- The purpose of this notice is to answer questions, provide an updated PWS and extend the response due date to April 8, 2022 10:00 A.M. Easter Standard Time. Solicitation 36C24122Q0161 VISN 1 Legionella & Dental Clinic Water Sampling Services Questions March 23, 2022 Question # Section Page Question VA Response #1 2.C.3 Scope of Work 6 of 85 This section and price sheet indicate THAB however the facility and building tables on pages 15 17 indicate HPC analysis for Dental. Also, historically analysis has been HPC vs. THAB. Please confirm that Dental Clinic analysis should be THAB vs. HPC as indicated in tables on pages 15 17. Dental sample analysis should be HPC. #2 2.C.4 Scope of Work 6 of 85 SOW indicates sampling every two weeks for cooling towers/chillers. Please confirm testing requirements only apply to those cooling towers that are operational or operational ready per the VHA Directive. Yes, sampling is only occurring in cooling towers that are operational. #3 2.C.4 Scope of Work 6 of 85 The VHA 2021 directive (section 4.a.1) states (1) Sample Number and Locations. The number of water samples is dependent on the size and configuration of the cooling tower. At a minimum, samples must be collected from each basin/sump, and the supply to and return from the chiller(s). If the chillers are on a common header, only one sample is required. NOTE: Depending on the time of the year and VA medical facility location, a VA medical facility may have some cooling towers that are operational and some that are not. These testing requirements only apply to those cooling towers that are operational or operational ready. Please confirm that only one HPC sample/chiller every two weeks when chiller is in operation is what is required. It is unclear from the SOW and table on pages 15-17 the number of chillers needing HPC analysis, if there is a common header, etc. Yes, HPC sampling is required every two weeks when the unit is in operation. It is estimated that a total of two HPC samples will need to be collected from each unit. This is an estimate and may vary based on the facility configuration. #4 2.C.4 Scope of Work 6 of 85 Would the VA consider adding a minimum trip charge CLIN to price sheet since the unit cost for collection and analysis may be relatively low for one sample every two weeks but the travel time and travel costs to some locations is non-trivial? Allowing a minimum trip charge allows for a lower unit cost for the chiller samples and possible cost savings to the VA. No. Travel fees must be included in the sample price. #5 2.C.4 Scope of Work 6 of 85 Please confirm that only cooling tower/chillers at Bedford, CWM VAMC, and W. Haven VAMC are included in the every two week sampling for HPC regime based on information in tables on pages 15-17. There are other locations with cooling towers but only Legionella quantities listed in the table. Currently, Bedford, CWM, West Haven and Togus are listed on pages 15-17 as needing the every two week Bacteria testing for cooling towers. SAM.GOV indicates: Original Date Offers Due: May 01, 2022 10:00 am EDT. 04-08-2022 10:00 EST #8 E.10 70 of 85 Please consider providing an extension if answers to the questions are not provided by March 29, 2022 at 10 a.m. to allow time to address the Q&As since the proposal is currently due by April 1, 2022, 10 a.m. Eastern Standard time and questions are due by March 28, 2022 at 10 a.m. 04-08-2022 10:00 EST UPDATED PERFORMANCE WORK STATEMENT VISN 1 (New England) Environmental Water Testing for Legionella Pneumophilia General Information: Title of Project: VISN 1 (New England) Environmental Water Testing for Legionella Pneumophilia Background: Legionella suppression efforts at Veterans Health Administration (VHA) inpatient facilities and Community Living Centers are guided by VHA Directive 1061 Prevention of Healthcare-Associated Legionella Disease and Scald Injury from Potable Water Distribution Systems, dated February 16, 2021. The Directive updates VHA s approach to Legionella control and mitigation. It enhances and expands on engineering controls, establishes mandatory standards, and identifies required processes at a wide range of facility types. The breadth and scope of these elements reflect the Centers for Disease Control and Prevention (CDC) statement that there is no safe level of Legionella in a water system . Healthcare facilities are included in the types of buildings that have been associated with the transmission of Legionella to people (Legionella Disease; LD). Persons at increased risk for LD include the immunocompromised (due to, for example, cancer treatment, certain chronic infections, diabetes, organ transplant, renal disease), those over 50 years of age, those with chronic lung disease, and smokers. However, LD outbreaks reported in the medical literature indicate that the disease can also occur in seemingly healthy individuals. Prevention of LD depends on minimizing the exposure of people to Legionella in building water distribution systems and cooling tower mists. The Legionella prevention activities follow the general risk management principles of hazard identification, analysis, and implementation of controls to limit the growth of Legionella. Engineering controls limit a hazard, monitoring these engineering controls, validating that the control measures are effective and modifying controls when sample results identify problem areas in the building water distribution infrastructure or cooling towers, are necessary for a successful program of LD prevention. Scope of Work: Overview: Environmental Validation is the process of testing the potable water distribution system for Legionella to determine if the primary and any supplemental engineering controls are successfully inhibiting growth of Legionella. Laboratories that process environmental water samples for Legionella must be certified by the Centers for Disease Control and Prevention (CDC) Environmental Legionella Isolation Techniques Evaluation (ELITE) program as proficient at performing the culture of Legionella species from environmental samples or the Public Health England (PHE) Legionella External Quality Assessment (EQA). Information about ELITE certified laboratories can be found at https://wwwn.cdc.gov/elite/Public/MemberList.aspx. Estimated Sample Volume: The estimated annual volume of environmental water samples to be tested is up to 4,200 samples as an aggregate from all VISN 1 sites. The total estimated sample volume over five years is approximately 21,000 environmental tests for Legionella. There are approximately 70 different buildings that may be tested at the 11 medical centers on a quarterly basis. The Contractor shall submit environmental potable water, cooling tower and chiller water samples for Legionella collected from VISN 1 facilities within 24 hours to the CDC ELITE Legionella testing laboratory to ensure the samples arrive during the Laboratory s normal operating hours. Any growth of Legionella species is considered a positive sample. At no extra charge, positive samples shall then be further tested by the CDC ELITE testing laboratory to identify the species of Legionella, thereby assisting the Facility Managers assess their remedial protocols. The CDC ELITE testing laboratory will either perform this identification testing (to include molecular characterization of environmental Legionella isolates when requested) or use their reference lab for this procedure. The CDC ELITE Laboratory will provide the requirements and recommendations on sample collection and shipping to each of the VISN facilities. The CDC ELITE testing laboratory shall accept and accession VISN 1 environmental water samples within 24 hours of receipt and perform required testing for Legionella and return results of this testing to the VA Facility Legionella Coordinator (see list) within 15 business days of collection in the format specified in Task 3 of this PWS. Required Water Analysis Tests: Contractor shall provide the following water tests upon request from VA facilities: ISO 11731 Water quality (Detection and enumeration of Legionella) for Legionella pneumophila and non-pneumophila species (by L. pneumophila serogroup 1, 2-14 and non-pneumophila species). PCR (Polymerase Chain Reaction) testing. The laboratory also must demonstrate proficiency at PCR detection of Legionella from environmental samples by the CDC-recognized lab conducting proficiency testing (currently the Wisconsin State Laboratory of Hygiene (WSLH)) or by the PHE EQA Legionella molecular scheme. Enough sample volume must be collected to run a culture test if the result is positive. Dental Unit Water Lines (DUWL) Testing. The Facility Water Safety Coordinator may also request the Contractor sample Dental Unit Water Lines (DUWL) in Dental Service for Heterotrophic Plate Count (HPC) using a laboratory certified by the EPA or state to perform environmental water testing. In the alternative, the Government may collect the DUWL samples using sampling kits provided by the certified environmental testing laboratory and submit the samples directly to this laboratory. All certified environmental testing laboratories performing HPC testing of DUWL samples under this contract must be approved in writing by the Government prior to accepting samples, and must provide documentary evidence of appropriate certifications, state licenses (when applicable), and a scientifically reliable QA/QC program. Contractor shall provide bacteria sampling via Heterotrophic Plate Count (HPC commercial dip slide method or lab culturing) for cooling towers/chillers within 7 days of the systems startup and every two weeks in accordance with Appendix D of VHA Directive 1061 and VHA Engineering Standard ES-2019-001 Cooling Tower Water Systems.(Attachment XX of this document) Specific Tasks/Deliverables: Description Requirements Delivery Date Task 1: Water sample collection, Legionella testing, and results reporting. Task 1A: Provide sample bottles to Government for water sampling collection and laboratory analysis. The Contractor will collect potable water samples at a minimum quarterly from the designated hot and cold-water system sampling points, and environmental water samples from cooling towers, chillers, water coolers and other areas, and ship the specimen bottles to the CDC ELITE lab. The contractor will provide a final report of the results for all samples analyzed (i.e., both positive and negative test results) to the Facility Water Safety Coordinator in the format specified within 15 business days of sample receipt. At the time of any environmental sampling, temperature, pH, and oxidant residuals will be taken, recorded, and provided with the report on each sample. A copy of the field data will be provided to the Facility at the time of sample collection. Testing will be conducted at water distribution distal sites chosen by the Facility in each appropriate building. The CDC ELITE Lab shall conduct further testing of positive samples to identify the Legionella species and serogroup and provide a final report of the Legionella speciation results to the Facility within 15 business days of sample receipt. This will be done at no charge to the government. Molecular characterization of environmental Legionella isolates will be performed if requested. The Government will collect potable water samples. The Contractor shall provide the Government water collection bottles at a minimum quarterly to perform sample collection to include bottle, preservative, labels, and adequate shipping box. Size and volume will be specified by the facility. Costs include laboratory analysis. The CDC ELITE Lab shall conduct further testing of positive samples to identify the Legionella species and serogroup and provide a final report of the Legionella speciation results to the Facility within 15 business days of sample receipt. This will be done at no charge to the government. Molecular characterization of environmental Legionella isolates will be performed if requested. Within 15 business days of initial receipt of water samples. Within 5 business days of request. Task 2: Collecting Samples for Retesting Purposes Task 2A: Provide sample bottles to Government for Retest sampling collection and analysis. Same as Task 1 but for resampling purposes. Same as Task 1A but for resampling purposes. Within 15 business days of initial receipt of water samples. Within 5 business days of request. Task 3: Legionella Sample Results Reporting. Sample results, signed by the CDC ELITE Laboratory manager or director and QA/QC manager, shall be reported to the Facility electronically from each discrete sampling event in an Excel spreadsheet template provided by the COR at no additional cost. Within 3 business days of receiving results from the laboratory. Task 4: Water sample collection from Dental Service, Heterotrophic Plate Count (HPC) testing, and results reporting. The Contractor will collect water samples from Dental Unit Water Lines (DUWLs), ship specimen bottles to the VA-approved EPA or state-certified environmental water testing lab, instruct the lab to perform testing for Heterotrophic Plate Count (HPC) using approved method, and provide a final report of the results of all samples analyzed signed by the laboratory director or manager and QA/QC manager, within 10 business days of receipt of sample(s). This service is on the request by the facility. Within 10 business days from initially receipt of water samples. Task 4a: Resampling water sample collection from Dental Service, Heterotrophic Plate Count (HPC) testing, and results reporting. Same as Task 4. Within 10 business days from initially receipt of water samples. Task 5: Conduct bacteria testing of Cooling Towers/Chillers. Contractor will conduct bacteria) testing (commercial dip slides or lab culturing) within 7 days of system startup and every two weeks thereafter according to Appendix D of VHA Directive 1061 and VHA Engineering Standard ES-2019-001 Cooling Tower Water Systems. Provide testing results within 10 business days of sampling. Task 5a: Conduct resampling bacteria testing of Cooling Towers/Chillers. Same as Task 5. Provide testing results within 10 business days of sampling Task 6: Polymerase Chain Reaction (PCR) Testing Task 6a: ISO 11731 analysis on positive PCR samples. Facilities that routinely do not have positive legionella sample results may elect to choose PCR testing. Upon request, the Contractor shall collect legionella samples according to Task 1 and run PCR analysis. Contractor shall run the standard ISO 11731 Legionella test for all positive PCR samples. Provide testing results within 72 hours of sampling Within 15 business days of initial receipt of water samples. Task 7: Responding to Technical Questions Pertaining to Environmental. Water Sample Results. Contracted Laboratory (CDC ELITE Lab for Legionella and EPA or state-accredited environmental testing lab for Dental water samples) will respond to basic inquiries and clarifications of sample results within 24 hours of inquiry from any VISN 1 Facility Water Safety Coordinator, Facility Manager, and/or healthcare providers. This does not include extensive consulting engagements, or in-depth discussions with water safety committees. Within 24 hours of any inquiry during normal business hours. Testing Sites and Details: When requested, the Contractor will collect potable water samples from the specified VA facility s hot and cold-water systems, ice machines, outlets (e.g., sinks, showers) and environmental water samples from cooling towers and submit these samples quarterly (minimum requirement) to the CDC ELITE Lab using the laboratory-provided sample collection container and instructions. The Contractor shall also measure and record the temperature, pH, and disinfectant (oxidant) residual of the water and include the recorded data with the submitted samples and submit to the Facility on the provided Excel format. Contractor shall provide field results to the Facility Water Safety Coordinator prior to leaving the site. In some situations, the Government may collect the environmental water samples and submit them directly, either to the CDC ELITE Lab for Legionella testing, or the EPA or state-certified lab for environmental water testing including Total Heterotrophic Aerobic Bacteria (THAB) testing (Dental Service). The Contractor shall also ensure that all services provided to the Government comply with all VA- and VHA- mandated policies, procedures, standards, and requirements. The Contracted CDC ELITE Laboratory shall perform initial lab testing services entirely upon their premises. Include data form upload sheet. The CDC ELITE Lab shall provide quantity, species ID and serogroup for all positive samples Molecular characterization of environmental Legionella isolates shall be provided upon request. The Contracted laboratory used for Legionella testing shall be CDC ELITE certified, and shall maintain all licenses, permits, accreditation and certifications required by law and VHA Directive 1061 including certification status in the CDC ELITE Program for Legionella testing, throughout the entire Period of Performance The Contracted lab used for Total Heterotrophic Aerobic Bacteria (THAB) testing of water from Dental unit water lines (DUWLs) shall be an EPA or state-certified lab for environmental water sample testing including Total Heterotrophic Aerobic Bacteria (THAB). The Contractor has an ongoing obligation throughout the Period of Performance to timely notify the Contracting Officer immediately, in writing, upon its loss (or any of its subcontractors) of any required certification, accreditation, or licensure. Contractor shall follow VA requirements for physical and information security. Contractor s assigned employees will be subject to background investigation and shall obtain temporary contractor badging for all employees servicing the contract prior to performing site work. For both the Legionella Environmental Sampling and the Heterotrophic Plate Counts the contractor will provide one price of each test that is sampled by the contractor at the VHA facility and then tested by the Laboratory and one price for each test that is sampled by the facility and directly sent to the Laboratory. Should a sample leak, become damaged or get lost in transit to the laboratory, the Contractor will, at no cost to the Government, conduct a resample and submit for analysis. Period of Performance : The testing start date shall be within 30 days after a signed contract authorizes the service. The period of performance is a base year, with up to four (4), one-year option years, based on satisfactory prior performance. COR Authority: In no event is the COR empowered to change any of the terms and conditions of the contract. All changes, to any section of this contract, shall be made only by the Contracting Officer pursuant to a properly executed written modification. The types of actions within the purview of the COR s authority are to ensure that the Contractor performs the technical requirements of the contract, and to notify the Contracting Officer of any deficiencies observed. A Letter of Designation shall be issued to the COR and a copy shall be sent to the Contractor at the time of award setting forth in full the responsibilities and limitations of the COR. Confidentiality and Nondisclosure: Acceptance of contract award shall constitute agreement by the Contractor and any of its Subcontractors authorized under this Contract that: a. The preliminary and final deliverables and all associated papers, results, electronic files, and other material deemed relevant by the Government which have been generated by the Contractor in the performance of this contract are the exclusive property of the U.S. Government and shall be submitted to the Contracting Officer at the conclusion of the contract. b. The Contracting Officer will be the sole authorized official to release verbally or in writing, any data, the draft deliverables, the final deliverables, or any other written or printed materials pertaining to this Contract. There shall be no information released by the Contractor. Any request for information relating to this contract presented to the Contractor or any of its Subcontractors shall be submitted to the Contracting Officer for response. c. Press releases, marketing material or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the Contracting Officer. d. All selected Contractor personnel assigned to work on this project shall be required to sign a non-disclosure statement regarding key aspects of the project and project content. Quality Control and Assurance: QASP: After contract award, the Contractor shall implement and follow the Quality Control Plan or Quality Assurance and Surveillance Plan (QASP) that is submitted as part of this proposal, and accepted by the Government. Using the Quality Control Plan or QASP with performance goals and acceptable quality levels is necessary to ensure quality for this project. The Contractor shall address any issues meeting established Government performance goals as needed. The Government shall evaluate the Contractor s performance under this agreement. Government surveillance of tasks not listed in the Quality Control Plan/QASP or by methods other than those listed in the Quality Control Plan/QASP may occur during the performance period of this agreement. Performance Evaluation Meetings: The Contracting Officer may require the Contractor to meet with the CO and COR on at least a quarterly basis as needed. The Contractor may request additional meetings with the Contracting Officer or COR when Contractor believes such a meeting is necessary for the efficient administration of this Contract.
