- This RFI (Request for Information) is issued solely for information and planning purposes and does not constitute a solicitation. All information received in response to this RFI that is marked as proprietary will be handled accordingly. In accordance with FAR 15.201(e), responses to this notice are not offers and cannot be accepted by the government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. This notice is published to conduct market research to determine if there is a sufficient number of certified Service Disabled Veteran Owned Small Businesses (SDVOSB), Veteran Owned Small Businesses (VOSB), Small Businesses (SB), or Large Business capable of performing the requirements to warrant a socio-economic set-aside or full & open competition. It is strongly recommended that interested parties register in the US Department of Veterans Affairs Vet Biz database, the Small Business Administration (SBA) database and the System for Award Management (SAM), as applicable. Registration may be accomplished at http://www.vip.vetbiz.gov/, www.sba.gov/ and https://beta.sam.gov respectively. Interested parties responding to this sources sought request shall submit the following information at a minimum: (1) company name and address, point of contact with phone number, and DUNS number; (2) documentation relating to capability of performance (3) intentions of subcontracting program to include set-asides if any; (4) proof of registration in the VA Vet Biz or SBA websites or intention of registration prior to submission of offers to a solicitation; (5) If you are a distributor a statement from the manufacturer that your company is an authorized distributor; (6) a statement to inform the government if you are a manufacturer or distributor; (7) any information regarding FSS Schedule availability; (8) SAM Registration (9) any other pertinent company documentation. The response date to this Sources Sought notice is February 17, 2021 at 3:00pm MST. This market research is for informational and planning purposes only to determine if a socio-economic set-aside, or full & open competition, or a sole-source procurement is appropriate. The Government will not pay any costs for responses submitted. Electronic submissions are acceptable via nakia.vance2@va.gov, and are highly preferred. NO ADDITIONAL INFORMATION IS AVAILABLE OTHER THAN WHAT IS NOTED HEREIN. ALL FIRMS ARE INSTRUCTED NOT TO CALL THE INDIVIDUAL VA MEDICAL CENTERS REQUESTING MORE INFORMATION PERFORMANCE WORK STATEMENT FOR THE XENEX DISINFECTION WARRANTY EXTENSION PROGRAM PURPOSE. a) Phoenix Veterans Health Administration (VHA) purchased a total of nine (9) Xenex UV Disinfection Robots to disinfect rooms after terminal cleaning. A twelve month service, extension, includes repair plan for each Robot. Only Xenex Certified Technicians can service and repair the Xenex UV Disinfection Robots. SERVICE AND MAINTENANCE, TRAINING, CONSULTATION AND INFORMATION SERVICES. The Xenex Warranty extension covers Robot Service and Maintenance, Training, and Consultation and Information Services. Robot Service and Maintenance includes (i) replacement bulbs, (ii) replacement parts for maintenance and repairs covered by the Warranty Program to be performed by Phoenix VHA Xenex Certified Technician ( XCT ), who were certified under the purchase agreement of the robots, (iii) repair or replacement of the Robot if it does not properly operate and Phoenix VHA XCT are not trained or cannot perform the repair, and (iv) software upgrades. Training includes (i) operator training in the proper use of the Robot, (ii) staff retraining materials, and (iii) training and certification of Phoenix XCT. The Xenex Certified Technician (XCT) program is intended to train hospital engineering and/or facility maintenance technicians. The program has been built to train technicians to diagnose, repair, and test Xenex Germ-Zapping Robots. An XCT enables the hospital to achieve the highest level of up-time for the robots and creates the best overall experience for the hospital. Consultation and Information Services include (i) an Account Manager assigned to Phoenix VHA, (ii) 24/7 customer support line, (iii) development and implementation of the Confidential Standard Operating Plan ( SOP ), customized for our facility, (iv) access to the Xenex Disinfection Management System Portal, and (v) quarterly portal utilization reports.
