Description
INTENT TO SOLE SOURCE The Department of Veterans Affairs, Network Contracting Office 01 (NCO1) intends to sole source a contract for c repair of one Alcon Vision , Constellation Assembly at the West Haven Connecticut VAMC. All work is to be completed in accordance with statement of work and schedule. This sole source contract will be solicited and negotiated with, Alcon Vision, LLC, 6201 South Freeway, Fort Worth, Texas 76134, under the authority of FAR 6.302-1 Only One Source. The North American Industry Classification System (NAICS) code for this procurement is 811219-Diagnostic Imaging Equipment Repair and Maintenance Services. The Small Business Administration (SBA) size standard is $22.0 Million. Interested vendors that can meet this requirement, should identify their interest and capability, and respond to this notice by 02/11/2022 by 2:00 PM EST. Interested vendors must send a capability statement no more than one page in length; to include Company Name, Duns Number, Socioeconomic Category, brief description of your business entities capability to handle this particular requirement, website and GSA Contract number (if applicable) and/or other VA Medical Center past contract information. No solicitation will be made available. This is not a request for quotations. All information shall be provided at no cost or obligation to the Government. Responses must be sent via email to Contract Officer,  Deborah.Reardon @va.gov. If no responses are received, Contracting will proceed with the sole source negotiation with Alcon Vision, LLC. Statement of Work (SOW) Service Laser Replacement Parts Introduction (Provide a brief description of what you are buying). The Alcon Constellation LXT service laser has a port that is broken and causing multiple errors. Alcon has deemed the laser port is inoperable and needs to be replaced. The barcode scanner is also defective and cannot be fixed. Background (Provide any historical information necessary to understand how and why the requirement evolved and where it is headed. Include appropriate information to help the reader/contractor understand the requirement). The Constellation LXT is used for intraocular pressure control, high speed cutting, illumination, laser, and other features that give more surgical control for the surgical service at VA Connecticut Healthcare System. While conducting a routine preventative maintenance, the Alcon representative noticed that the service laser had an error message that had been appearing for over a year. The laser port was deemed inoperable after an attempt to repair it. In addition, the barcode scanner does not read the barcodes anymore. This unit is serving as a backup system and the service will not have a backup in service until these items are replaced. Make: Alcon Laboratories Inc. Model: Constellation LXT Serial Number: 1102502401X Scope (Provide an overview of the PWS emphasizing the most important aspects of your requirement and defining the boundaries of the contractor s performance responsibilities). Alcon Laboratories will provide new replacement parts for the Constellation LXT, including the laser and barcode scanner. Alcon will send a field service engineer to install the new parts. Alcon Laboratories will coordinate installation with operating room and clinical engineering. Alcon to provide any refresher user training needed by the end users, if needed. Applicable Directives (List all documents referenced in the next section 5.0, Performance Requirements, with which the contractor will have to comply in performance of the contract effort. If only portions of a given document apply, specifically identify the applicable portions). N/A Performance Requirements (Identify what the contractor is required to do (major tasks and subtasks). The contractor s responsibilities will include the following as described on Quote # 1192821: Delivery of Equipment Installation of Equipment Deliverables (List all data required to manage or monitor contractor performance). The contractor will provide all necessary operator and maintenance manuals. The contractor will complete installation of new replacement parts and verify equipment functionality.
