- 1. BACKGROUND. The VALLHCS Community Living Center Palliative Care area has (20) Guldmann GH+ ceiling mounted patient lifts. Each of these ceiling lifts are equipped with electronic combi-locks, which are used to at the transition between a room-covering rail system and a single-track rail system and allows the patient to be safely transported from one room to another. The electronic combi-lock is designed to remotely align two rails so that the patient can be transported into an adjoining room, bathroom, etc. The caregiver has encountered multiple failures, and on many occasions the electronic combi-lock are either inoperable, or not operating as it is intended, causing the patient lift system to be obsolete. This is a safety issue to both the patient and the caregiver. It has been determined by FMS, Patient Safety and Safe Patient Handling that the corrective action to reduce the chance of failure would be to convert the patient lift combi-locks from electronic to manual. 2. STATEMENT OF WORK. The Contractor shall provide all labor, equipment, materials, tools and supplies to perform the required services for the Department of Veterans Affairs, Loma Linda Healthcare System (VALLHS), 11201 Benton Street, Loma Linda, CA 92357 to remove the (20) Guldmann GH+ electronic combi-locks and replace them all with Guldmann GH+ manual combi locks. The contractor shall be responsible for any drilling, any cutting, any adjustments or other alterations of the lift system to accommodate the installation of the manual combi-locks. The contractor shall be responsible for any electrical work associated with installing the manual combi-locks. The contractor shall be responsible for ensuring that the lift system operates properly once the manual combi-locks have been installed. 3. SPECIFIC REQUIREMENTS: a. The Contractor shall schedule with Contracting Officer Representative (COR) or Designee (48) hours in advance prior to start of work. b. The Contractor shall, prior to start of work, ensure that all Contractor personnel have a valid Loma Linda PIV badge flash badge. c. The Contractor shall, prior to start of work, ensure that all energy sources (electrical, water, etc.) are locked and tagged out, and upon completion of work, will be re-energized and verified. d. The Contractor shall, prior to work each day, ensure that all workers or a designated lead or supervisor check in with the Mechanical Supervisor, or the Mechanics Shop located in room 1D-02. e. The contractor shall provide a report of services provided. f. The contractor shall provide all required licensed and certified labor, technicians, equipment, materials, tools, transportation, supervision, safety requirements, and personal protective equipment (PPE). g. The Contractor shall conform to the Occupational Safety and Health Administration (OSHA) regulations, National Electrical Code (NEC), National Fire Protection Association (NFPA)70, and Interim Life Safety Measures (ILSM). h. The Contractor shall be responsible for clean-up, safe removal, and off-site disposal of items used during the project. This includes, but is not limited to, clean up materials used during the project, any demolished materials, trash, etc. i. The Contractor shall perform work in accordance with existing, relevant standards and codes to minimize the possibility of injury to personnel, damage to government property/equipment. j. The Contractor will not damage existing infrastructure and/or healthcare system property/spaces (e.g., surfaces, stairwells, corridors, panels, walls, elevators, service areas, floors, carpet, road, pavement, etc.), including but not limited to, duct work, door frames, doors, electrical work, conduit, low voltage conductors, piping, CAT V cable, drywall, flooring, ceiling, plumbing work, lighting, and/or blocking ingress/egress routes. k. Damage to government property may result in additional action. l. Every effort will be made to safeguard and care for all human life and property. m. The Contractor shall conform to following standards, policies, regulations, governing agency, and any other local, state, or Federal guidelines that are not listed here. OSHA Regulations, www.osha.gov NFPA, www.nfpa.org ALL supporting documents will be provided via electronic and hard copy to: CHIEF, FACILITY MANAGEMENT SERVICE FMS VALLHCS 11201 BENTON STREET LOMA LINDA CA 92357 4. HOURS OF OPERATION: a. Normal Work Hours: The normal work hours at VA Loma Linda Healthcare System, 11201 Benton Street, Loma Linda, CA 92357 is Monday through Friday, 7:00 AM to 3:30 PM. NATIONAL HOLIDAYS: The Contractor is not required to provide services on the following National Holidays. New Year s Day 1st January Martin Luther King s Birthday Third Monday in January President Day Third Monday in February Memorial Day Last Monday in May Juneteenth National Independence Day 19th of June Independence Day 4th of July Labor Day First Monday in September Columbus Day Second Monday in October Veterans Day 11th of November Thanksgiving Day 4th Thursday of November Christmas Day 25th of December Also included would be any other day specifically declared by the President of the United States to be a National Holiday. When a holiday falls on a Sunday, the following Monday shall be observed as a legal holiday by U.S. Government agencies. When a holiday falls on a Saturday, the preceding Friday shall be observed as a legal holiday. 5. PERIOD OF PERFORMANCE: All work must begin within (30) days of contract award. Work will be done during working hours; specifically, Monday through Friday from 7:00 am - 3:30 pm. Work will be coordinated with all required parties, including, but not limited to; Contractor, VALLHCS, Green Environmental Management System (GEMS) Coordinator, Safety, Contracting Officer Representative (COR), Chief, Maintenance and Operations Chief, and the Structures/Mechanical Supervisor. Work timelines may be subject to change due to VALLHCS operational requirements. Contractor shall be able to perform work with no additional charges within (60) days of contact by the designated VALLHCS Contracting Officer Representative (COR). 6. PLACE OF PERFORMANCE: VA Loma Linda Medical Center 11201 Benton Street Loma Linda, CA 92357 7. LICENSING REQUIREMENTS: Offeror must possess a 30-hour Occupational Safety and Health Administration (OSHA) safety card. 8. OTHER REQUIREMENTS: a. Security: The Contractor shall comply with all Government security requirements. The Contractor shall submit a list of all employees hired to perform the work specified in this contract to the Contracting Officer Representative (COR). b. VA Policy: Possession of weapons is prohibited. Enclosed containers, including tool kits, are subject to search. Violations of VA regulations may result in citations answerable in the United States (Federal) District Court. c. Parking Policy: It is the responsibility of Contractor employees to park only in designated parking areas. Parking information is available from the VA Police. The VA shall not validate or make reimbursement for parking violations of the Contractor s employees under any circumstances. d. Smoking Policy: Smoking is strictly prohibited on the grounds of any VHA facility. Per VHA Directive 1085 dated March 5, 2019, it is VHA policy that all VHA healthcare facilities are smoke free. e. Invoice and Payments: Contractor shall submit all invoices electronically via the Tungsten-Network System in arrears for payment of work completed in that billing period. Contractor shall submit original invoice electronically to: Department of Veterans Affairs Financial Services Center http://www.tungsten-network.com (Toll-free telephone number: (877) 489-6135) Payment shall be made in arrears upon receipt of a properly prepared invoice. The invoice SHALL be itemized to include the following information. Any information listed below that is not provided on an invoice, will render that invoice incomplete and the invoice will be returned for immediate correction. Invoice number and date. Contract Number. Obligation Number. Dates of services/Period of Performance. Description of services. Unit Cost billed. Extended amount due. Invoices sent to other than the above e-mail address and failure to comply with the above information shall delay payment to the Contractor.
