- THIS IS A NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT AND IS NOT A REQUEST FOR COMPETITIVE QUOTES. The Department of Veterans Affairs, Portland VA Medical Center intends to award a firm fixed price sole source contract under the authority of 41 USC §3304(a)(5), 38 U.S. Code § 8127(c), as implemented by FAR 6.302-5 (Authorized or Required by Statute) to: FIDELIS SUSTAINABILITY DIST LLC 204 W SPEARS ST., STE 3319 CARSON CITY, NV 89703-4160 USA The objective of this sole source award is for Fidelis Sustainability Dist., LLC Synaptive Medical MODUS V 3D PACKAGE UPGRADE. Sole source is based on only one responsible source as Synaptive Medical is the manufacturer of Synaptive Medical MODUS V 3D PACKAGE UPGRADE and Fidelis Sustainability Dist, LLC is the only source for MODUS V 3D PACKAGE UPGRADE. The NAICS Code for this requirement is 334510; the small business size standard is 1,250 employees. This notice of intent is not a request for competitive quotes; however, all responsible sources may submit a capability statement, proposal, or quotation, which shall be considered by the agency. All information received by 11:59AM PST on March 23, 2022 may be considered by the Government. Responses shall be submitted to Scott James, Contract Specialist at scott.james@va.gov. The email subject line shall state, Intent to Sole Source Fidelis Sustainability MODUS V 3D PACKAGE UPGRADE . The Government will not pay for information submitted. This will be the only notice of the Government's intent to solicit, negotiate, and award to one source to satisfy this requirement.
