- This is not a Presolicitation or Solicitation announcement. This is a sources sought notice only. The purpose of this sources sought notice is to gain market knowledge of potential small business sources and their size classifications; HUBZone, 8(a), Small Disadvantage Business, Woman Owned Business, Veteran Owned Business, Service Disabled Veteran Owned Business relative to NAICS Code 238220, Plumbing Heating and Air Conditioning, with a small business size standard not to exceed $16,500,000.00 gross annual receipts. Responses to this sources sought notice will be used by the Government to make appropriate determinations about potential sources only. No solicitation is currently available at this time. After review of the responses to this sources sought announcement a synopsis and/or solicitation announcement may be published at a later time. Responses to this notice shall include company name, address, point of contact, size of business pursuant to North American Industrial Classification Code (NAICS), a capability statement and must respond to the following questions: 1) Is your business a small business under NAICS code 238220? 2) Does your firm qualify as a small disadvantaged business? 3) If disadvantaged specify if your firm is certified under Section 8(a) of the Small Business Act? 4) Are you a certified HUBZone firm? 5) Are you a woman-owned or operated business? 6) Are you a certified Service-Disabled Veteran Owned or Veteran Owned business? 7) Are you a large business 8) Are you able to meet the limitation on subcontracting- See attached form. Please provide a capability statement addressing your organizations ability to perform as a prime contractor the work described in the Attached Draft PWS. This is not request for proposal and in no way obligates the Government to award any contract. No solicitation is currently available at this time. Please provide responses to this notice to: Tnauri.woodbridge@va.gov. Telephone responses will not be accepted. Please provide responses in writing no later than Monday, April 18, 2022. Responses to this sources sought announcement will not be considered as a response to any solicitation, a request to be added to a prospective offerors list, or to receive a copy of any solicitation. Statement of Work Department of Veterans Affairs (VA) Animal Lab Multi-Stack Repairs The Purpose of this one-time contract in the West Haven Animal Lab is to repair and to replace the Multi-stack Environmental Units in Building 2, 9th floor. All work shall be completed within 15 days of date of award. Location of Facility: West Haven VAMC 950 Campbell Ave West Haven, CT 06516-2770 Scope of Work: All contracted work shall be performed by a qualified vendor. The contractor shall: Provide all material, labor, safety equipment, LOTO procedures, and disposal necessary to provide services Check in with Customer Lock Out/ Tag Out Unit Reclaim Refrigerant as needed Pressurize Circuits Provide leak checks for all units Repair all leaks associated with the units Replace Filter Driers Evacuate and charge with new Refrigerant Functionally test system to confirm/ensure proper operation Replace pipe insulation if needed All work shall be performed and completed on a schedule between the hours of 7:00AM and 5:00PM EST. Dates and times of repairs shall be coordinated between the contractor and Government. Interference to Normal Functions: When the contractor s work interferes with hospital functions, such as when work produces excessive noise, odors, dust, utility service interruptions, or other interferences with normal hospital operations that cannot be contained within the area of work, the contractor shall schedule said work at other than normal hours and as directed by the VA designee at no additional charge to the Government. Debris Removal: The Contractor/Installer shall be required to remove waste materials and debris daily from the campus. All waste material and debris shall be removed off the site by the contractor and shall be disposed of in accordance with applicable State and Federal regulations. Property Damage: The Contractor shall take all necessary precautions to prevent damage to any government property. The Contractor shall report any damages immediately and shall assessed current replacement costs for property damaged by the contractor unless corrective action is taken. Any damaged material will be replaced in a timely manner or corrected by the Contractor with like materials at no extra cost to the government upon approval of the Contracting officer. Additional Information: The Department of Veterans Affairs is a smoke free environment and smoking is not permitted at the facility. VHA Supplemental Contract Requirements for Combatting COVID-19 1. Contractor employees who work in or travel to VHA locations must comply with the following: a. Documentation requirements: 1) If fully vaccinated, contractors shall show proof of vaccination. i. NOTE: Acceptable proof of vaccination includes a signed record of immunization from a healthcare provider or pharmacy, a copy of the COVID-19 Vaccination Record Card (CDC Form MLS-319813_r, published on September 3, 2020), or a copy of medical records documenting the vaccination. 2) If unvaccinated, contractors shall show negative COVID-19 test results dated within three calendar days prior to desired entry date. Test must be approved by the Food and Drug Administration (FDA) for emergency use or full approval. This includes tests available by a doctor s order or an FDA approved over-the-counter test that includes an affiliated telehealth service. 3) Documentation cited in this section shall be digitally or physically maintained on each contractor employee while in a VA facility and is subject to inspection prior to entry to VA facilities and after entry for spot inspections by Contracting Officer Representatives (CORs) or other hospital personnel. 4) Documentation will not be collected by the VA; contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information. 2. Contractor employees are subject to daily screening for COVID-19 and may be denied entry to VA facilities if they fail to pass screening protocols. As part of the screening process contractors may be asked screening questions found on the COVID-19 Screening Tool. Check regularly for updates. a. Contractor employees who work away from VA locations, but who will have direct contact with VA patients shall self-screen utilizing the COVID-19 Screening Tool, in advance, each day that they will have direct patient contact and in accordance with their person or persons who coordinate COVID-19 workplace safety efforts at covered contractor workplaces. Contractors shall, at all times, adhere to and ensure compliance with federal laws designed to protect contractor employee health information and personally identifiable information.
