- Released pursuant to Federal Acquisition Regulation (FAR) Part 10: Market Research, this is a Request for Information. Purpose: Fayetteville, Arkansas VA Medical Center | Radiology Room 1156 Site Prep | Project# 564-22-109 In an effort to enhance opportunities for Service Disabled Veteran Owned Small Businesses (SDVOSB) as required by Public Law 109-461, specifically the Veterans First Contracting Program, NCO16 is performing preliminary market research to identify interested, qualified, capable, SDVOSB and VOSB firms possessing the capacity necessary to compete for a VHA procurement opportunity. In case no SDVOSBs or VOSBs possess the capacity, all Small Businesses possessing the necessary capacity are encouraged to provide a response. Results of the preliminary market research will be used to determine the potential future solicitation s set-aside requirement. This source sought notice is issued solely for informational, market research, and planning purposes. It does not constitute an Invitation for Bid (IFB), Request for Quote (RFQ), Request for Proposal, or a promise to issue a subsequent RFQ/IFB/RFP. Currently, no solicitation document exists. Issuance of this notice does not establish any obligation on the part of the Government and is not to be construed as a commitment by the Government, implied or otherwise, to issue a solicitation or award a contract. In addition, respondents are advised that the United States Government is under no obligation to pay for any information or administrative cost incurred in response to this notice. At this time, proprietary information is not being requested. Respondents shall refrain from including proprietary information in responses to this notice. The Department of Veterans Affairs anticipates awarding a one-time, Firm-Fixed Price (FFP) single award contract. Project Scope Summary: The Contractor shall provide all labor, equipment, materials, testing, site investigations, supervisory, and professional services to perform all work associated with a radiology room site prep for new Philips equipment following the manufacturers site planning requirements. Once the project begins the site will require the work to continue until completion. Statement of Work for Digital Radiography (DR) Turnkey Room 21-1156 The purpose of this scope of work ( SOW ) is to define the extent of the Turnkey engineering, procurement and contracting work required to complete the project described above. Anything not specifically included by mention in this description is excluded from the agreed upon SOW. In the event of a conflict between the work described in the SOW definition set forth below, and the supplemental documents attached to this Turnkey Contracting Proposal, the SOW shall govern. The SOW should be thoroughly reviewed by all involved parties to ensure that all areas of concern are addressed, as the items described therein shall govern execution of the project described herein ( Project ). Additional items not addressed in this proposal may be included in the project but are subject to negotiation. Contractor shall perform these services in accordance with the TIL - Master Construction Specifications (PG-18-1) which can be found at https://www.cfm.va.gov/til/spec.asp. This statement of work is specifically based on Philips Site Planning drawing # N-SOU190606 subject to all clarifications stated herein. Scope of Work All other existing millwork located throughout the project area will remain in existing condition and location. All existing doors, frames, locksets, keying, kick plates, closers, hinges, stops, bumpers, guides, coordinators, etc. are existing and to remain undisturbed. All existing domestic water supply piping, valves and fittings, domestic and storm water waste piping and fittings to remain. The existing fire alarm components and system are to remain in the current condition and location without additions or modifications. DIVISION GENERAL REQUIREMENTS Includes the following: The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. Maintain a job site office area. Keep a current and up to date copy of the construction documents in the job site office, marked with red lines for all changes that occur during the work. Provide all required shop drawings and submittals and keep a copy of all approved shop drawings and submittals in the job site office. Turn over all approved files as well as all appropriate operation and maintenance manuals to the Owner upon completion of the project. Provide all necessary samples. Maintain a full-time job superintendent. Conduct weekly job progress meetings, which include job site safety discussions. On a weekly basis, provide (2) copies of the following to Philips designees of: Job status report and action plan; job progress and safety meeting report; an updated job schedule showing actual vs. plan; job site progress pictures with location key; any other pertinent correspondence. Provide all necessary temporary utility hook-ups. Provide all necessary permits and pay for all inspection fees. Pay all applicable taxes on the work. Standard job site work hours are 6:30AM to 5:00PM. Permission to work at the site during any periods other than standard work hours must be approved in advance, in writing. Noise restrictions at the job site are as follows: Activities will be coordinated with facility staff and scheduled accordingly. HEPA filters and infection control procedures as required by the facility. Maintain negative pressure in the construction area as required by the facility. Provide for daily broom cleaning of the job site and debris removal and appropriate disposal (including any Philips equipment containers and packing materials). Use of walk off mats as required by the facility. The entire job site shall be thoroughly cleaned upon completion of the work, prior to turnover to the customer, and re-cleaned after the imaging equipment installation has been completed. Sterile final cleaning/terminal cleaning is not included. The storage, staging and delivery of materials to the job site shall be as follows: Within the work area. Parking for construction workers is restricted to: Designated areas. Compliance with the Owner's security regulations and dress codes is required. Use of the Owners' facilities is limited to: X-Ray Room 21-1156, Adjacent corridors, and related Electrical Room. A clean unrestricted access route to the project site is to be provided by the customer. The proposal assumes that all the on-site work will be accomplished in one phase, so that once work begins on site it will continue until all work has been completed. DIVISION EXISTING CONDITIONS Includes the following: The installation of code compliant temporary partitions to secure areas, control dust, protect adjacent areas and equipment as required are included. The demolition and appropriate removal and disposal of all existing walls, floors, ceilings, finishes, foundations, roofing, structure, equipment, and utilities as required accommodating the new work. All items intended to be salvaged by the owner will be so noted and removed by the owner prior to the start of the demolition work. This scope of work does not include the removal of any materials, including but not limited to asbestos, deemed hazardous by local authorities, the EPA, OSHA, or any other authority having jurisdiction over the work. If such materials are discovered at any time that the work is proceeding, the work will immediately cease, the owner will be notified, and the work will again proceed after the owner has removed all the hazardous material from the job site. DIVISION CONCRETE Includes the following: The existing foundation is to remain undisturbed and is presumed to be structurally sufficient. Includes the anchoring of Flush Mounted Floorplate and Floor Rail. DIVISION METALS Includes the following: The new building structure shall be of the following construction type: Existing to remain. Unistrut (or equal) equipment support is required as follows: Removal of existing 3 rail system including rails, vertical supports and bracing. Attachment points to deck above to remain. Installation of Longitudinal 4 rail system at existing ceiling elevation. Engineered Structural Drawings and Calculations. Partition framing is existing and will remain undisturbed. All necessary backing, kickers, headers, etc. will be constructed to match existing construction. The exterior of the existing construction will be left undisturbed. DIVISION THERMAL AND MOISTURE PROTECTION Includes the following: All penetrations through rated partitions shall be fire caulked and sealed with materials matching the facilities standards of construction. DIVISION FINISHES Includes the following: All existing drywall and/or plaster construction disturbed by the work shall be patched, repaired or replaced as required with materials and construction type compatible with the existing construction. All new construction shall have interior finishes as follows: Acoustical ceilings within the room will be removed and replaced entirely with 2 x 2 acoustical tiles and ceiling grid to match existing construction. Existing ceiling height is to remain. All drywall repairs are to be primed and finished painted. The balance of the walls shall be final coat painted in no more than two different colors as selected by the owner from samples submitted by the material supplier. All floors shall receive 1/8th inch thick Delane solid vinyl tile 12 x 12 . All materials to be as selected by the owner from samples provided by the material supplier. Exam Room shall receive 4"" vinyl base. All door frames shall be painted within the room only All existing finishes disturbed by the work shall be patched, repaired or replaced as required with materials and construction type compatible with the existing construction. DIVISION - FURNISHINGS Includes the following: An elevated shelf shall be provided under the counter for the Acquisition Workspot. DIVISION SPECIAL CONSTRUCTION Includes the following: Installation of Philips supplied equipment noted to be installed by contractor is included. FLOOR PLATES: Installation of the Philips supplied equipment base plate(s) is included. EXISTING RADIATION SHIELDING X-RAY: This proposal does not include design or post renovation testing of the radiation shielding. The scope of work is based upon the assumptions noted in this proposal that existing radiation shielding is sufficient. If the facility provided radiation shielding design, indicate that an upgrade to the existing radiation shielding is required; a change order to the turnkey agreement for the additional work will be required. DIVISION ELECTRICAL Includes the following: The power feeds, transformers, distribution panels and circuit breakers, conduit, wiring, junction boxes, back-boxes, pull boxes, pull wire, raceways, cable trays, wall and floor duct, lighting fixtures and lamps, receptacles, cover plates, switches, connections, couplings, and other miscellaneous parts are to be reused. All items must meet facility standards and local codes. The existing power feeders shall remain in place and connect to the existing panel (1PXH1) and must meet Philips imaging system requirements for power quality as specified in the Philips site planning documents. All required circuit breakers, taps, sub-panels, etc. as required are included. Existing room lighting is to remain as is. Existing 120V receptacles are adequate and are to remain in existing condition and location without additions or modifications. A shunt trip with connection to the fire alarm system shall be supplied and installed ahead of the Philips system PDU for equipment shutdown as required by local code. Installation of misc. Philips equipment related items as specified on the referenced Philips Site Planning Department documents is included. Requested Information: 1. Current SBA size standard for NAICS 236220 is $39.5M. 3. Please provide a capability statement. Describe your firm s in-house capability to accomplish the summarized project scope/goal; how your firm intends to meet the 15% self-performance requirement per PL109-461. 4.
