys. The firm shall permanently employ a Fire Protection Engineer and/or Life Safety Code Specialist, both with minimum 10 years health care occupancy experience. The fire protection engineer shall have a B.S in Fire Protection Engineering Or, have a B.S. in a different field of engineering and have professional registration in the field of fire protection engineering. Either the fire protection engineer, or the life safety code specialist shall be a member of either the NFPA 101, Life Safety Code technical committee or NFPA 99 Health Care Facilities technical committee. The lead surveyor shall be one of the two above. Experience conducting Statement of Conditions surveys within the VA is preferred. 9. For the listed buildings at the Marion IL campus: a. All fire barrier, all smoke barrier and all hazardous room doors will be inspected and tested accordance with NFPA Standards 80 and 105 and provide a door/door frame report (approximately 170 doors at the Marion campus in 2021). Identify: nonfunctioning latches / locks / closers / sequencers, excessive gaps / undercuts, missing hardware, damaged doors, non-rated doors and/or frames. b. Conduct a 10% random above ceiling barrier inspection including areas with recent renovations to verify barriers. Not looking for penetrations but note those found. 10. The contractor will provide pricing for Base Year (2022) plus four (4) option years (2023, 2024, 2025, and 2026) first quarter updates to the SOC including: a. Two to three day survey of the buildings listed for the main facility to determine compliance with NFPA 101 Life Safety Code b. Detailed survey of all health care occupancy areas listed in item 1.a c. Survey of all hazardous areas, exit paths, and fire protection features d. On-Site review of the electronic Statement of Conditions (eSOC) e. Identification and documentation of deviations from NFPA 101 for development of work orders and/or Long term Corrections f. Three (3) copies of the updated SOC drawings and a new drawing CD. 11. The facility will provide AutoCAD drawings of the individual buildings to be surveyed and existing smoke zone barrier locations. The drawings provided will not be or contain information from previous Statement of Conditions reports. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the brand name. If you are interested, and are capable of providing the sought out supplies/services, please provide the requested information as well as the information indicated below. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? Is your company considered small under the NAICS code identified under this RFI? Are you the manufacturer, distributor, or an equivalent solution to the items being referenced above? If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? Does your company have an FSS contract with GSA, VA NAC, NASA SEWP, or any other federal contract, that can be utilized to procure the requirement listed above? If so, please provide the contract number. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached SOW. Responses must be received no later than 2-9-2022 at 12PM CST If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this sources sought.
