Include the Following on Proposal: Tax ID:_______________� UEI:_______________ Point of Contact: Name:_______________________________________ Phone: ______________________________________ Email: ______________________________________ AMENDMENT 01: This amendment: Corrects language that this is a set-aside for SBIF.� The tiered approach still applies. Changes the contract type to a Requirements Contract. Updates Schedule B. Includes the questions and answers. Please submit bid respond to the amended RFQ-10-22-004-PYO. ******* This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number RFQ-10-22-004-PY is issued as a request for quotation.� The associated North American Industry Classification System code is 611699, All Other Miscellaneous Schools and Instruction; the small business size standard is $12 million. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04 dated January 30, 2022. The procurement is being conducted pursuant to the authority of FAR Part 13, Simplified Acquisition Procedures, FAR 37.101, Nonpersonal Services Contract and if applicable, The Buy Indian Act (25 U.S.C. 47).� This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Billings Area Indian Health Service (IHS) intends to award a Requirements contract to one (1) Contractor in response to Request for Quotation (RFQ) 10-22-004-PYO. �The services specified in the Schedule are estimates only. �This solicitation will be awarded as a Requirements contract for services specified, and effective for the period stated, in the Schedule. Except as this contract may otherwise provide, if the Government�s requirements do not result in orders for the listed items in the Schedule, that fact shall not constitute the basis for an equitable price adjustment. The Government shall order on an as-needed basis in order to fulfill the FBSU requirements. If IHS does not receive at least two offers that are reasonably priced�pursuant to the Buy Indian (25 U.S.C. 47),�IHS will then evaluate quotes and consider award to other small business concerns under the following the 7-tier cascading set-aside (in order of first consideration to last consideration): 1st Tier-Small Business Indian Firms that are Indian Small Business Economic Enterprise (ISBEE) 2nd Tier-Indian Owned Enterprise (IEE); 3rd Tier-Service Disabled Veteran Owned Small Businesses; 4th Tier-Certified HUBZone Small Businesses; 5th Tier-Small Disadvantaged Businesses; 6th Tier-All Other Small Business Concerns; 7th Tier-All Other Than Small Business Concerns. The purpose of this acquisition is to contract for web based competency electronic training services through a learning management system for mandatory employee training for the Fort Belknap Service Unit (FBSU), Harlem Montana.� FBSU provides healthcare for the service population on the Fort Belknap Indian Reservation, located in north-central Montana. FBSU is comprised of the Fort Belknap Hospital, in Harlem, Montana, and a Health station, Eagle Child Clinic, located at Hays, Montana, 39 miles south of Harlem.� FBSU has an obligation to meet the mandatory training requirements and education criteria for its employees as set forth by The Joint Commission, Occupational Health and Safety Administration (OHSA), and the policies, procedures, and regulations of the Department of Health and Human Services (DHHS) and the IHS as well as any other required regulation.� The objective is to ensure that FBSU employees are familiar with and informed of the laws and regulations that are applicable to the Indian Health Service and FBSU.� FBSU intends to accomplish this during the onboarding process by using a standardized web based training platform with trainings that can be broken down by department and role, and tailored to FBSU standards.� The Contractor shall be responsible for providing a user friendly learning management system with training libraries so FBSU can delivery training contact, assign courses, create courses, assign courses, monitor employee performance.� The training courses shall meet current Joint Commission and OSHA standards in fields including but not limited to Infection Control, Fire Life Safety, Safety Training, Blood Borne Pathogens, Work Place Ergonomics, and Billing Fraud.
