Page 4 of 4 THIS IS A SOURCES SOUGHT NOTICE ONLY. This is not a solicitation for bids, proposals, proposal abstracts, or quotations. The purpose of this Sources Sought Notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. The responses received from interested contractors will assist the Government in determining the appropriate acquisition method. Puget Sound Health Care System (PSHCS) Seattle Division requires on-site, full service maintenance and repair services to ensure Tablo Units used for dialysis treatment are kept in proper operating condition and ready for clinical use. Specific Tasks. General Requirements: This contract includes all parts and service to properly provide preventative maintenance, on-site repairs, to ensure that all units are fully operational according to manufacturer specifications prior to departure. Contractor shall provide all necessary travel, labor, materials, tools, documentation, and parts required for the inspection, support, and repair of the equipment, software, and accessories listed in the Schedule of Equipment. Schedule of Equipment: Item # System Name 1 Tablo Unit 1 2 Tablo Unit 2 3 Tablo Unit 3 4 Tablo Unit 4 For items 1-4, the contractor shall provide the following: Maintenance and repair services, Monday to Friday, 8am to 5pm, as needed, excluding Federal holidays Regular planned maintenance per manufacturer guidance All labor, travel, and parts needed for any and all service trips to the Seattle Division, including repair call outs. Response on-site within three business days Updates to hardware and software that are released during the period of performance of this contract Provide Carbon and Sediment filters and filter changes Provide 24x7 phone support The contractor shall provide the Station Point of Contact reports for all work performed, as outlined in the Delivery Schedule. Deliverable Title Deliverable Specifics Delivery Method Calendar Days After CO Start Full-Service Report (FSR) for Tablo Unit 1 Comprehensive Service Report and Corrective and Preventative Maintenance Checklist A copy of the FSR will be emailed to email addresses to be provided by onsite staff within 24 hours of completion, as well as to the Station Point of Contact. Within 24 hours of service completion Full-Service Report (FSR) for Tablo Unit 2 Comprehensive Service Report and Corrective and Preventative Maintenance Checklist A copy of the FSR will be emailed to email addresses to be provided by onsite staff within 24 hours of completion, as well as to the Station Point of Contact. Within 24 hours of service completion Full-Service Report (FSR) for Tablo Unit 3 Comprehensive Service Report and Corrective and Preventative Maintenance Checklist A copy of the FSR will be emailed to email addresses to be provided by onsite staff within 24 hours of completion, as well as to the Station Point of Contact. Within 24 hours of service completion Full-Service Report (FSR) for Tablo Unit 4 Comprehensive Service Report and Corrective and Preventative Maintenance Checklist A copy of the FSR will be emailed to email addresses to be provided by onsite staff within 24 hours of completion, as well as to the Station Point of Contact. Within 24 hours of service completion The contractor shall also, in addition to the reports in the Delivery Schedule, provide a monthly report showing all work accomplished that month and any and all work planned to be accomplished in the next month. The report will identify unresolved problems and their proposed solution, as well as resolved problems and the steps taken to resolve them. Report due by last day of the month. Performance Monitoring. The contractor shall be monitored by the Station Point of Contact or their designee. The Station Point of Contact shall submit an annual performance evaluation to Network Contracting Office 20 summarizing the performance viewed over the period of performance. Other Pertinent Information or Special Considerations. All contractor personnel must have at least two years of field experience as a Field Service Engineer or meet OEM standards for a fully qualified Field Service Engineer to perform maintenance on the equipment. . Potential Businesses having the capabilities necessary to provide the above stated equipment services are invited to respond to this Sources Sought Notice via e-mail to derek.crockett@va.gov no later than 02/14/2022. No telephone inquiries will be accepted. RESPONSES SHOULD INCLUDE THE FOLLOWING INFORMATION: company name, address, and business size, point of contact name, phone number, and e-mail address, OEM Certifications for Technicians to provide the services must be included in the response. OEM documentation confirming access to all required OEM parts must be included in the response. Please note whether system is presently offered on a current Government contract. NAICS Code 811219 is applicable to determine business size standard. Any questions or concerns may also be directed to derek.crockett@va.gov via e-mail.
