This Request for Information (RFI) is intended for information and planning purposes only at this time; and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Because this is a Request for Information announcement, no evaluation letters and/or results will be issued to the respondents. No solicitation exists. Therefore, do not request a copy of a solicitation. The Department of Veterans Affairs VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified sources capable of providing the FOLLETT BRAND NAME refrigerators and freezers with the specific descriptions listed in the STATEMENT OF NEED below. STATEMENT OF NEED Title of Project: Follett refrigerators and freezers for activation of new Laboratory at Lebanon VA Medical Center. Scope of Work: The vendor shall provide all resources necessary to accomplish the deliverables described in this statement of need (SON), except as may otherwise be specified. The refrigerators must be in compliance with CLIA guidelines and FDA regulations. The freezer must be in compliance with Joint Commission and FDA regulations. The contractor shall provide installation for all Follett refrigerators, freezers and combination refrigerator/freezers. Background: The Lebanon VA Medical Center is building a new laboratory to accommodate expansion of services in Pathology, Histology, Microbiology, Blood Bank, Urinalysis, and COVID 19 testing modalities. Performance Period: Delivery and installation shall occur within 30 days of contract award. The need date is June 1, 2022. The Contractor shall deliver and install all equipment required under this SON within thirty (30) calendar days or less from the date of award, unless otherwise directed by the COR. Electrical component: 208 volt electric on site. Please refer to P11 Electrical Riser Diagram. The Engineer will need to perform a safety check and enter the equipment into the system for annual Preventative Maintenance (PM) Checks. Placement in the Laboratory shall be coordinated through the Contracting Officer Representative (COR). Delivery of equipment shall be made to: Lebanon VA Medical Center, 1700 S. Lincoln Ave. Lebanon, PA 17042. Access to an unloading dock will be available. Delivery and installation shall be coordinated at least 72 hours prior to required delivery/installation date with the Contracting Officer Representative (COR). Please contact the COR with any questions or concerns. Work is to be performed at the location Monday through Friday between the hours of 7:00 AM and 4:00 PM. Responses to this RFI should include company name, address, point of contact, phone number, and point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industry Classification code (NAICS) 333415\, Air-Conditioning and Warm Air Heating Equipment and Commercial and Industrial Refrigeration Equipment Manufacturing. The Small Business Size Standard for this NAICS code is 1250 employees. This notice will help the VA in determining available potential sources only. All firms responding to this Request for Information are advised that their response is not a request for proposal, therefore will not be considered for a contract award. If a solicitation is issued, information will be posted on the Contracting Opportunities web site for all qualified interested parties at a later date and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supplies or services.
