- THIS IS NOT A SOLICITATION ANNOUNCEMENT. THIS IS A REQUEST FOR INFORMATION ONLY. This Request for Information (RFI) is intended for information and planning purposes only at this time; and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs. Because this is a Request for Information announcement, no evaluation letters and/or results will be issued to the respondents. No solicitation exists. Therefore, do not request a copy of a solicitation. The Department of Veterans Affairs (VA), VISN 4, Network Contracting Office 4 (NCO 4), is seeking information and potential qualified sources capable of meeting the following minimum requirements and salient characteristics below for a brand name or equal to Misonix Bone Scalpel. STATEMENT OF NEED ULTRASONIC BONE SCAPEL A.1. INTRODUCTION The Corporal Michael J. Crescenz VA Medical Center has a requirement for an ultrasonic bone scalpel (Brand Name) for the Ortho Spine Department located at 3900 Woodland Avenue, Philadelphia PA. 19104. The Misonix (NeXus Console) that is delivered to the Medical Center shall be the most current version available out in the marketplace. A.2. BACKGROUND An ultrasonic surgical device that cuts bone and spares soft tissues. This relative selectivity for bone ablation makes Bone Scalpel ideally suited for spine applications where bone must be cut adjacent to dura and neural structures. The VA needs to procure a new Misonix (NeXus) bone scapel and accessories that will enable multiple surgical specialties within Ortho and Neurosurgery to continue performing procedures in-line with the standard of care. The procurement of a Misonix (NeXus) would provide ease of use for both the attendings and residents who operate at the CMC VAMC as the OR currently uses this device and accessories. The current model that is being replaced has reached its end-of-life utilization. Without the new bone scapel the VA would have to utilize Community Care to perform ortho-spine surgeries at a considerable cost to the VA for patient care and possible hospitalization. The bone scapel will be shared among the entire team of ortho-spine surgeons and neurosurgeons. Both the attendings and residents are familiar with the Misonix Bone Scapel, and the OR stocks the needed blades and hooks for the ultrasonic bone scapel. A.3. SCOPE OF WORK VA anticipates a firm-fixed price unit price from this solicitation Contractor shall deliver all items after receipt of award issued by the Government The Medical Center is looking for delivery of the bone scapel with accessories to the Operating Room located at 3900 Woodland Avenue, Philadelphia PA 19104. INSPECTION AND ACCEPTANCE: The warehouse located at 3900 Woodland Avenue, Philadelphia PA 19140 shall receive all equipment prior to Biomedical Engineering inspecting the equipment Disputes shall be resolved by the Contracting Officer C.1. DELIVERY/STORAGE REQUIREMENTS: Deliver materials to job in manufacturers original sealed containers with brand name marked thereon or maintain coverings in place and in good repair if applicable Deliver specified items only when site is ready Store equipment in dry condition inside facility Any government requested delayed delivery up to 90 days after initial delivery date, shall be at no additional cost to the Government PO# must be on outside of shipping box for easy identification Delivery Days are Monday through Friday during normal business hours which are defined as 7:30am to 3:00pm. National Holidays are excluded. Any delivery service performed for emergency orders may be delivered other than normal business work hours and must be approved. The Contractor is not required to provide delivery services on the following National Holidays, nor shall the Contractor be paid for these Holidays and any other declared national holiday by the President. Crescenz VAMC located at 3900 Woodland Avenue, Philadelphia PA 19104 The Contractor shall coordinate all deliveries, staging areas, and parking arrangements with the VA Police and Warehouse personnel. D.1. DELIVERABLES: Operation and Maintenance Manuals Binders (1) each Digital (1) each IF AVAILABLE Deliver compilation of all manufacturers recommended maintenance schedule and operation materials packed in binder(s) The vendor must provide preventive maintenance services, or preventive maintenance training to Users and Biomedical Technicians when appropriate, to meet at minimum Joint Commission standards E.1. OPERATOR TRAINING: Contractor shall provide technical training for Surgeons/Residents/Nurses and Biomedical Engineering. The cost for this training and all training related expenses such as travel, lodging, and other expenses are included in the contract price. This training for Biomedical Engineering shall cover the preventative maintenance, troubleshooting, and general maintenance of the equipment. Training shall allow for classroom instruction if applicable as well as individual hands- on programming, troubleshooting and diagnostic exercises. This training program shall include at a minimum following: Technical Training, Operational Training, and Hardware/Software trouble shooting. Training shall occur onsite during normal business hours, 8:00am 4:30pm, Monday through Friday. Scheduling of all training shall be coordinated with the Corporal Michael J. Crescenz VA Medical Center after delivery is complete F.1. ASSEMBLY: All employees of the general contractor and subcontractors shall comply with CVAMC policies including but not limited to vendor credentialing, parking, check-in procedures, name badges, smoking policies, and confidentiality. H.1. POINT OF CONTACT: The POC for this project is Laurie Rentz COR, Supervisory Program Analyst for Surgical Services The information identified above is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications mentioned above. Responses to this RFI should include company name, address, point of contact, phone number, and point of contact e-mail, DUNS Number, Cage Code, size of business pursuant to North American Industrial Classification System (NAICS) 339112 (size standard of 1000 employees). (4) If you are a large business, do you have any designated distributors? If so, please provide their company name, telephone, point of Contact and size status (if available). (5) If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? (6) If you intend to subcontract any work on this contract, what portion of the total cost will be self-performed/will be performed by your organization? Please provide estimated detailed percentage breakdowns related to sub contracted work and completion of job. This notice will help the VA in determining available potential sources only. Do not contact VA Medical Center staff regarding this requirement, as they are not authorized to discuss this matter related to this procurement action. All firms responding to this Request for Information are advised that their response is not a request for proposal, therefore will not be considered for a contract award. If a solicitation is issued, information will be posted on the FedBizOpps (FBO) web site for all qualified interested parties at a later date and interested parties must respond to the solicitation to be considered for award. This notice does not commit the government to contract for any supplies or services. The government will not pay for any information or administrative cost incurred in response to this Request for Information. Information will only be accepted in writing by e-mail to Contracting Specialist at lashawn.knight@va.gov.
