General Surgery operates four days a week and will do anywhere from one to five laparoscopic (minimally invasive) surgeries per day. We must have laparoscopic instrument sets available for each of these cases which will allow the surgeon to manipulate internal organs and structures through tiny incisions no greater than an inch long. The requested CareFusion laparoscopic instrument sets will replace the laparoscopic instruments we currently utilize. We currently have 7 sets of Karl-Storz laparoscopic instruments, but they were purchased over 10 years ago and are aging out, difficult to assemble and manipulate. We are requesting 7 new instrument sets to perform the quantity of cases that we are scheduling (this includes backup sets to be available in case of contamination). Scope of Work The proposed CareFusion laparoscopic instrument sets will replace our current 7 sets of Karl-Storz laparoscopic instruments. Laparoscopic instruments are used in minimally invasive surgery procedures allowing the surgeon to manipulate internal organs and structures through a tiny incision no greater than an inch long. CareFusion (Snowden-Pencer) laparoscopic instruments bring together ergonomics, versatility, and durability with a wide selection of jaw patterns suited to each surgeon s needs. Furthermore, the design of the handles more evenly distributes pressure, which helps reduce the potential for intraoperative hand fatigue and temporary digital nerve compression. Objectives to be Achieved: Instruments may be used through any size port including 5mm ports and 12mm ports. Compatible with our ESUs (electrosurgical unit). No change in sterile processing. Use is not limited to General surgery service. Will allow for use across multiple services in the OR including General, Thoracic, GYN, and Urology. CareFusion (Snowden-Pencer) take-apart instruments can be easily assembled and disassembled, allowing visual inspection during cleaning. Vendor Requirements: Vendor is responsible for uncrating, assembling, and delivering to a specific location in the VAMC. Vendor will provide training to OR staff on proper use and provide training to Biomedical Engineering and SPS staff on proper maintenance, cleaning, and repair for General surgery instruments purchased. The vendor shall provide complete printed sets as well as electronic version of operator manuals, service manuals, IFUs, and parts lists for the instruments. The company warranties each medical device and accessory bearing the V. Mueller and Snowden-Pencer brand names to be free of functional defects in workmanship and materials when used normally for its intended use. Any V. Mueller product proven defective will be replaced or repaired at no charge. They are protected by a full-service 1-year warranty, plus a lifetime warranty against manufacturing defects. Essential physical characteristics; Dimensions (if applicable) When put together, each instrument consists of a Snowden-Pencer Ergonomic Ring handle, a shaft, and an insert. Each insert has a shaft diameter of 5mm and a shaft length of 36cm. This procurement is for new OEM medical equipment only for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor, or authorized reseller for the proposed equipment/system, verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the government. Award will be based upon a comparative evaluation of quotes in accordance with the Simplified Acquisition Procedures of FAR 13. Comparative evaluation is the side by side pairwise comparison of quotes based on factors resulting in a Contracting Officer decision for the quote most favorable to the Government. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The following factors will be considered: Price Technical capability or quality of the item offered to meet the Government requirement Delivery of order will need to be 30 days ARO, Freight on Board (FOB). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
