- Description
- The Defense Health Agency intends to award a sole source contract under the authority of FAR 13.106-1(b) to ForTec Medical, Inc., 10125 Wellman Rd, Streetsboro, OH. 44241 for the performance of the described requirement in support of the Naval Medical Center, Portsmouth, Virginia. The requirement intends to provide the provision, maintenance, and transportation of lasers and the staff to provide the operation of lasers to be used in the Directorate for Surgical Services (DSS) at the Naval Medical Center Portsmouth. � Service will be ordered on an as needed basis; the Government will provide a list of scheduled procedures to the Contractor generally 15 days prior to the requirement for service. The Contractor shall recognize that the schedule provided may change within those 15 days. The Government estimates that about one (1) time per year, 24-hours will be the maximum notice given to the Contractor. The Contractor must have the ability to provide full on-site support with a minimum 24 hour notice.� The Government reserves the right to request services in alternate treatment rooms located within the MTF proper. The Government estimates forty (40) sessions for Technician Services, two-hundred sixty five (265) sessions for Laser Support Services (with specific lasers called out), and a disposables itemized list. The Contractor shall provide laser system specialists to support laser procedures in the Medical Treatment Facility (MTF) DSS. SPECIFIC QUALIFICATIONS FOR LASER SUPPORT SERVICES The laser operators shall have completed an Associate in Science in Laser Technology program that included laser physics, laser safety administration, laser delivery systems, laser utilization in surgical procedures and laser treatment modalities. Training of all operators shall be equivalent to the recommendations from ANSI Z136.1 �Safe Use of Lasers� and ANSI Z136.3 �Safe Use of Lasers in Health Care Facilities Standard.� The contractor shall be current with and have completed all continuing education requirements specified by their professional licensure or certification. The Contractor shall continuously maintain a current list of all individuals who are providing services under this contract. The Contractor shall provide a copy of the updated list to the COR monthly, or more often as needed to keep the information current. The laser system specialists shall provide support for the following medical procedures.� Colposcopy Condyloma ablation Dermal ablation Laparoscopic laser ablation Lithotripsy Laser Laryngeal Microsurgery/ Microlaryngoscopy Photovaporisation Stapedectomy Tympanomastoidectomy Tympanoplasty Vein laser therapy Vulva ablation The period of performance for this contract is anticipated to be twelve (12) months. This notice of intent is not a request for competitive proposals and no solicitation document exists for the requirement. Sources interested in responding to this notice are required to submit a capability statement that includes management and technical data and cost information, in sufficient detail and with convincing evidence that clearly demonstrates the capability to perform the required work. Capability statements shall not exceed 1 (8.5 x 11 inch) page using a font size no smaller than 10-point. All capability statements received by the due date of this notice will be considered by the Government. A request for documentation or additional information will not be considered as an affirmative response. A determination by the Government not to compete based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement or to proceed with a sole source contract.
