Description
Statement of Work Mental Health Anti Ligature Door Sensors Gen 3 Upgrade Specifications of Work A.1. The Contractor shall provide and install Top Door Alarm Gen 3 System in the Behavioral Health, 5 East Wing, of the Jack C. Montgomery VA Medical Center in Muskogee, Oklahoma. The Top Door Alarm System is an effective deterrent device that alerts caregivers to potential acts of suicide. A.2. The Contractor shall provide materials and installation for a Top Door Alarm System Upgrade to TDA Gen 3 to replace a 15 door TDA Gen 1 System. The contractor is to add two new doors to the system bringing the total alarmed doors to 17. A.3. The contractor shall provide certification, commissioning and testing of the system per manufacturers requirements and specifications. A.4. The contractor shall remove and dispose of the old Gen 1 materials and wiring. A.5. The contractor should provide the following materials for the system plus any other items necessary to have a complete and functional system: 17 Gen 3 Key Switches. VA to provide key cylinder and core. 2 full Surface Continuous Electrified hinges (2 Door Add) 2 top and bottom Beam Entry way kits (2 door add) 2 Multi-door hubs 1 Master j-box, local display / interface panel and hallway siren A.6. The following items are not required of the contractor and will be provided by the medical center: Anti-Ligature door handles, locksets, and/or any hardware not associated with the TDA System Conduit raceways for LV wiring Key cylinders and cores for reset switches (VA Facility provides these keyed to their keysets) Access panels required for work ICRA Barriers (Provided by facility) A.7. The Installation should include per system: Installation of remote panel Complete Alarm System Wiring Modification of doors to accept beams as required Modification of frames to accept Power transfer hinge and beams as required Installation of Hinges and Beams on door frames as required Installation of strobes, horns and key switches as required Repair of adjacent surfaces impact by work Clean-up and removal of debris associated with installation A.8. The following is excluded from the installation: Electrical Circuit to Power Main Control Panel, 110V, connected to emergency power panel in nearest electrical closet. Conduit, raceway or flex line will be provided by facility for Top Door Alarm Cabling Access panel installation if necessary Any door or door frame preparation not specifically associated with TDA system install Demolition and disposal of any existing anti ligature alarms. A.9. The Contractor shall provide written documentation of all work performed and notify the COR prior to the starting or stopping of any VA equipment. This is necessary to prevent any adverse effects on patient care or the mission of this facility. A.10. The Gen 3 TDA system is bid as installed as one complete system as described herein and in present condition. The bidder should inspect the system prior to submitting the bid, in order to be fully aware of the scope of services required. Failure to not inspect the system will not relieve the successful bidder from performing in accordance with the scope of work involved. A.11. All contracting personnel must report to the COR to sign in before and after work on the premises. After service has been completed, the contractor must sign out and furnish a detailed field service report. This report must be delivered to the Engineering Office before contractor leaves the station. Payment will be withheld pending receipt of this report. The service report from the contractor must contain, at a minimum, the following information: a. Date and time of the contractor s arrival on station b. Service report of actions performed c. Technician(s) name CERTIFICATION, STAFF TRAINING AND COMMISIONING B.1. The manufacturer requires all installations to be installed by a certified installer and commissioned by a manufacturer representative. (Three week minimum notice required for trip scheduling) B.2. Quote should provide for 1 trip and up to 16 hours of non-union Top Door Alarm certified support for staff training. ANNUAL CERTIFICATION C.1. As per manufacturer specifications, system will require an annual inspection and recertification minimum of once annually by a qualified manufacturer inspector starting one calendar year from installation certification. The VA will be responsible for these inspections. SECURITY D.1. The contractor shall be liable for security of all areas accessed by the service technician(s). The service technician(s) shall sign-out and return a key from the COR to obtain entry to required areas. All doors, passageways, windows and accesses to equipment and service areas must be locked and/or secured after passage. In no instance will any door, window or access be rendered unlocked, propped open, mechanism taped open or rendered to prevent securing. The contractor shall be responsible to physically ensure such accesses are secured to prevent unauthorized entry. D.2 The contractor shall abide by rules and regulations for the VA and any specific requirements set forth by the Chief of the service of the areas accessed. GENERAL REQUIREMENTS E.1. Federal Holidays: The ten (11) holidays observed by the Federal Government are: New Year s Day, Martin Luther King s Day, President s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veteran s Day, Thanksgiving, Christmas and any other day specifically declared by the President of the United States to be a national holiday. E.2. Invoices: Contractor shall submit monthly in arrears, a properly prepared invoice for service performed during the month. Only one (1) payment per month will be made. E.3. Evidence of Coverage: Before commending work under this contact, the contractor shall furnish to the Contracting Officer, a certificate of insurance indicating the coverage outlined and containing an endorsement to the effect that cancellation of, or any material change in the policies which adversely affect the interests of the Government in such insurance shall not be effective unless a 30-day advance written notice of cancellation of change is furnished the contracting officer. E.4. In lieu of the insurance coverage required, the contractor may furnish evidence of financial responsibility in the form of a qualified self-insurance program, an irrevocable letter of credit, or in a letter in which the contractor agrees to accept the financial responsibility and further states that he/she is financially able to meet all claims up to the amount specified. The furnishing of such evidence of insurance coverage or financial responsibility MAY NOT be waived. E.5. VHA Supplemental Contract Requirements for Combatting COVID-19 Contractor employees who work in or travel to VHA locations must comply with the following: Documentation requirements: If fully vaccinated, contractors shall show proof of vaccination. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a health care provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.  Contractor must immediately notify their COR or Contracting Officer if contract performance is jeopardized due to contractor employees being denied entry into VA Facilities. For indefinite delivery contracts: Contractor agrees to comply with VHA Supplemental Contract Requirements for any task or delivery orders issued prior to this modification when performance has already commenced.
