SOW Document UNIT ITEM DESCRIPTION 1EA 251033 ExacTVu- Micro-Ultrasound Imaging System (120V) with Single EV29L Transducer 1EA 251129 FusionVu micro-US/MRI Fusion Application 1EA 251035 EV29L Transducer - 29 MHz High Resolution Transrectal Sidefire Transducer (Linear) 1EA 251036 EV9C Transducer - 9 MHz End-fire Prostate Endocavity Transducer (Curved) 1EA 251037 EV5C Transducer - 5 MHz Abdominal Transducer (Curved) including Color Doppler and Power Doppler 1EA 251042 Complete ExactVu Standard Transperineally Package, including Steppe 1EA 20-0929 CORTRAK Enternal Access System Printer TRAINING Training is intended for 2 physicians, with 1 training session (hereinafter called Training""). A good knowledge of prostate endorectal ultrasound practice is necessary as a prerequisite to the Training. If the User wishes to train an additional doctor, the invoiced amount will be $5,000.00 not including taxes nor transportation costs. Training sessions will take place in the following manner: a) E-learning: Before the first session at the site, the physicians responsible for treatments will have access to Exact Imaging s E-learning modules. Completion of all modules is required before first biopsies are performed. b) On-site Training Sessions: During the first sessions on the ExactVu , an Exact-certified application specialist will assist the two physicians during a minimum of 10 biopsy exams each. The procedures will have to be scheduled over 2 sessions of 2 consecutive days each. In the event an additional visit from an application specialist is requested by the User, it will be billed at a set price of $5,000.00 not including taxes nor transportation. c) Beyond the On-site Training Session, a Quality Assurance (QA) program will be offered to the User. This program will: Ensure all users achieve optimal results with the Micro-Ultrasound system Receive a certificate as Micro-Ultrasound expert by Exact Imaging Drive publications efficiently based on the collected clinical data Be listed in the physician finder on Exact Imaging s website for this final training session, the User shall provide de-identified pathology, imaging, and targeting data to Exact Imaging (subject to separate data sharing agreement). Exact Imaging will perform timely analysis of 9 metrics related to improving detection rate, provide feedback report with comparison to other centers and personalized support to improve biopsy yield, and provide support when the institution decides to publish. Operators who successfully complete all requirements of the Quality Assurance program, will receive a certificate indicating their expert level in the use of the ExactVu Micro-Ultrasound system. II EXACTVU TRAINING OF THE NON-PHYSICIAN STAFF One or more staff members (nurse and/or technician) will have to be designated by the Buyer to be present at the on-site training session. These persons will have to be trained by the Application Specialist on setting-up the equipment, basic operation of the system, cleaning, and disinfecting. Capability Requirements: 1) Supporting both a high frequency (29 MHz) and a conventional frequency (~ 3 12 MHz) transducer 2) Being the only commercial ultrasound system for clinical urological applications that operates at 29 MHz and enables resolution down to 70 ยต 3) Providing a comprehensive, evidence-based risk indication protocol called PRI-MUS to help urologists identify and stratify the possible severity of micro-ultrasound-based characteristics of the prostate 4) Providing a system interface (software and hardware) and workflow that is optimized for urology 5) Expanding to facilitate micro-ultrasound/MRI fusion on the same plat 6) Providing the only cognitive biopsy assistant to help urologists perform cognitive fusion imaging benefitting from the highest resolution micro=ultrasound live imaging aligned with anatomical MRI targets 7) Enabling transrectal or transperineally prostate biopsy with no need for additional hardware or software pack.
