Description

Solicitation for Blanket Purchasing Agreement per�pricing schedule. These numbers are used as�estimates only and do not include delivery, fuel,�and hazmat fees. Estimates are based on prior�purchases not under contract. A COMPLETE QUOTE SHALL INCLUDE THE FOLLOWING INFORMATION: a. Sign and fill in contractor�s information on SF1449 Solicitation Form (page 1. Please be sure to include contractor�s DUNS and CAGE number). b. Pricing schedule below. Alternative Pricing Schedules will not be considered. Pricing shall take into account total cost of additional fees will be reduced if individual gas orders are combined into a single delivery. c. Fill out 52.204-24 (d) representation highlighted portion on page 18 d. Past Performance Information. Provide at least three (3) past performance references for recent and relevant work performed by the quoter within the last three years. Include project title, description, dollar value, dates performed, location, and contact information (name, title, valid email address, and phone). e. Proof of technical acceptability by providing A narrative explanation of how the product will be acquired, shipped, and delivered. In addition, the contractor must explain how they will guarantee the product throughout the timeframe under which deliveries will occur. Quotations will be evaluated in accordance with 52.212-2 Evaluation � Commercial Items
