- Combined Synopsis/Solicitation |�70FBR622R00000017 The Federal Emergency Management Agency (FEMA) has an urgent requirement and is considering the issuance of a single or multi-vendor fixed rate purchase order in support of disaster relief operations for Hurricane Ida � DR4611. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice, FAR 13.106-1, and FAR 15.405. The solicitation number 70FBR622R00000017 is being issued for request for price (RFP). The solicitation document and incorporated provision and clauses are those in effect through Federal Acquisition Circular (FAC) 2022-04. The associated North American Industrial Classification System (NAICS) code for this procurement is 238910 Site Preparation Contractors. This action is being competed using the policies and procedures for procurements under the Robert T. Stafford Disaster Relief and Emergency Assistance Act (42 USD 5150) and FAR 52.226-3, 52.226-4, 52.226-8 and 6.208 Set-Asides for Local Firms during a Major Disaster or Emergency. This action is set-aside for vendors residing in or primarily doing business in federally declared disaster counties in the state of Louisiana. Description of Requested Items/CLIN Structure Purpose The Department of Homeland Security, Federal Emergency Management Agency (FEMA) requires the rental of an excavator and a motor road grader and related services in support of disaster declarations in support for DR4611-LA Quote Submission Instructions Responses to this Request for Price (RFP) are due electronically to ashlee.young@fema.dhs.gov by 5:00 pm CST, on Wednesday, April 13, 2022. This solicitation requires an active registration with the System for Award Management (SAM) in order to be considered for award, pursuant to applicable regulations and guidelines. All quote submissions must include the following information on a quote submission cover letter. 1.�������� Tax Identification Number 2.�������� Dun & Bradstreet Number (DUNS) 3.�������� Authorized Representative Contact Name 4.�������� Contact Email Address 5.�������� Contact Telephone and Fax Number 6.�������� Complete business mailing address ___________________________________________________________________________ Evaluation Factors The vendor shall provide a response in accordance with all requirements of this request for price and statement of work. Factor 1. Price The contractor shall provide a pricing chart which includes the monthly rental fees per requested product, delivery, and pickup fees. Additionally, the Contractor must provide an image, product description, and specification of proposed equipment for this solicitation. Factor 2. Delivery Schedule The contractor shall provide the earliest delivery date for each location to accommodate the fulfillment of the equipment rental requirement: 1. Experience and Technical Competence in the Type of Work Required The contractor shall provide a business capability statement that responds to all requirements outlined in the statement of work and contractor assurance statement. Past Performance The contractor shall provide 3 references for work similar in nature, size, scope, and complexity, as proven by the successful management of current and previous contracts of a similar magnitude, scope, and complexity.
