Combined Synopsis/Solicitation Notice Combined Synopsis/Solicitation Notice Page 3 of 3 Combined Synopsis/Solicitation Notice *= Required Field Combined Synopsis/Solicitation Notice Page 1 of 3 DESCRIPTION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested, and a written solicitation document will not be issued. This solicitation is a request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2022-04 01/30/2022. The associated North American Industrial Classification System (NAICS) code for this procurement is 811198, with a small business size standard of 8 million. VISN 16 requires refurbishment of mobile trailer. The contractor shall provide all labor, transportation, parts, testing equipment, tools, and expertise necessary to refurbish Emergency Support trailer located at 2200 Fort Roots Drives, Building I, Little Rock, AR 72114 in accordance with the Statement of Work (SOW). Period of Performance (POP) Approximately 90 days after receipt of order. All interested companies shall provide quotation(s), for the following: STATEMENT OF WORK Service Contract Title: Telehealth Trailer Refurbishment General: The contractor will furnish all management, supervision, labor, materials, transportation, supplies, parts, test equipment, tools, and expertise necessary to assess and refurbish the telehealth trailer located at North Little Rock VAMC. All work will be performed at the North Little Rock VAMC. Period of Performance approximately 90 days from date of award. Work to be performed in phases as to make the unit capable of being deployed if required in a short period of time. Performance Location Address: 2200 Fort Roots Drive, Building I Little Rock, AR 72114 POC Bill Catton 501-412-8636 Scope. The scope of this service contract shall apply to the Telehealth Trailer refurbished items listed in Table below: Description One (1) Each Per Task Remove existing interior walls, existing cabinets, counter tops materials mounted on walls, electrical outlets and switches and replace with new walls, cabinets, counter tops, electrical outlets, and switches. Repair water damage behind outer walls and treat any water damage areas with Decon. Remove existing interior doors and cover with antibacterial type materials. Reinstall doors. Seal around exterior windows and doors. Replace damaged insulation in walls. Cover walls with antibacterial type materials. Seal all seams and seal walls at floor. Install new upper aluminum cabinets to replace the old cabinets. Install new lower aluminum cabinets to replace old cabinets. Install new counter tops with antibacterial type materials to replace old counters. Reinstall sinks and fixtures. Reinstall any fixtures that was removed from walls, reinstall electric outlets and switches. Contractor will supply all new materials to be installed including adhesive, fasteners, cabinets, aluminum, etc. All work will be performed at the VA warehouse in North Little Rock. Conformance Standards: Contract service shall ensure that the equipment/system functions in conformance with the latest published edition of NFPA-99, OSHA, manufacturer s Network Equipment/System. Upgrades/updates shall be installed in strict accordance with manufacturer s specifications and must operate within manufacturer s specifications and must operate within manufacturer s specified parameters. Hours of Coverage: All scheduled service/repairs shall be performed during normal business hours. Normal business hours are (8:00 AM to 5:00 PM) (CST) Monday Friday. EVALUATION PROCEDURES AND AWARD Evaluation and award will be conducted in accordance with the requirements of FAR 13.106-2(b)(3), the quotes will undergo a comparative evaluation to determine which contractor provides the best value to the government in terms of technical capability, while also providing a competitive price. For quotes to be considered acceptable and eligible for evaluation, they must be prepared in accordance with and in compliance with the instructions given in this solicitation and address all the requirements set forth within this solicitation. The Government does not anticipate requesting revisions to quotes, so Quoter s should provide their best quote in the response to this solicitation. However, the Government reserves the right to request quote revisions that seek additional price discounts, provide revisions to the technical and or prices aspects of the quote, or to clarify aspects of the quote. In the event quote revisions are requested, the Government will not necessarily be seeking revisions from all Quoter s. The Government will not be conducting ""discussions"" as defined in FAR Part 15. Prospective Contractors are required to submit two separate evaluation documents in addition to all other documents required by this solicitation. If the parts are not submitted as separate documents, the quote will be considered non-responsive. Technical Acceptability will be based upon the conformance to the guidelines outlined in this solicitation. Quotes will be evaluated on a GO, NO-GO basis. The technical evaluation will evaluate the technical content received as required in the instructions of this solicitation. This is a subjective evaluation that is based upon the Governments judgment as to whether the Quoter has satisfactorily addressed the technical content requirements as set forth within this solicitation. One (1) Part B Price and price related factors. 1. Price - The contractor shall provide a separate and complete price schedule as provided in the solicitation including all offered discounts. 2. A Quoter s price quote will be evaluated for reasonableness. For a price to be reasonable, it must represent a price to the Government that a prudent person would pay when consideration is given to prices in the market. Normally, price reasonableness is established through adequate price competition, but may also be determined through price analysis techniques as described in FAR 13.106-3. 3.
