- STATEMENT OF WORK ZEISS OS CIRRUS HD-OCT 5000 Brand Name INSTALLATION, DELIVERY AND TRAINING STATEMENT OF WORK PART A GENERAL INFORMATION A.1 INTRODUCTION- This contract is for the, installation, delivery and training of one (1) CADIODAY V2.5 HOLTER ANALYSIS SYSTEM for the VA Butler Healthcare located at 353 North Duffy Rd. Butler, PA 16001. A.2 BACKGROUND- The Butler VA Medical Center Health Care Center (HCC) has a requirement for one The Butler VA Medical Center Health Care Center (HCC) has a requirement for one (1) Zeiss CIRRUS HD-OCT (Model 5000) to be purchased for the Butler Health Care Center. This unit is outdated and in need of upgrading. Currently the CIRRUS 6000 is on FDA hold. The company has agreed to offer the CIRRUS 5000 with a Protection Program that will allow the VA priority status upon it s release as well as full trade-in value. Upgrade includes: The ZEISS OS CIRRUS HD-OCT (Model 6000) with AngioPlex has the capability to capture and provide OCY Angiography. This capability for OCT Angiography is provided by a 1 single OCT scan that allows for visualization of both micro vascular and structural information in a non-invasive, dye-free, scan by the OCT. It has the ability to import and integrate the Cirrus Model HD OCT 4000 raw data. The raw data must be interchangeable between the Cirrus 4000 and Cirrus 5000 unit (or new unit being procured). The unit must have exact Macular Scan Pattern of 512x128 for retina scan with exact scanning speed at 27,000 A-scans a second within a 6mmx6mm. The unit must have exact scan pattern of 200x200 in a 6mmx6mm cube for Glaucoma Scan with exact scanning speed at 27,000 A-scans a second. The scan pattern must be able to capture and provide RNFL, ONH, Neuro Retinal Rim Tissue, and 3-D Cube capabilities all in one scan Scanning speeds: Scan at exactly 27,000 A-scans/sec, as well as the ability to scan at 68,000 A-scans/sec. A.3 SCOPE OF WORK The current model (OCT 5000) is unable to be upgraded to Windows 10. The VA Medical Centers are in the process of upgrading to Windows 10 which will include medical equipment. The facility attempted to purchase the CIRRUS 6000, however the company did a voluntary recall on the product. They are offering a Protection Program for anyone that purchases the model 5000 to receive priority upgrade with full trade-in value upon release. The purchase of an OCT machine in this outpatient clinic, ensures patients will be cared for in a more patient centric fashion, will lend to continuity of care and lack of delay in care. The upgrade to the York unit will assure all patients are receiving the same standard of care at all facilities. STATEMENT OF WORK PART B WORK REQUIREMENTS B.1 The vendor: B1.1 Will provide delivery of one (1) ZEISS CIRRUS HD-OCT (Model 5000) to the VA Butler Healthcare Center at an anticipated delivery date shall be in March coordinated between vendor and the VA Butler Healthcare when a suitable installation date can be established based on the VA Butler Health Care Center construction schedule. B.1.2 The CardioDay V2.5 Holter System must come with the following specifications: B1.2.1 Optical source - Superluminescent diode (SLD), 840 nm B1.2.2 All raw patient data from the CIRRUS family of products can be transferred into the CIRRUS 5000 seamlessly and used immediately for Guided Progression B1.2.3 Analysis or any other CIRRUS analytical program B1.2.4 A-scan depth - 2.0mm (in tissue), 1024 points B1.2.5 Axial resolution - 5 um (in tissue) (Bscan resolution) B1.2.6 Transverse resolution - 15 um (in tissue) (Enface resolution) B1.2.7 Anterior Segment Imaging: 15.5 mm in length x 5.8 mm in depth. B1.2.8 Global Pachymetry Scan: 24 radial scans at minimum 9mm in length. B1.2.9 Global Pachymetry: Thickness in each of the area of central 0-2mm, 2-5mm, and 5-7mm range. B1.2.10 Progression Analysis: 2 Baseline 7 additional scans for comparison B1.2.11 Retina Scan Patterns: B1.2.12 HD 1 Line 100x 1 Line (100x averaged) B1.2.13 HD 21 Line 21 Lines (4 or 8x averaged) B1.2.14 HD Radial 12 Lines (8x averaged) B1.2.15 HD Cross 10 Lines - 5 horizontal, 5 vertical (8x averaged) 512x128, 200x200 B1.2.16 Anterior Segment Premier B1.2.17 ChamberView 15.5 mm x 5.8 mm B1.2.18 Wide Angle to Angle 15.5 mm x 2.9 mm B1.2.19 HD Cornea 9 mm x 2 mm B1.2.20 HD Angle 6 mm x 2.9 mm STATEMENT OF WORK C GENERAL REQUIREMENTS C.1 For the delivery of the purchased equipment to its final installation destination, the vendor Shall adhere to the job site requirements listed below: C1.1 Vendor responsible for unloading, handling, unpacking; clean up dumpster provided by GC C.1.2 Vendor to schedule deliveries through VA who would in turn schedule with GC. At the time of these deliveries, most if not all products will have to go through the loading dock, so a schedule will be arranged for dock use. C.1.3 VA to obtain and maintain current certificates of insurance for each vendor. C.1.4 Vendor responsible for protecting product after installation STATEMENT OF WORK PART D SUPPORTING INFORMATION D.1. Place of Performance VA Butler Healthcare Medical Center and/or Health Care Center (hereafter referred to as, The VA , VAMC , or HCC ) D.2. Period of Performance Period covers installation and verification/testing of operations to ensure the equipment operate as marketed. D.3. Special Considerations D.3.1. Contractor Furnished Materials and Services D.3.1.1. Equipment to transport equipment (e.g., dollies, pallet jacks, etc.) D.3.1.2. Tools necessary to finalize installation of equipment (e.g., installation of casters, setup of shelving) D.3.2. Government Furnished Materials and Services D.3.2.1. Elevator access, power, and as optimal an operating environment as can be reasonably achieved. D.3.3. Qualifications of Key Personnel Each party will determine the level of skills and adequate training for personnel supplied. Butler VA has a CIRRUS 5000-31800 that will be utilized as a turn in. the Vendor should provide a Trade-in credit for the return of the CIRRUS 5000-31800. With all new CIRRUS 5000 purchases, Zeiss is offering a Protection Program. This Protection Program provides the Government with a significant trade-in value for any new CIRRUS 5000 ordered after July 12,2021. This trade-in credit will then be put towards the next version of Zeiss s OCT, once it s released. Details on the Protection Program can be found on the last page of the first attachment. Additionally, customers will also have the option to add a Technology Upgrade to their order. The Technology Upgrade option guarantees the Government priority order allocation, delivery, installation and patient data transfer from the CIRRUS 5000 in connection with this transaction to the next-generation OCT for those customers who elect to purchase this option. With this program, you will essentially be receiving two years of warranty at no additional cost. You will get the year of warranty on the 5000 during the bridge period, and then you will receive the manufacturers warranty from the date the new unit is installed. During these two years, you will receive any free updates Zeiss comes out with for the new device. If there are any upgrades that require additional software or parts, those would be separate under the warranty coverage. On that point, with this program, any new reports, software, or upgrades Zeiss makes to the new OCT while the 5000 is in the office will be included with the installed unit at no charge. D.3.4. Contractor s Statement of Release - In consideration of the modification agreed to herein as complete equitable adjustment, the Contractor hereby releases the Government from any and all liability under this contract for further equitable adjustments attributable to this modification. D.3.5. VA Butler Healthcare will provide a secure area for operation of the purchased equipment.
