- THIS IS A SOURCES SOUGHT ANNOUNCEMENT FOR INFORMATION ONLY. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT WILL BE AWARDED FROM THIS ANNOUNCEMENT. The purpose of this notice is to conduct market research and obtain information from qualified businesses with special interest in seeking: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), 8(a), Hub Zone Small Business, Women Owned Small Business (WOSB), Small Disadvantaged Business (SDB), Small Business, or Large Business. No solicitation is being issued at this time. All information submitted in response to this announcement is voluntary; the Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. Indianapolis has a requirement for Cepheid GeneXpert Preventative Maintenance. The locations are listed below. Indianapolis (Indy) Richard L. Roudebush VAMC at 1481 W 10th St, Indianapolis, IN 46202 The appropriate NAICS Code 811219, Other Electronic and Precision Equipment Repair and Maintenance, Size Standard: $22 Million Responses: Please submit your response in accordance with the following instructions: Responses shall be no more than 10 pages in length. Responses are due by 02/11/2022 at 10:00 AM EST. Responses shall be submitted via email. Emails cannot exceed 10mb Subject line of the Email shall include this Sources Sought Notice Number Firm Name Indy Cepheid GeneXpert Preventative Maintenance. If your response contains proprietary information you will place Proprietary Information in front of the Sources Sought Notice Number. No marketing materials are allowed as part of this Sources Sought Notice. The Government will not review any other information or attachments included in your submission that is not listed herein or exceeds the10-page limitation. Provide how your company invoices for services in the commercial marketplace. All prospective Contractors are reminded that in accordance with FAR 4.11 they shall be registered in the System for Award Management (SAM) database and shall complete annual Representations and Certifications in SAM prior to any offer. SAM is located at http://www.sam.gov and consolidated the capabilities of CCR/FedReg, ORCA, and EPLS. There is no cost to use SAM. Contractor shall provide a public price list/contract price list for the services described herein. Contractor shall provide a detailed response to include your firm s ability, capability, and responsibility to deliver the services presented in this Sources Sought Notice. Contractor shall provide if they plan to sub-contract all or part of the work, and to what firm to include the subs duns number and business size. Determination by the Government not to compete this proposed contract action based on responses to this notice is solely within the discretion of the Government. Preliminary - Specifications/Work Statement I. DESCRIPTION OF SERVICES: Cepheid will provide a service contract with up to four (4) option years to maintain working order of the GeneXpert IV (MX# 1311422; S/N 815155) located at Richard L. Roudebush VAMC at 1481 W 10th St, Indianapolis, IN 46202. Additionally, the contractor shall provide a service contract for the maintenance and service of the GeneXpert XVI (EE# 67889; S/N 800925) at Richard L. Roudebush VAMC to last until December 31, 2022. The services provided by the contracts are to include: An initial reinstatement of the equipment(s) 24/7 phone support Onsite coverage 5 days per week, from 8am to 5pm local time Annual preventative maintenance check-ups performed by a qualified field service engineer Provision of parts, labor, and travel as needed for services Software upgrades Guaranteed 98% uptime All other services as mentioned in the vendor quote(s) II. PAYMENT FOR SERVICES: An initial payment for the first term of the contracts will be made, followed by annual payments for subsequent option years based on the terms of the service contract. Payment is to be made 30 days from each invoice date. Failure to provide purchase order number on the invoice may result in the return of the invoice. III. Security: Contractor does not require access to the VA network nor patient information to perform the requested services. No PHI/PII will be involved. The C&A requirements do not apply, and a Security Accreditation Package is not required. IV. VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities.
