- Under Statutory Authority 41 U.S.C. 1901 the VA Great Lakes Acquisition Center (GLAC) intends to negotiate a sole source contract with ALCON LABORATORIES, INC. for the repair the Alcon ORA Interoperative Aberrometer System. ALCON LABORATORIES, INC. is the only authorized vendor for Alcon ORA Interoperative Aberrometer System. This acquisition is conducted under the authority of 41 U.S.C. 1901 Simplified acquisition procedures as implemented in FAR 13.106-3(b)(3)(i), only one responsible source and no other supplies or services will satisfy agency requirements. This will be a firm fixed price contract for a One Time service contract. The NAICS Code is 811219. This Notice of Intent is not a request for competitive quotes. No solicitation documents are available and telephone requests will not be honored. However, if a firm believes it can meet the requirements it must furnish information about its products and services, as well as references from other customers who are using these products and service to the Contracting Officer by 10:00AM (Local time; Wisconsin) 04/27/2022. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Officer will proceed with the sole source negotiation with the ALCON LABORATORIES, INC. Capability statements shall be submitted by email only as a MS Word or Adobe PDF attachment to Della Bond at della.bond@va.gov. TELEPHONIC INQUIRIES WILL NOT BE ACCEPTED. STATEMENT OF WORK FOR ALCON REPAIR 1. The contractor shall provide all labor, material, tools, travel, management, and equipment necessary to provide services such as upgrades, repairs, preventative maintenance and/or replacement equipment for the Alcon ORA Interoperative Aberrometer System at the Milo C. Huempfner Veterans Administration Community Based Outpatient Clinic (VA CBOC) located at 2851 University Ave, Green Bay, WI 54311. This procurement is a One-time service. The performance of this service contract shall be for a period not less than thirty (30) days and shall not exceed 60 days from the date of acceptance. 2. Background The Milo C. Huempfner VA CBOC is requesting a service contract to repair the Alcon ORA Interoperative Aberrometer System. The Alcon ORA interoperative aberrometer system is used as a technological aid in performing 5-10 cataract surgeries per week at the VA CBOC. Currently, the system does not pass the start-up verification self-test and cannot be used for patient care. The equipment is currently inoperable and the VA CBOC has lost the ability to perform continuous assessment of the patient s eye throughout cataract surgery procedures and must rely on physician s manual calculation. 3. Objectives This service contract shall be utilized for performing repairs of malfunctioning equipment or parts. The services and personnel shall be provided by the contractor that has specifically trained field support or technical support representatives for all repair, updates, and upgrades of the Alcon ORA interoperative aberrometer system. The parts required for the repair of this equipment are specifically designed and are not substitutable. The trained field support representatives provided by the company shall minimize the potential of repair delays caused by misdiagnosis of equipment issues and incorrect parts being ordered. The minimization of delays of veteran care due to equipment inoperability shall be limited to the time period required for the appropriate acquisition for repair. 4. Scope a) The Contractor shall provide on-site troubleshooting of the verification failed reference error currently being displayed on the Alcon ORA system. b) If determined necessary, the contractor shall provide and configure a new aberrometer, part # 8065998328R, part description ASSY, ORA VPLUS AB, 190.5 REPRO. 5. Equipment Identification Alcon ORA S/N: 2001245101X EE#: 115962 6. Scheduled Maintenance Work Scheduled maintenance work shall be required in the performance of this contract and shall be performed during the hospital s normal business hours of 8:00 a.m. and 5:00 p.m., local time (Wisconsin), excluding Federal Holidays, to include any other day specifically declared by the President of the United States to be a Federal Holiday. The contractor shall notify the COR at least 7 calendar days prior to the normal maintenance visits and 8 hours prior to any emergency activities. The Milo C. Huempfner VA CBOC may consider allowing work outside of the above-listed hours of operation at the convenience of the Government. Federal Holidays observed by the Milo C. Huempfner VA CBOC are: New Years Day Labor Day Martin Luther King, Jr. Birth Day Columbus Day Washington s Birthday Inauguration Day Juneteenth National Independence Day Labor Day Independence Day Veterans Day Memorial Day Thanksgiving Day Christmas Day Columbus Day When one of the above designated Federal Holidays falls on a Sunday, the following Monday shall be observed as a Federal Holiday. When a Federal Holiday falls on a Saturday, the preceding Friday shall be observed as a Federal Holiday by United States Government agencies. If different times shall be needed, this shall be discussed and mutually agreed upon with the COR and shall still be considered normal business hours. If work must be performed outside of mutually agreed normal business hours ( over-time work), the Contractor must obtain prior approval at least 24 hours from the COR. If such work shall be at the cost of the Milo C. Huempfner VA CBOC, a separate purchase order number must be obtained by the Contractor prior to performing any over-time work. Work shall be scheduled in such a manner as to allow as little disruption to patient services in all campus buildings affected. On completion of billable work, the contractor shall provide timely billing of items that Milo C. Huempfner VA CBOC is responsible per the service agreement. 7. Documentation Upon completion of services, the Contractor shall provide the following information in a written service report to the VA Green Bay CBOC Biomedical Engineer: a. The date of service. b. The model. c. The serial number. d. The location (building and room number) of the equipment services. e. The name of the Field Service Engineer (FSE) providing services. f. The services performed and any parts replaced or repaired. This report shall be signed by both the contractor performing services and the VA Biomedical Engineer. Either a hard copy or an emailed copy of the service report shall be supplied within 48 hours of completion to the VA Biomedical Engineer.
