- COMBINED SYNOPSIS / SOLICITATION Mountain Home AFB (MHAFB), ID, 83648 Bldg. 1788 Mold Remediation & Repair of Storm Damage *** PLEASE SEE ATTACHEMENTS, TO INCLUDE FORMATTED VERSION OF THIS NOTICE*** In accordance with FAR 12.603(c)(2): This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F3F3CE2038A001 FY22 Mold Remediation & Repair Storm Damage Bldg. 1788, is being issued as a Request for Quotation (RFQ). This solicitation document, which incorporates provisions and clauses for commercial items, are those in effect through the current Federal Acquisition Circular in effect at time of posting this notice. This procurement is a 100% Small Business set-aside in accordance with (IAW) FAR 19.502-2(a). The Government intends to award a Firm Fixed Price Contract based on Lowest Price Technically Acceptable. The associated NAICS code for this acquisition is 562910 with a size standard of $22M Dollars. USAF Mold Policy SITE VISIT: A site visit has been scheduled for Thrusday, 28 April 2022 at 0930 AM (MST � Local Time). All interested parties who would like to attend All potentially interested vendors need to be at Mountain Home AFB Visitor�s Center No Later Than 09:30AM in order to be escorted/ receive a visitor�s pass to gain entry onto the installation. Attendees will be able to drive onto the installation to attend the site visit. Please bring required Identification, Driver�s License, vehicle insurance and registration. Also Please respond to the identified Point of Contact(s) and provide a list of attendees Name No Later Later Than 3 days prior to the Site Visit date. All Prime Contractors will be limited to a total of two (2) personnel per company for the site visit. The following provisions and clauses in their latest editions apply to this acquisition; offerors must comply with all instructions contained therein: See Attached Provisions and Clauses. ���������������������������������������������� 5352.201-9101 ACC Ombudsman Oct 2019 (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions).
