THIS IS A REQUEST FOR INFORMATION (RFI)/SOURCES SOUGHT DISCLAIMER This RFI/Sources Sought is issued solely for information and planning purposes and does not constitute a solicitation. All information received in response to this RFI/Sources Sought that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI/Sources Sought. The submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and allowed under this RFI/Sources Sought in accordance with (IAW) FAR Part 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is an RFI/Sources Sought. The purpose of this RFI/Sources Sought is to gain knowledge of potential qualified sources and their size classification relative to NAICS 325413, In-Vitro Diagnostic Substance Manufacturing (unless another NAICS would be more appropriate, and that shared information is welcome). Responses to this RFI/Sources Sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this RFI/Sources Sought, a solicitation announcement may be published. Responses to this RFI/Sources Sought synopsis are NOT considered adequate responses for a solicitation announcement. The information identified below is intended to be descriptive, not restrictive, and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the Government that the interested parties can provide the Services that fulfill the required specifications. If you are interested and are capable of providing the RFI/Sources Sought for services, please provide the requested information pursuant to the following questions: Response to this RFI/Sources Sought should include company name, address, and point of contact to include E-mail address and phone number. Include also, your SAM Unique Entity ID, DUNS number, and Cage Code. Please indicate the size status and representations of your business, such as but not limited to: Service-Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc., Is your company considered small under the NAICS code 621399 identified under this RFI/Sources Sought, Does your company have a Federal Supply Schedule (FSS) contract with GSA, VA NAC, NASA SEWP, or any other Federal contract, that can be utilized to procure the requirement listed in this RFI/Sources Sought? If so, please provide the contract number. Please provide general pricing of your solution for these supplies, for market research purposes only, e.g. Catalog list prices, FSS prices, or any other publicized price lists are applicable/acceptable, Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached State of Work (SOW). Responses to this notice shall be submitted on or before March 22, 2022 by 11:00 a.m. Central Time Zone. Responses to this notice shall be submitted via E-mail to Teresa Cabanting, Contract Specialist, at teresa.cabanting@va.gov and copy furnish to Jennifer Sotomayor, Contracting Officer, at jennifer.sotomayor@va.gov. Telephone responses shall NOT be accepted. All questions for this RFI/Sources Sought are to be sent via E-mail to Teresa Cabanting and copy furnished to Jennifer Sotomayor. Telephone questions will NOT be accepted. If a solicitation is issued it shall be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this RFI/Sources Sought. Responses to this RFI/Sources Sought notice are not a request to be added to a prospective offerors list or to receive a copy of the solicitation. VA Heartland Network - VISN 15 STATEMENT OF WORK Microbiology Products DESCRIPTION/SPECIFICATIONS It is the intent of VISN 15 to establish a Firm-Fixed-Price, 12 Month Base Period with four, 12 Month, Option Periods type contract for Micro-biology Supplies to include but limited to BRAND NAME only REMEL media, organisms, test kits and general testing and supplies for use within the Microbiology laboratory. Attachment A, List of Supplies provides a listing of the above-mentioned supplies and there are some specific line items that are Brand Name only by REMEL, (There are other supplies listed that are not Brand Name required). The resulting contract will cover multiple locations within VISN 15. KCVAMC will provide an estimated volume by supplies as reflected in Attachment A for each individual Medical Center and Outpatient clinic laboratory. The Government estimates the volumes per facility as listed in Attachment A but does not guarantee volumes as listed; they are only estimates. The participating facilities may order products via Electronic Data Interchange (EDI), telephone, facsimile or other written communication, identifying the products by number, quantity, purchase price, address for delivery, and any special instructions. Unless otherwise agreed, all deliveries shall be accompanied by delivery tickets or sales slips that shall contain the following information as a minimum: Name of Contractor Contract number Model number or National Stock Number (NSN) Purchase order number (as applicable) Date of purchase Quantity, unit price, and extension of each item (unit prices and extensions need not be shown when incompatible with the use of automated systems, provided that the invoice is itemized to show the information) Date of shipment The delivery terms for all items orders shall be FOB Origin. Contractor shall make shipping arrangements and prepay all shipping and handling costs. Contractor shall promptly replace all products lost or damaged in shipment. Contractor shall be responsible for all disputes with the shipper and all insurance claims related to the shipment. Contractor shall accept return of erroneously shipped products, in accordance with the following: Contractor shall credit Customer 100% of the Purchase Price for products returned due to shipping errors, provided that Customer requests a Return Material Authorization (RMA) number from the Contractor in its notice to Contractor of a shipping error, and that Customer returns the products within 30 days after its receipt of a RMA number. Returns shall have an RMA number written clearly on the outside of the shipping container. Contractor shall not accept returns of products ordered in error by Customer. SCOPE OF PROCUREMENT: The desired supplies/reagents shall have the capability of performing as defined in the statement of work. The supply/reagent shall be acquired for each of the clinical laboratories located at the VISN facilities listed in Attachment A. The Contractor is required to provide a continuously stocked inventory of supplies/reagents and controls required to ensure that patient care operations are not interrupted. These items shall be of the highest quality, sensitivity, specificity and tested to assure precision and accuracy. Expiration date must be clearly marked on supply/reagent, standards and control containers. Alert/Notification of any delays in shipment as well as any or all technical advisory/recalls/alerts, prior to or simultaneously with field alerts should be forwarded to the designated individuals determined at contract award. Special handling for emergency orders of supplies: In the event that the supplies are found to be defective and unsuitable for use with the Contractor s equipment, or the Contractor has failed to comply with the requirements for routine supply delivery, the Contractor is required to deliver the supplies within 24 hours of receipt of a verbal order for emergency delivery. If either circumstance has occurred, the Contractor shall deliver to the Government site in the most expeditious manner possible without additional cost to the Government, the necessary consumables in sufficient quantity as required to allow operation of the Contractor s equipment for one week (under normal Government test load volume). If additional requests for emergency supply delivery are required by the Government, they shall be honored by the Contractor until the arrival at the laboratory of the of the monthly standing order/routine supplies delivery. REAGENTS: Refer to Attachment A for desired reagent menu and estimated annual volumes. This solicitation intends to use a Brand Name or Equal Description of the product required. 1.1 GENERAL REQUIREMENTS: Technical Features: REMEL Products shall be manufactured by REMEL. Media Media should be tested by the manufacturer prior to shipment using Clinical Laboratory Standards Institute (CLSI) M22 standard to evaluate sterility, growth, selectivity and or inhibition and biochemical response. Certificate of Quality should be provided with shipment of prepared media. Two packing slips will be sent with each shipment. Packing slips will contain the following information: Reagents lot number and expiration date Space to document visual inspection by receiving laboratory Susceptibility Disks Susceptibility disks must have antibiotic code and concentration printed on each disk Zone disk size must conform to CLSI guidelines Cartridges of disks must be shatterproof, and spring loaded Organisms Organisms must be ATCC certified strains that can be used as quality control for rapid identification, minimum inhibitory concentrations of antibiotics and antibiotic susceptibility panels Reagents and other testing supplies Products listed in Attachment A should be name brand or equivalent
