Description This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued. This solicitation is issued as an RFQ. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-05 03/07/2022. This solicitation is Full and Open competition The associated North American Industrial Classification System (NAICS) code for this procurement is 238210, with a small business size standard of 200 employees. The FSC/PSC is 6145. The NCO 10 for Cleveland VA Medical Center is seeking to purchase low voltage controllers, wiring from bed connector to TV location, phono jacks and necessary installation services. Cares Tower 2 Nurse Call Upgrade Bed Controllers and Wiring 3. Background. The Hill-Rom Nurse Call System was recently upgraded on Cares Tower Level 2 to match those systems located on the other floors located within the CLC. This updated the Graphical room stations, audio bed connectors, added pillow speakers, updated the Icon Based light LED dome/Zone lights in the hall, and included the install and certification of the system. The upgrade did not include the 18/3 wiring from the bed connector to the TV locations, the phono jacks on the TV side of the wall, the 18/3 wire that is needed from the low voltage light controller to the bed connector. This effort is for the 18/3 wire that connects the bed connector to the TV locations, ¼ phono jack located on the TV side of the wall, and the 18/3 wire from the low voltage light controller to the bed connector, and the low voltage controllers. This effort will complete the transition of the floor from a rehab floor to a Long-Term care floor. 4. Scope. The Cleveland VAMC is requesting the purchase of low voltage controllers for 37 beds, 18/3 wiring, 74 Curbell ¼ phono plugs for pillow speaker, firestops and sleeves as needed (VA Approved), and install services. The vendor will install the low voltage controllers for the 37 beds, install 74 curbell ¼ phono plugs, install VA approved firestops and sleeves where required, provide and run 18/3 wiring to connect the bed connector to the TV and connecting the low voltage light controller to the bed connector, (74) curbell ¼ phono jacks , 18/3 wiring will need to be ran from the bed connector locations to the tv locations, install ¼ phono jacks on the Tv side, 18/3 wire ran from the low voltage light controller to the bed connector. Dual patient rooms need to be equipped with the HCI Bedmate 15 (BM15TV) monitor, and they need to be placed upon bed arms. Single patient rooms will need HCI 32 (RM32AP) monitors. There are two lighting controls. One is to connect a single over bed reading light. The other controls three downlights. They will be wired to boxes. An electrician will be necessary for the install. 5. Specific Tasks. The vendor will provide necessary equipment and services to complete the nurse call upgrade for 37 patient beds. Deliverables: Low voltage controllers for 18/3 wiring 74 Curbell ¼ phono plugs for pillow speaker control Firestops and sleeves that are VA approved Installation services 6. Performance Monitoring All requirements contained in this SOW must be met and must meet minimum industry standards. The COR will ensure the install will be completed and periodic inspections will be conducted with the vendor to ensure the quality of work is within the standards of the contract. 7. Security Requirements The contractor shall be required to comply with all security policies/requirements. All security policies/requirements must be met, and employees cleared prior to the contractor performing work under this contract. Employees that cannot meet the security and clearance requirements shall not be allowed to perform work under this contract. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). Not Applicable 9. Other Pertinent Information or Special Considerations. There are no known special considerations or pertinent information at this time. a. Identification of Possible Follow-on Work. No possible follow-on work is anticipated at this time. b. Identification of Potential Conflicts of Interest (COI). There are no known conflicts of interest at this time. c. Identification of Non-Disclosure Requirements. Not applicable. d. Packaging, Packing and Shipping Instructions. The vendor will provide these services at the Cleveland VAMC. 10701 East Blvd Cleveland, OH 44106. e. Inspection and Acceptance Criteria. The COR will work with Engineering to ensure they system is properly running and completed. 10. Risk Control Not Applicable. 11. Place of Performance. All work will be performed at the Cleveland VAMC. 12. Period of Performance. The period of performance shall begin 30 days from date of award. with no option years for the service. Vendor will be on site during normal working hours 13. Delivery Schedule. All required items, Low voltage controllers for 37 beds, 18/3 wiring, 74 Curbell ¼ phono plugs for pillow speaker control, Firestops and sleeves will be delivered at the start of the project. Delivery shall be provided no later than 30 days after receipt of order (ARO). The following are the decision factors: Ability to meet the request at the best value to the government. The award will be made to the response most advantageous to the Government. Responses should contain your best terms, conditions. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows: ""The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition.""
