Description
This is a Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Purpose and Objectives: Your responses to the information requested will assist the Government in determining the appropriate acquisition method. NAICS code: 485999 and size standard is $16.5M. NOTE: Respondent claiming SDVOSB and VOSB status must be registered, and CVE verified in VetBiz Registry www.vetbiz.va.gov. . DESCRIPTION/SPECIFICATIONS OF SERVICES The Contractor shall furnish services to beneficiaries for whom such care is specifically authorized by the Department of Veterans Affairs. The contractor shall furnish each Veteran authorized care under this contract with transportation and escort services in the form of non-skilled care. This includes transportation to a predetermined designated location to their scheduled procedure at Robley Rex VAMC. The contractor must remain on site throughout the duration of the Veteran s scheduled procedure. The contractor will provide a contact number to the unit which the Veteran is receiving care. Upon discharge from the admitting unit, the contractor will provide a return trip back to the designated location and ensure that the Veteran is safely settled in their home environment. QUALITY AND QUALIFICATION REQUIREMENTS The contractor must hold a current and valid driver s license to transport beneficiaries deemed eligible for Medical Companion Program services. The contractor must comply with all state traffic and safety laws. The contractor is responsible for providing vehicles to transport beneficiaries. Vehicles must be clean, neat, and free of potential hazards. Vehicles should be properly maintained, inspected, and repaired to ensure safe operating condition. The contractor shall employ 1 sufficient professional personnel to carry out the policies, responsibilities, and duties of the program. The vendor should have at least 1 vehicle and 1 back up available. Veterans need an escort the entire time that they are at the VA Medical Center. If a scheduled service cannot be fulfilled, the contractor should notify the medical companion program coordinator immediately so that other arrangements can be made. QUALIFICATION/PERSONNEL POLICY AND DOCUMENTATION The contractor will be responsible for their employees responsible for furnishing services under this contract. To carry out this responsibility, the contractor will provide the following for these personnel: Workers compensation Professional liability insurance Health examinations Income tax withholding, and Social security payment The employees, agents and subcontractors will not be considered VA employees for any purpose. Payment of any leave, including holiday pay, sick leave or annual leave is the responsibility of the contractor and is included in the per hour rate. The contractor will provide and document general orientation for all contract employees who are providing work on this contract before the commencement of work on site. Documentation of orientation will be provided to the VA COR. This orientation will include but not be limited to the following topics: Fire & Safety policy and procedures Infection Control policy and procedures Emergency Preparedness/Disaster policy and procedures TB Exposure Control policy and procedures HIPPA policy and procedures, and Initial Competency Assessment Contractor is required to develop and maintain the following documents for each contractor employee working on the contract: Credentials and qualification for the job; a current competence assessment checklist (an assessment of knowledge, skills, abilities and behaviors required to perform a job correctly and skillfully; includes knowledge and skill require to provide care for certain patient populations, as appropriate); a current performance evaluation supporting ability of the contractor employee to successfully perform the work required in this solicitation; and all listing of relevant continuing education for the last two years. The contractor will provide current copies of these records at the time of the contract award and annually on the anniversary date of the contract award to the VA, COR or upon request, for each contractor employee working on the contract. When changes occur in the contractor s personnel, the contractor must provide evidence of orientation and items requested above. Notwithstanding other contract requirements, upon request from the contracting officer, the contractor will remove from the work site any contact employee who does not comply with orientation requirements or meet competency requirements for the work being performed. TB Skin testing must be completed prior to employment and annually on all contractor s employees who have contact with VA patients. The contracting agency is responsible for developing a policy on TB exposure control and updating it annually. TB testing of employee must be conducted annually, and a copy of the proper paperwork must be submitted with the annual review of the TB exposure control policy. Information sought: Response to this announcement shall not exceed 10 pages and should include the following information: (1) A tailored capability statement indicating the firm s ability to provide a product and related services; (2) The respondents DUNS number, organization name, address, point of contact, and size and type of business (e.g., SDVOSB/VOSB, etc.,) pursuant to the applicable NAICS code; (3) Information should be provided electronically in a Microsoft Word or Adobe PDF format.
