Description
1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.� 2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) FAC 2022-04 Effective January 30, 2022, the complete text of any of the clauses and provisions may be accessed in full text at www.acquisition.gov/far/index.html 3. This Solicitation is unrestricted.� The associated North American Industry Classification System (NAICS) code for this work is 621399. The small business size standard is $8 million. Due Date: All offers shall be submitted by 4:30PM EST 2/25/2022 to Alicia Powell at Apowell@wapa.gov Facsimile are not authorized under this announcement. DESCRIPTION: Western Area Power Administration (WAPA) employees shall be provided a physical in accordance with the requirements of WAPA�s Medical Order 340.1, Federal Employee Occupational Medical Program (attachment A list of employees authorized to obtain an occupational health exam as a result of this purchase order will be provided to the medical provider in advance.� The list also identifies those individuals whose physical examination testing must meet the requirements for a commercial driver�s license (CDL) and/or medical DMV certificate for their job classification. The Occupational Medical Physician will provide services as a Medical Advisor.� As such, the Medical Advisor will assist, on a case-by-case basis by providing medical services in worker�s compensation reviews, accommodation reviews, annual flu shot for at around 25 employees, fitness for duty reviews, Automated External Defibrillator review, and other medical opinions and reviews as called upon. All work shall be completed in accordance with the Performance Work Statement (PWS). The PWS is included as Attachment 1 to this solicitation. 5. Period of Performance is anticipated to be as follows: Base year: Option Year 1 Option Year 2 Option Year 3 Option Year 4 6. Offerors are requested to submit a quote for the line items on the Schedule of Services; Attachment PWS 7. QUESTIONS Submit technical questions to the Contract Specialist in writing, via email to Alicia Powell at apowell@wapa.gov on or before Noon EST 2/25/2022. Telephonic (verbal) questions Will Not be addressed. All responses to questions, which may affect offers, will be incorporated into a written amendment to the Request for Quote. PROVISIONS AND INSTRUCTIONS TO OFFERORS Addenda to 52.212-1 Instructions to Offerors -- Commercial Items ((Nov 2021)) applies to this acquisition. QUOTE SUBMISSION Quotes shall be sent electronically via e-mail to Alicia Powell apowell@wapa.gov All quotes shall include one (1) separate copy of each of the items listed below. Submission of a quote that does not contain all items requested below may result in elimination from consideration of award. Offerors must submit each item of quote in a separate electronic attachment clearly identified as described below. Offerors are responsible for including sufficient details to permit a complete and accurate evaluation of each quote. Pricing: Pricing shall be submitted as requested in the Schedule of Services.� Total price will be evaluated by the Government. Proposed price should reflect a clear understanding of the requirements, is consistent with the various elements of the offer.� Section B Offerors Representations and Certifications:� A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically).� An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at www.sam.gov/portal/public/SAM/. All Offerors shall be registered and current in the System for Award Management (SAM) database. Technical Proposal: Offeror shall submit company's detailed technical approach and methodology for completing all requirements of the Performance Work Statement (PWS). The Technical Volume shall not exceed thirty (5) pages in length, minimum 12-point font.� Page limitation includes any drawings, charts, pictures, etc., and excludes section dividers, table of contents, figures/tables, glossary of terms and cross-referencing indices and resumes -all of which are not considered content pages. Past Performance Record: Offerors shall provide contract data for three (3) relevant PRIME contracts and relevant SUBCONTRACTS that the offeror is currently performing or has completed during the last five (5) years. The following information shall be included: Contractor Name and Contract Number; Customer points of contact (minimum of two; provide name, telephone number, fax number and e-mail addresses); Length of contract to include Period of Performance (dates); Initial Contract Price; Description of product/service provided (include sufficient detail to demonstrate similarities and dissimilarities to the requirements of this solicitation) BASIS FOR AWARD The Government will award a firm fixed priced contract to the responsible offeror whose offer, conforming to the requirements in this Request for Quote (RFQ) will be most advantageous to the Government, price and other factors considered.� A best value award will be made based upon the following factors: Technical, Past Performance, Socio-Economic Status and Price. 52.252-1 SOLICITATION PROVISIONS INCORPORATED BY REFERENCE (FEB 1998) This solicitation incorporates one or more solicitation provisions by reference, with the same force and effect as if they were given in full text.
